7 Eligibility criteria For Deceased Insured person - Insured person must be should have been an employee who is or was payable under the act and entitled to any of the benefit given by act. - If the person is not present in the insurable employment but in periodical payment of PDB. - The person entitled to Medical benefit on the date of his death. - Insured person whose medical benefits are ceased and is not a PDB recipient cannot avail FE benefits. For claimants - Claimants should be the eldest surviving member of the family. - If the claimants is not the surviving member of the family then provide the documents that the deceased insured person did not have a family or he did not live with his family at the time of his death. - if the claimant is a minor the claim can be submitted by his/her guardian Indicating the relationship to the claimant. 16 Claim Creation How to create Claim The claim for FE should be submitted by claimant entitled to receive the same under the ESI act, 1948, to the appropriate branch office by post or by form 25-A supported by documents proving the death of Insured person (Death certificate) Branch office staff will enter the IP no. And the benefit claim category as Funeral Benefit to generate claim online NOTE : If the IP no. is invalid or record is not found in the database, the applicant need to prompt the message to enter the correct IP no. The corresponding claim form is displayed and the data from the claim form submitted by the claimant need to be captured onto the online claim form along with a scan copy of claimant declaration need to be uploaded.
