1 2017 February Newsletter Reasonable Accommodation at Workplace CDE makes every effort to ensure its work sites are free from physical and communication barriers, including making formal reasonable accommodations for employees with disabilities or impairments, for example, making a modification or adjustment to the work environment or circumstances to enable the employee to do the essential functions of the job. The interactive process for providing effective reasonable accommodation is an obligation that requires CSD and employee to communicate frequently and openly to determine an effective accommodation. The process starts with a request for accommodation by an employee with a disability or impairment or the supervisor s recognition of the need for such an accommodation for the employee. The employee can obtain Reasonable Accommodation (RA) Request form (OEO1200A) under Benefit forms on HR Forms/Information tab on HR Intranet. The employee needs to complete (a) the first page of RA form with (b) the request form Medical Information form (OEO1200B) and (c) the physician s written response to section B of the form. When completed, the employee can submit all completed paperwork to the immediate supervisor. The 10 working days begins on the date the completed request is received by the supervisor. The need for reasonable accommodation is determined on a case-by-case basis. The policy will be posted on the HR intranet under the Training tab.
2 Spanish Translation Request Reminder We ask you to use the online request on HR Intranet to allow our Spanish translators to able to fulfill your request on time. They are still getting hard copies through campus mail that can delay their ability to get your request done in timely manner. If the material contains information that is handwritten, you may send through campus mail otherwise anything that is done on the computer, should be sent using the online request. Workers Compensation paperwork It is imperative for the employees and supervisors to submit the Workers Compensation Form 3301, Employer s Report of Occupational Injury or Illness Form 3067 and the report of minor injury to LeeAnn Dreffs, APA within 5 working days. 3 Teaching & Services Credential Holders Do you know that Commission s CTC Online system allows teachers to renew the following types of documents online? Clear Credentials Level II Credentials Emergency 30-Day Substitute Permit After online renewal, the document should be available in a view and print format on CTC website within 10 working days (excluding the holidays). However, Preliminary credentials are not renewable. These documents need to be cleared with the program sponsors to be cleared in order to be reissued. Information on clearing your preliminary document can be found on the Clear Your Credential section of CTC website. To view your current document credential or find the status of your application, go to and click credentialing information button that will take you to Educator Login menu shown on right side where you can log in to view your profile information. Click the Next button at the bottom right-hand corner of the screen.
4 You will be directed to a page with three tabs towards the top of page (underneath your profile information showing your name). Click on the Application tab to view any pending applications.
5 You may sort the display view by clicking the down arrow ( ) in the "Status" column to sort the list with your most recent application on top. Clicking the arrow ( ) to the left of your application to display the details of a credential application.
