1 ADDENDUM #2 (Issued Thursday, June 27, 2013) WAKULLA COUNTY BOARD OF COUNTY COMMISSIONERS RFP Title: Debris Removal and Disposal RFP Number: # Addendum #2 is issued to amend RFP, Section 7.0 Proposal Response Requirements by adding Tab 8 W/MBE Participation Requirements, as follows: TAB 8 W/MBE Participation Requirements W/MBE participation is important to the Wakulla County and will be reflected in the evaluation of Proposal submitted in response to this RFP. A. It is the goal of this RFP to have at least a 10 percent certified W/MBE participation per contract period. The Proposer will include with its proposal a summary of how it plans to include and monitor certified W/MBE participation for this project per contract year, if activated. B. Provide a list of the locally certified W/MBE businesses that will be utilized and the services they will provide under this RFP. Note Appendix C-8 if any proposed sub-contractors are on this list. C. Wakulla County does not certify W/MBE s but recognizes W/MBEs certified by the City of Tallahassee, Leon County, State of Florida Office of Supplier Diversity and FDOT BDE program. If no suitable W/MBE businesses can be found or if NONE will be used in the provision of services in the RFP and its addendums, the Proposer shall document such using Attachment #2: Statement of Good Faith Effort and placed behind Tab 8. W/MBE Participation Requirements. A score of 80% of the eligible points on the Statement of Good Faith Effort is required to achieve an acceptable good faith effort determination, which shall be equal to 5 points in terms of the W/MBE evaluation score. E. As part of the evaluation process, a total of 10 points shall be awarded for MBE participation: 5 points for the MBE Plan and 5 points for utilizing certified W/MBE businesses. Failure to submit a plan will result in the Proposer not receiving the full 10 points toward the evaluation score. F. Refer to Wakulla County s W/MBE Statement of Use, see Attachment #2.

3 ATTACHMENT #1: STATEMENT OF GOOD FAITH EFFORT (SGFE) RFP , Debris Removal and Processing Proposer Name: Date: This form is to be completed if the Proposer fails to achieve the W/MBE goals established for project. The Proposer is allowed to use an alternate method that demonstrates the good faith efforts made to achieve the goals established as long as all of the requested information is included. Failure to include all requested information may result in the Proposal being determined as non-responsive to the W/MBE goals. The following list is not intended to be exclusive or exhaustive and the County may look not only at the different kinds of efforts the Proposer has made, but also the quality, quantity, intensity and timeliness of those efforts; it is the responsibility of the Proposer to exercise good faith efforts. Any act or omission on the County shall not relieve the Proposer of this responsibility. Criteria listed below are consistent with RFP , Debris Removal and Processing and any and all Addendums issued: 1. Attendance at Pre-Proposal Conference, if held: Yes No Not Held 2. Proposer provided written notice to certified W/MBE s listed in the directories of those entities recognized by Wakulla County in its W/MBE Statement of Use, as acceptable certified W/MBE s, that perform the type of work to be subcontracted and advising the W/MBE(s) of the specific work the Proposer intends to subcontract; that their interest in the Contract is being solicited; how to obtain information for the review and inspection of contract plans and specifications; and statement that all subcontractors would be held to the same standard terms and conditions as the Proposer. List any assistance provided to interested W/MBE s, with name of firm and the type of assistance provided (e.g., reviewing the RFP, Scope of Work, Bonding, lines of credit, insurance, etc.) (25 points) 7. Describe any additional efforts or circumstances, which may assist the County in determining Proposer s Good Faith Efforts. (5 points) A MINIMUM SCORE EQUAL TO 80% OF THE ELIGIBLE POINTS IS REQUIRED TO ACHIEVE AN ACCEPTABLE GOOD FAITH EFFORT DETERMINATION, WHICH WILL BE EQUAL TO 5 POINTS UNDER THE SCORE OF THE W/MBE PARTICATION.
