Project # Date of Issuance: March 12, Date of Substantial Completion: March 12, Contractor: Kirk Bros. Co., Inc. - Engineer: Glaus, Pyle, Schomer, Burns & DeHaven, Inc. (Copied to page 126). A motion was made by Mr. Chmiel and seconded by Mr. Adkins approving the hiring of the following three (3) employees, as presented by Bob Gallagher, DJFS Director: * Christy Denney, Social Program Coordinator - Pay rate: $20.47/ hour - Start date: April 6, 2015 * Tracy Friel, Fiscal Supervisor - Pay rate: $25.26 / hour - Start date: March 17, 2015 (from Interim to Fiscal Supervisor ) * Kelly Peacock, Income Maintenance Worker 3 - Pay rate: $18.46 / hour - Start date: March 23, 2015 ( new appointment) A motion was made by Mr. Chmiel and seconded by Mr. Adkins approving the following contracts for Title IV D Child Support Services, as recommended by Bob Gallagher, DJFS Director: Athens County Prosecuting Attorney - Total value: $60,561.01; contract period: 01/01/2015 through 12/31/2015; program to be charged: Operations, CSEA. * Athens County Court of Common Pleas, Domestic Relations Division - Total value: $231,634.39; contract period: 01/01/2015 through 12/31/2015; program to be charged: Operations, CSEA. * Athens County Court of Common Pleas, Juvenile Division - Total value: $68,853.71; contract period: 01/01/2015 through 12/31/2015; program to be charged: Administrative Cost Pool, CSEA. 4 C O M M I S S I O N E R S J O U R N A L 101 A T H E N S C O U N T Y, REGULAR SESSION March 129 A motion was made by Mr. Chmiel and seconded by Mr. Adkins authorizing the signature of Commissioner Eliason on the Guardian Ad Litem Agreement where, Alyssa Nicol agrees to accept appointments by the Juvenile Division of the Athens County Court of Common Pleas to serve as a guardian ad litem for the Court when available to do so. The Athens County Board of Commissioners agrees to extend the General Liability coverage provided by CORSA to Athens County to cover Alyssa Nicol while performing services for, and while acting within the scope of authority granted by, the Juvenile Division of the Athens County Court of Common Pleas. A motion was made by Mr. Chmiel and seconded by Mr. Adkins approving the Athens County Dog Shelter agreement release of liability for volunteers, as recommended by Jeff Koons, Athens County Dog Warden. Signed volunteer forms kept on file at the Dog Shelter. A motion was made by Mr. Adkins and seconded by Mr. Chmiel to ratify Commissioner Eliason s signature on the VOCA and SVAA Grant Award and Acceptance Form as requested by the Athens County Sheriff s Office. Grant Period 10/01/ /30/2015 Award Number 2015-VOCA VOCA Award: $ (Copied to page 127). A motion was made by Mr. Chmiel and seconded by Mr. Adkins approving payment of the Arcadis Invoice in the amount of $1,012.64, for Fall 2014 Semi Annual reporting and 2014 Annual reporting for the 691 Landfill. Professional services from January 26, 2015 to February 22, A motion was made by Mr. Chmiel and seconded by Mr. Adkins authorizing Commissioner Eliason s signature on the OWDA Disbursement Request #18 for the US 50 Wastewater Improvements OWDA # Invoice Amount: $18, Guardian Ad Litem - Comm. 5 C O M M I S S I O N E R S J O U R N A L 101 A T H E N S C O U N T Y, REGULAR SESSION March 130 A motion was made by Mr. Chmiel and seconded by Mr. Adkins approving the following Resolution for Dakota Mays: Resolution - Dakota Mays A RESOLUTION HONORING Dakota Mays And the Nelsonville Wrestling Team WHEREAS, the Board of County Commissioners of Athens County wish to honor Dakota Mays and the Nelsonville Wrestling Team on their accomplishments; and WHEREAS, Dakota Mays is the 2015 State Champion for Division III, 160lbs weight class; and WHEREAS, Dakota started wrestling at the age of 7, placing every year he wrestled. 4th grade placing 8th in the State of Ohio, 5th grade placing 2nd place in the State of Ohio 6th grade placing 4th in the State of Ohio Repeating 6th grade he wrestled in two classes Junior High and Grade School, winning State Champion in both classes 7th grade placing 2nd in the State of Ohio 8th grade placing 4th in the State of Ohio Freshman year grade placing 6th in the State of Ohio Sophomore year grade placing 7th in the State of Ohio Junior year grade placing 4th in the State of Ohio SENIOR YEAR, STATE CHAMPION DIVISION III, 160lbs CLASS WHEREAS, the Board of County Commissioners of Athens County, offer their sincere congratulations to Dakota Mays. 7 C O M M I S S I O N E R S J O U R N A L 101 A T H E N S C O U N T Y, REGULAR SESSION March 132 A motion was made by Mr. Adkins and seconded by Mr. Chmiel to suspend the rules and declare an emergency and add the following to the agenda: * Plains Water & Sewer Billing Issue The roll being called upon for adoption, the vote resulted as follows: Mr. Eliason, yea; Mr. Adkins, yea; Mr. Chmiel, yea. A motion was made by Mr. Adkins and seconded by Mr. Chmiel to credit the PWS bill for customer Rhonda Dietrich, in the amount of $32.00 (for the no usage amount) and waiving the minimum bill for the month. A motion was made by Mr. Adkins and seconded by Mr. Chmiel approving and authorizing Commissioner Eliason s signature on the following Resolution of the Athens County Commissioners Authorizing Participation in ODOT Cooperative Purchasing Program Form, as presented by County Engineer, Jeff Maiden. RESOLUTION OF THE ATHENS COUNTY COMMISSIONERS AUTHORIZING PARTICIPATION IN ODOT COOPERATIVE PURCHASING PROGRAM Suspend-Add to Agenda - PWS Billing Issue ACWSD - Credit - R. Dietrich Engineer - ODOT Co-Op Purchasing Program Resolution Meeting or Approval Date: Tuesday, March 17, 2015 WHEREAS, Section (B) provides the opportunity for Counties, Townships, Municipal Corporations, Conservancy Districts, Township Park Districts, Park Districts created under Chapter 1545 of the Revised Code, Port Authorities, Regional Transit Authorities, Regional Airport Authorities, Regional Water and Sewer Districts, County Transit Boards, State Universities or Colleges to participate in contracts of the Ohio Department of Transportation for the purchase of machinery, material, supplies or other articles. NOW, THEREFORE, Be it ordained by Athens County SECTION 1. That the Athens County Engineer, Jeff Maiden, hereby requests authority in the name of Athens County to participate in the Ohio Department of Transportation Summer (418-16) and/or Winter (018-16) contracts for road salt. SECTION 2. That the Athens County Engineer, Jeff Maiden, is hereby authorized to agree in the name of the Athens County Commissioners to be bound by

8 C O M M I S S I O N E R S J O U R N A L 101 A T H E N S C O U N T Y, REGULAR SESSION March 133 all terms and conditions as the Director of Transportation prescribes. SECTION 3. That the Athens County Engineer, Jeff Maiden, is hereby authorized to agree in the name of the Athens County Commissioners to directly pay vendors, under each such contract of the Ohio Department of Transportation in which The Athens County Engineer participates, for items it receives pursuant to the contract. SECTION 4. That Athens County Engineer, Jeff Maiden, agrees to be responsible for resolving all claims or disputes arising out of its participation in the cooperative purchasing program under Section (B) of the Ohio Revised Code. The Athens County Engineer, Jeff Maiden, agrees to waive any claims, actions, expenses, or other damages arising out of its participation in the cooperative purchasing program which Athens County may have or claim to have against ODOT or its employees, unless such liability is the result of negligence on the part of ODOT or its employees. /s/lenny Eliason (Authorized Signature) Date Lenny Eliason, President Athens County Commissioners A motion was made by Mr. Adkins and seconded by Mr. Chmiel to authorize submission of the Local Government Innovation Fund (LGIF) Grant for 911 and to have the Interim County Planner, Bob Eichenberg, help with the writing of the Grant. 911 Director, Dan Pfeiffer will contact Mr. Eichenberg in order to proceed. ~ Judge McCarthy - Court Cost Increase - Discussion regarding the mandated fee Increases of $26.00, for Domestic Relations Cases and $15.00 for Juvenile Division Cases, which will take effect on March 23, This increase is to compensate the Ohio Legal Assistance Foundation Fund. Judge McCarthy also discussed the status of the Court Fee system which he stated has not been raised in quite some time, (related to Civil cases only). Also discussed was the need for a fourth Constable, which he feels is necessary in order for them to effectively carry out their duties. Commissioner Eliason stated he felt video arraignment might help the situation and also brought up the idea of funding the Constables LGIF Grant - Authorize Submission Common Pleas Judge McCarthy - Discussion - Fee Increases - Additional Constable - Recording System

9 C O M M I S S I O N E R S J O U R N A L 101 A T H E N S C O U N T Y, REGULAR SESSION March 134 under the Sheriff Department. Commissioner Adkins feels that there is a positive working relationship with Sheriff Smith and should be discussed with him. The Commissioners will review the situation for future consideration. ~ Judge McCarthy also brought up the issue of the recording system in the main courtroom and the fact that it is between ten and twenty years old and they are having some difficulties with it and thinks it may be around $20, to replace it. He also discussed the Court Technology Grant and upgrading the Courtview Docketing System which could possibly cost between $10, to $20, He hopes to include this cost in the grant. A motion was made by Mr. Adkins and seconded by Mr. Chmiel to adjourn the above meeting.
