It s important to note that you have 31 days from your hire date to enroll in medical, dental, and vision plans. This enrollment period is the only time you can enroll in benefits for 2017, unless you have a qualifying status change during the year. 6 DEPENDENT ELIGIBILITY INFORMATION Who s An Eligible Dependent? Many plans offer you the option to cover additional members of your family. In insurance terms, these family members are called dependents. Here s what you need to know about who s eligible for Darden benefits. ELIGIBLE DEPENDENTS: Child (covered up to age 26) Spouse A biological child A legally adopted child A stepchild A foster child A child for whom you are legal guardian, including grandchildren Disabled child must have been disabled prior to age 19 and must be physically or mentally handicapped, and incapable of self-support Children covered by a court order (QMCSO) Legally married spouse Common-law spouse (if your state recognizes common-law marriages) 4

7 ENROLL NOW: MAJOR MEDICAL Expressly For You: Your Major Medical Options ENROLL NOW The Affordable Care Act (ACA) requires that every U.S. resident have Major Medical insurance coverage. Those who go without medical coverage risk financial penalties and tax consequences. Fortunately, there are plenty of choices for Major Medical coverage, including plans offered by state and federal marketplaces as well as Darden and other employers. Carefully consider your available Major Medical options: 1. DARDEN MAJOR MEDICAL PLANS You can choose from 5 different Major Medical plan levels offered by multiple insurance carriers. Not all insurance carriers are available nationwide, but all the plans generally offer the same basic coverage from carrier to carrier. 8 ENROLL NOW: MAJOR MEDICAL 2. GOVERNMENT MARKETPLACE The government marketplace offers multiple insurance choices from different insurance carriers. Darden s State Medical 411, powered by Stride Health, helps you navigate your options to find the right plan for your budget. You can easily check to see if you qualify for a government subsidy to save money on your monthly premium cost. Individuals who qualify for a subsidy save over $300 per month on average! Find your plan at 3. A PLAN FROM A NON-DARDEN SOURCE Perhaps there s another plan that s a better package for you, like one belonging to a spouse or parent or a plan through another employer or school. Check out these alternatives to learn more about options and enrollment dates. 4. NO MAJOR MEDICAL COVERAGE The choice is yours, but going without ACA-approved Major Medical coverage could subject you to a government tax penalty. 100% COVERAGE for things that help you stay healthy: Preventive care* is covered 100% by all plans. *Includes annual checkups, annual mammograms, well-woman and well-child checkups. TIP: When selecting medical coverage, be sure to take advantage of the HELP ME CHOOSE tool, available during the enrollment process. It s easy to use, and it can be a big help in deciding which medical plan is right for you. TIP: Choosing a Bronze, Bronze Plus or Silver Plan? Consider contributing to a HEALTH SAVINGS ACCOUNT (HSA)! An HSA is an easy way to save and pay for healthcare expenses like prescriptions, copays and other costs, and you ll enjoy a triple tax advantage. Read more in the HSA and FSA section. You MUST enroll for your Darden Major Medical coverage within 31 days of your hire date or go without coverage until Open Enrollment in the fall. 6

9 ENROLL NOW: HSA AND FSAs Make Savings Part of the Package ENROLL NOW HEALTH SAVINGS ACCOUNT (HSA) If you enroll in a Darden Bronze, Bronze Plus or Silver Major Medical plan, an HSA lets you set aside pre-tax dollars for eligible out-of-pocket medical expenses like: Deductibles Medical treatment before your deductible is met Copays Over-the-counter medicines (with a doctor s prescription) Non-covered medical and healthcare supplies and dental treatment And much more! Prescriptions Your HSA funds are accessed with a debit card. Any HSA funds you don t access roll over each year and grow automatically. Your HSA account is portable: if you leave Darden, you can take it with you! By lowering your annual taxable income, an HSA may reduce your taxes. Also, eligible HSA withdrawals are non-taxable. You can enroll or change your contributions any time during the year as long as you remain enrolled in your health plan. You can contribute up to $3,400 for employee-only coverage and $6,750 for all other coverage levels. If you are age 55 or over, you can make additional catch-up contributions of up to $1,000 per year. HEALTH CARE AND DAY CARE FLEXIBLE SPENDING ACCOUNTS (FSAs) FSAs let you set aside money before taxes to pay for eligible out-of-pocket expenses. Darden offers two types of FSAs: 1) HEALTH CARE FLEXIBLE SPENDING ACCOUNT (FSA) lets you contribute between $200 and $2,550 per year for eligible health-related expenses like prescriptions, therapy, etc. Employees are able to roll over $500 at the end of the plan year. Note: If you enroll in both the Health Savings Account and the Health Care FSA, your FSA will be a limited-use FSA. See My Total Rewards > Benefits tab for plan details. 2) DAY CARE FLEXIBLE SPENDING ACCOUNT (FSA) FOR CHILDREN & ELDERS lets you contribute between $200 and $5,000 (before taxes) per year and helps cover the costs of: Child care services for eligible dependent children under age 13 Services to care for other qualified dependent family members (e.g., elder care) Unspent FSA funds do not roll over at the end of the plan year. ENROLL NOW You MUST enroll for FSA accounts within 31 days of your hire date unless you have a qualifying life event. You can enroll in an HSA anytime as long as you are enrolled in a Bronze, Bronze Plus or Silver Plan. 7

10 ENROLL NOW: OTHER MEDICAL OPTIONS Other Medical Options The following programs are NOT ACA-approved Major Medical insurance options. They are programs designed to meet special coverage needs. 1 ST DAY CHOICE (through Aetna)* This is an affordable plan that helps you access routine care and manage costs. This plan reimburses you from the first dollar you spend for services such as office visits, prescription drugs and hospital stays. BENEFIT BUNDLE (through New Benefits)* This low-cost option helps you take care of basic/routine health needs for just $1.24/week. 24/7 Medical Help through Teladoc: speak with a medical professional over the phone for non-critical healthcare needs. Discounts for Retail & Urgent Care Clinics. Discounts on Prescriptions with your Prescription Discount Card. *Not available in HI, VT and WA. ACCIDENT COVERAGE (through Aflac) Did you know that total treatment for a broken leg can cost more than $20,000? Accident Coverage works as a medical-cost safety net, paying cash benefits of up to 25% of the total treatment and related costs associated with accident injuries. Covered injuries include lacerations, burns, sprains, dislocations, fractures and paralysis. Benefits are often paid within one day of the claim, so the money is available fast. CRITICAL ILLNESS (through Aflac) Critical Illness coverage helps protect you and your family from the sometimes devastating costs of a serious illness or condition such as a heart attack, stroke or sudden illness. Combine it with other medical coverage or use it on its own to pay costs not covered by your Major Medical coverage. It pays cash benefits, often within one day of your claim for treatment expenses. Aflac s plan gives you two payment options for your Critical Illness expenses: A lump-sum payment of up to $20,000 paid directly to you. Treatment-based benefits that pay over time. 11 ENROLL NOW: DENTAL Dental Coverage: Bronze, Silver, Gold and Platinum Levels Dental plans are offered by multiple insurance carriers at Bronze, Silver, Gold, and in limited areas, Platinum levels. Note that the Bronze-level plan does not cover major dental services and orthodontic treatment. 12 ENROLL NOW: VISION Take a Look: Your Vision Options As with Major Medical and Dental, these plans are offered by different insurance carriers depending on where you live and work, but the plans coverage is generally the same. Use the Find Your Provider tool to see if your provider is in a network before enrolling. There are financial advantages to choosing eye care providers inside your plan s network. 13 ENROLL NOW: OTHER BENEFITS Now Delivering: Additional Benefits FOR MORE DETAILS: Visit My Total Rewards or call the Total Rewards Service Center at SHORT-TERM DISABILITY You are automatically enrolled in this company-provided benefit. Coverage generally applies to non-work-related sicknesses or injuries such as maternity, surgery and hospitalization when you are disabled for more than 7 days. Upon approval of your disability, you will be paid up to 90 days of full pay or a combination of full and 2 / 3 pay depending on your length of service. Restrictions and exclusions apply. See My Total Rewards > Benefits tab on krowd for more details. LONG-TERM DISABILITY Long-Term Disability (LTD) provides a monthly income if you are totally disabled. Payments will start after you have been disabled for 90 days. If enrolled, you will generally receive 66.67% of your eligible monthly pay. Restrictions and exclusions apply. LIFE INSURANCE You are automatically enrolled in company-provided Basic Term Life Insurance coverage of one or four times your annual salary. You can purchase optional Supplemental Coverage up to two or five times your annual salary for maximum coverage of six times your annual salary. The maximum benefit for Life Insurance, between company-provided and supplemental insurance, is $1.5 million. For complete plan details and costs, go to krowd > My Total Rewards > Benefits tab. You can also purchase Dependent Life Insurance for your eligible spouse and child(ren). The cost of coverage is $0.63 per week. Dependent Life Insurance pays you a lump sum amount if your covered spouse/child dies. Children are covered up to age 23. Eligible Spouse: $25,000 Eligible Child: $7,500 11

A storm blows through and a Team Member needs temporary housing; a threat arises and children need to be moved to a safe location. Whether you need help or want to help, Darden Dimes makes a real difference in our Team Members lives. COMMUTER BENEFITS PROGRAM A commuter program that allows you to save money on mass transit costs to and from work while you are actively employed by purchasing transit/vanpool passes with pretax dollars directly from your pay. You can deduct up to $255 per month on a pretax basis for commuter mass transit such as bus, commuter rail, ferry, train, light rail or vanpool. SAVE FOR YOUR FUTURE ENROLL ANYTIME AFTER ELIGIBILITY REQUIREMENTS ARE MET: DARDEN SAVINGS PLAN 401(k) If you re at least 21 years of age, you can contribute up to 25% of your pay on a before- or after-tax basis. DARDEN SAVINGS PLAN 401(k) COMPANY MATCH Darden will match up to 6% of your contributions after one year of service and 1,000 hours. This match can range from 25% to 120% depending on company performance (5-year vesting schedule). ESPP (EMPLOYEE STOCK PURCHASE PLAN) Team Members with one year of service averaging more than 20 hours per week during the plan evaluation period can take advantage of a 15% discounted purchase of Darden stock. 17 IMPORTANT NOTICES Below is a summary of important rights you have under Darden s health plans. (These notices do not apply to voluntary benefits, such as 1 st Day Choice and Benefit Bundle.) More detailed notices describing these rights are available upon request. If you have questions or would like a paper copy of any notice, contact the Total Rewards Service Center. HIPAA Special Enrollment: If you waive coverage under the Darden medical plan for yourself or your dependent(s) during Open Enrollment because of coverage under another plan, you can enroll in Darden medical plan coverage if you or your dependent(s) lose eligibility for that other coverage (or if the employer stops contributing toward the cost of the other coverage). If you obtain new dependents as a result of marriage, establishment of a new domestic partnership, birth, adoption, or placement for adoption, you can enroll yourself and your dependent(s) in the Darden medical plan. You must contact Total Rewards to request special enrollment within 31 days of the loss of other coverage or marriage/domestic partnership and within 60 days of birth, adoption, or placement for adoption. CHIPRA Special Enrollment: If you are eligible for health coverage from Darden but are unable to afford the premiums, your State may have a premium assistance program that can help you pay for coverage, using funds from Medicaid or children s health insurance programs (CHIP). Contact your State Medicaid or CHIP office to find out if premium assistance is available. If you or your dependent(s) become eligible for premium assistance under Medicaid or CHIP, Darden is required to permit you and your dependent(s) to enroll in medical plan coverage (if you are eligible). You must request this special enrollment opportunity within 60 days of being determined eligible for premium assistance. Notice of Privacy Practices: Darden maintains a Notice of Privacy Practices that provides information to individuals whose protected health information (PHI) will be used or maintained by Darden s health plans. The Notice of Privacy Practices describes how the plans protect PHI, how it will be used, and your rights with respect to your own PHI. The notice is available on the Total Rewards website, or you can contact the Total Rewards Service Center to request a paper copy. Newborns and Mothers Health Protection Act: Darden s medical plan will not restrict benefits, and will not require authorization from the plan or the insurance issuer, for any childbirth-related hospital stay not in excess of 48 hours following a normal delivery or 96 hours following a cesarean section. Women s Health and Cancer Rights Act: Darden s medical plan covers mastectomy-related services, including all stages of reconstruction and surgery to achieve symmetry between the breasts, prostheses, and complications resulting from a mastectomy (including lymphedema). Normal coinsurance/copayments may apply. Medicare Part D Creditable Coverage: Medicare Part D prescription drug coverage is available to everyone with Medicare. Darden has determined that the prescription drug coverage provided through Darden s medical plan is, on average for all plan participants, expected to pay out as much as standard Medicare prescription drug coverage pays and is therefore considered creditable coverage. If you or a dependent are Medicare-eligible, you should compare your available Darden coverage, including which drugs are covered at what cost, with the coverage and costs of the Part D plans offering Medicare prescription drug coverage in your area.
