2 Do I need to do anything?
3 Re-enroll if you need to Add, cancel, or change your health, dental, or vision plan. Add or remove a spouse, sponsored dependent, or dependent child from any of your plans. Contribute to a Flexible Spending Account. Add or change the amount of life insurance or AD&D coverage you have. Enroll in Short Term Disability - or if you are currently enrolled in Short Term Disability AND your pay rate has changed. 3
4 Don t Forget to Roll Over FSA $ You are able roll over up to $500 from your Health Care FSA left unused to the following plan year. You have until June 30, 2015 to incur claims for FY and you can file claims through 12/31/15. Any remaining funds, up to $500, from plan year, may be rolled over and be available in the plan year. Any remaining funds as of June 30, 2016, up to $500 may be rolled into the plan year. 4

6 Health Premiums Remain Below Average UK will absorb the majority of the cost increase seen in premiums. The University of Kentucky is contributing an additional $1.3M to cover the increased cost for FY Health plan premiums will increase 1.2% overall and employees who receive the health credit will see a $1-$5 increase per month for UK-HMO, UK-PPO and UK-RHP plans. UK EPO premiums will increase between $3-$9 per month. 6
7 RHP Expands Provider Network Provider regions have been expanded, increasing your options when choosing a doctor. RHP provider network in Region A includes all the eligible counties in the eastern half of the state. Region B will include all RHP eligible counties in the western half of the state (includes Louisville area providers). 7
8 What will stay the same? RHP Provider Networks A & B Expanded 8 Both Region A and Region B will continue to have access to UK Healthcare facilities including Chandler and Good Samaritan Hospitals.

20 Flexible Spending Account Prepaid Card Cards will be mailed to you automatically from ConnectYourCare if you are a new participant. Quick access to health care flexible spending funds. Present the card as a credit card for doctor office copayments, prescription and other eligible expenses. Keep your receipts some purchases require follow-up (KY Clinic Pharmacy, some medical and dental). If you purchase prescriptions using your Express Scripts coverage, the expenses will be automatically processed. 20
21 Shared Leave Staff shared leave eligibility (0.5 FTE or greater) Benefits employees who exhaust accrued paid leave to care for a spouse, sponsored-dependent or child. 22 Financial Counseling Financial Counseling is a free voluntary counseling and referral service for UK employees (regular,.5 FTE or greater). Intended to help employees gain control of their finances. Up to four sessions are available per fiscal year. Sessions generally last approximately 1 hour. Appointments may be scheduled Monday through Friday by calling Mike Pelfrey at
23 CRISIS 23 This program provides temporary financial assistance in the form of a one-time payment of up to five hundred dollars to University of Kentucky staff members who are experiencing personal hardship.
