1 US INBOUND AND US OUTBOUND Frequently Asked Questions: Transition of global healthcare for international assignees to Cigna during 2016 In the event that the content of this communication or any representations made by any person regarding Caterpillar s employee benefits plans and programs conflict with or are inconsistent with the provisions of the governing documents, the provisions of the plan documents are controlling. To the fullest extent permitted by law, Caterpillar has reserved the right to amend, modify, suspend, replace or terminate any of its plans, policies or programs, in whole or in part, at any time and for any reason, by appropriate company action 1. Why did Global Mobility decide to utilize a Global Healthcare Provider for our assignees? There are currently multiple healthcare providers for our assignees; moving to a single global assignee healthcare program allows us to deliver fair and consistent, market competitive healthcare globally while allowing Caterpillar greater governance and compliance of our assignee healthcare provisions. 2. What are the key enterprise benefits of transitioning to a Global Healthcare Provider for assignees? Implementing a single global assignee healthcare program will enable Caterpillar to deliver: Fair and consistent, market competitive healthcare globally Improved support of assignee and family health and wellbeing while on assignment Enhanced member services and healthcare experiences while on assignment Greater governance and compliance of our assignee healthcare provisions 3. How was Cigna selected? The project team, in conjunction with Global Supply Network and Legal Services, undertook a robust selection process to evaluate appropriate global vendors in the healthcare market. As a result, Cigna was selected. 4. Who is Cigna? Cigna is one of the largest health service companies in the world with over 220 years of corporate experience and approximately 84 million customer relationships worldwide. It has over 50 years of experience in global health benefits covering over 926,000 members in 192 countries. For further information, please visit the Cigna website at 5. Who was involved in the vendor selection process? The project team consisted of global and regional mobility team members and global healthcare contacts from each Caterpillar region, along with external broker support from Mercer, our global international benefits consulting and brokerage services provider. 6. When will assignees transition to Cigna? US outbound (US Payroll) and US inbound assignees currently on the United Healthcare plan will transition June 1, The goal for the remaining assignee population is to transition during 2016 depending on local plans and legal requirements However, please be aware that most current assignee healthcare provisions are managed by the home benefits group, and therefore transition dates have been arranged based on the date the home healthcare plan will transition into the global scheme. This will mean assignees in one location will transition into the new plan on a variety of dates throughout How will I be impacted? Our primary aim is to minimize disruption to you and our assignee community, while ensuring we effectively implement the new supplier in a timely fashion to maximize the improvements and reduce risk. If there are any concerns or issues raised during the transition please contact Cigna directly. Alternately, you can contact your HR for escalation to the appropriate regional mobility team. 8. When should I expect to receive information from Cigna? You will be provided with welcome information from Cigna and a full membership pack in a timely manner before your transition date. Cigna will also offer webinars around your transition to support you through the transition and

2 help you learn more about Cigna s services. 9. What if I have questions about my coverage with Cigna? Cigna will issue a welcome before you transition to the new healthcare plan. Once you have received your welcome , you will be able to log onto Cigna Envoy where you can download your virtual membership card, view benefits, coverage information, country guides and search for local healthcare providers. Cigna also operates a 24-hour helpline where you can obtain additional information. 10. Will there be any changes to my healthcare provision/benefits? As part of the transition, Caterpillar has reviewed all current healthcare plans and endeavored to align with the current provision to ensure a fair, consistent level of care for all assignees on a global basis including medical, dental, vision and emergency evacuation. As you will appreciate, combining multiple medical plans will mean some modifications from current levels. We would encourage you to review your full benefits and coverage information when you receive your welcome packs to understand any variation in your benefits. To support US-specific healthcare requirements, there will be three plan types: US outbound (US Payroll), US inbound and the rest of the world (excluding the US). 11. I am a US outbound assignee. Will I be required to continue to contribute towards my healthcare provision/benefits? US outbound assignees will continue to contribute toward their healthcare premiums through payroll deduction. The 2016 premiums will be similar to the 2016 PPO Option B premiums, based on family size. You may contact the Caterpillar Benefits Center at Aon Hewitt by calling , within the United States or , outside of the United States, to obtain premium information. Representatives are available Monday through Friday from 8 AM to 6 PM US Central Standard Time. US inbound assignees and global assignees may also continue to pay for a portion of their healthcare through their home country benefit scheme. 12. Will pre-existing illnesses be covered under the Cigna healthcare plan? Yes. There will be no penalty or waiting period for any existing medical conditions. 13. Who should I contact if I need medical services up to my transition date? The current insurer continues to be responsible for all aspects of your coverage until the agreed transition date. Prenotification for treatments required up to the transition date should be arranged via your current insurer and all claims for treatment incurred up to the transition date should be submitted to your current insurer for reimbursement. Pre-notification for treatment to be undertaken after the transition date should be arranged with Cigna once you have received the welcome pack. Cigna will be the plan provider from the agreed transition date and will be responsible for arranging coverage and reimbursement of any claims incurred from this date onwards. 14. What happens if I am in the middle of treatment when the change of healthcare insurer takes place? Upon receipt of your welcome , you can contact Cigna for assistance with the transition of your insurance. Cigna may approach your medical provider and inform them they will be covering the medical costs from the agreed transition date. All calls are treated in confidence and no medical information will be shared with Caterpillar 15. Who should I contact if I want to plan treatment post go-live but would like some advice sooner? As soon as assignees receive their welcome from Cigna, they can make contact to discuss medical issues related to treatment taking place from June 1, Will Cigna cover me if I am travelling on business? Yes, if assignees are traveling on business for Caterpillar after their transition date, the Cigna coverage will ensure they are covered for any medical treatment needed and in the event of an emergency. 17. Will my spouse and children also have coverage? Yes, if they are eligible dependents (either on assignment with you or residing in the home payroll or other country) under the terms of the healthcare plans. You will not be required to re-enroll any eligible family members who are currently covered by your existing provider. We do ask you check your membership to ensure it is up to date and all family member details are correct as your personal details are used for data protection checks and to allow Cigna to pay claims as quickly and accurately as possible.

3 18. Is Cigna able to provide coverage everywhere? Cigna has a number of joint ventures throughout the world who partner with them to provide an easier way for members to receive medical treatment globally. In some countries, assignees are provided with a separate card or a joint card for one of the Cigna Links partners. Assignees are encouraged to use this when obtaining treatment (e.g members in the Middle East will be provided with a local SAICO card, and members in China will be provided with a joint card with the joint QHMS logo). Using Cigna Links makes the claims processing easier as the provider will bill Cigna direct, so assignees will have no out of pocket expenses. Current Cigna Link s partners are in the following areas: Middle East, Spain, China, Australia, Singapore, Malaysia, Indonesia, Brazil and Africa. 19. When will I receive my local mandatory plan membership card? If you are in a country where there is a local mandatory plan membership required, Cigna will include the information regarding this membership within your welcome s and through their CignaLinks partners. For example, SAICO in Abu Dhabi, Saudi Arabia and Dubai. 20. Who should I contact for claims reimbursement and what is the deadline for submitting claims to my current provider? Claims with a service or treatment date before the transition date must be submitted as per your current process (please check with your current provider on timescales for submissions of these claims). After the transition date you are encouraged to use the Cigna Network providers to eliminate the need for you to pay up front and then have to submit a claim. If you require high cost treatment, you should contact Cigna to arrange a guarantee of payment with the provider directly to eliminate out of pocket expenses. Where reimbursement is required, the target is for all claims to be reimbursed within 10 business days using Cigna Envoy or via the Mobile App. 21. What should I do if I require a doctor or medical provider post transition? From the transition date onwards you can either: o Log on to Cigna Envoy where you can search to find your nearest suitable medical practitioner o Contact the Cigna helpline and you will be referred to the nearest suitable medical practitioner O You should also be able to continue using your current medical provider. However, you should contact the Cigna helpline before undergoing treatment. Whenever possible, Cigna can arrange to submit payment directly to the service provider eliminating the need for you to pay medical bills out of pocket yourself. 22. What should I do if I experience a medical emergency after the transition date? In the event of a medical emergency, you should contact the Cigna helpline immediately or International SOS. Cigna will work in partnership with International SOS to provide emergency support. If you (or a family member) require emergency medical treatment and suitable treatment is not available locally, Cigna will arrange for repatriation to the nearest medical center or to your home payroll country. It is important you let family and colleagues know you are covered by Cigna. You should always carry your Cigna virtual membership card or have your membership number and contact details for Cigna close at hand. 23. Will Cigna still partner with UnitedHealthcare (UHC) in the US? No, Cigna has their own network in the US and will provide support and coverage for you. 24. How long will it take me to register for Cigna Envoy and download my virtual membership cards? The welcome will include the relevant information required for you to register with Cigna Envoy. It should take less than ten minutes to complete the registration and retrieve membership card(s). You can also download the Cigna Envoy App on both iphones and Android phones after you have registered. 25. A. I am a current US payroll employee (not on an ISE assignment). Does my flexible spending account (FSA) continue when I go on an ISE assignment? Those covered under the Cigna International plan are not eligible to enroll in a flexible spending account (FSA). However, if an employee is eligible for enrollment in a FSA for a plan year and becomes eligible for the Cigna International plan offering due to an international assignment, the employee may continue to participate in the FSA plan until the end of the current plan year. No carryover of funds is allowed for the next year if covered under the Cigna International plan. B. I am a current US payroll assignee (currently on assignment) who has elected to contribute to a Health Care Flexible Spending Account (FSA). Does my Flexible Spending Account continue while I am covered under the Cigna International Plan? Those covered under the Cigna International Plan are generally not eligible to participate in the Caterpillar Inc. Flexible Spending Account Plan. However, if you are an expatriate who becomes eligible for the Cigna International Plan during a year, you remain eligible for the Flexible Spending Account Plan until the end of the plan year in which

4 you became eligible for the Cigna International Plan. No carryover of funds is allowed for the next year if you are covered under the Cigna International Plan. 26. If I am currently on assignment what is the transition process to Cigna on June 1, 2016? All existing UK Payroll assignees, US Payroll assignees, assignees into the United States currently covered by UnitedHealthcare and Asia Pacific assignees currently covered under the ACE plan will automatically be enrolled onto the Cigna healthcare plan and no further action is needed from assignees aside from registering with Cigna once they receive the welcome kit. 27. I have just begun my repatriation process. What does this transition mean for me? For current assignees repatriating effective June 1, 2016, it is critical the Cartus consultant is aware of their physical arrival so their healthcare benefits tie to their home payroll country. For current assignees repatriating after June 1, 2016 they will transition to Cigna until they physically repatriate. 28. How does an existing assignee refill prescription drugs? The assignee should contact Cigna for assistance in filling prescriptions 29. How does a new assignee fill prescription drugs prior to starting a new assignment? The assignee should continue to work with OptumRx prior to the assignment in order to fill up to a 6 month supply of medication. 30. Where do I go if I have additional questions? We understand you will have questions about this change and Cigna will have a customer service team available to address any queries you may have. By now, you should have received a welcome from Cigna. If you have not received that , please contact them directly at In addition, a recorded version of the informational Webex session with Cigna is now available on Benefits.cat.com for your review. Additional Questions and Answers 31. Will my home leave visits or R&R trips under the Global Mobility program or personal vacation be covered under Cigna? The Cigna Global Health Plan will provide coverage worldwide. It will provide coverage while on assignment in your host location, during business travel to other locations and during your home leave visit, R&R trip or personal vacation. Please review the benefits summary documents to understand the coverage available in and outside the US as these vary. 32. If I repatriate during the year, will I be eligible to participate in the Flexible Spending Accounts? Upon repatriation employees may elect to enroll in a flexible spending account up to the date of October 31 of the current plan year due to the administration and payroll limitations in place. 33. Are trailing dependents covered by the Cigna plan? If you are a US Payroll employee receiving healthcare coverage under UnitedHealthcare for you and your dependents, effective June 1, 2016, the assignee, any dependents and any trailing dependents will be enrolled on the Cigna plan, regardless of whether or not they are accompanying you on assignment. 34. I am (or will be) an accompanying/trailing spouse with my own UnitedHealthcare coverage, will I transfer to Cigna? If you are a US Payroll employee as an accompanying/trailing spouse receiving healthcare coverage under UnitedHealthcare for yourself, you will be enrolled on the Cigna plan. 35. Do any deductibles paid to date under UnitedHealthcare transfer to Cigna? The Cigna plan is entirely independent to your existing UnitedHealthcare plan with alternate benefits, limits, copayments and deductibles. No deductibles and Maximum Out of Pocket expenses will transfer to the Cigna Global plan. 36. If I have met my deductible or Maximum Out of Pocket with UHC prior to the transition to Cigna Global and I repatriate later this year, will those contributions through the UHC benefit plan transfer? Yes, those contributions in the current calendar year with UnitedHealthcare will resume if you repatriate in this current calendar year. 37. Does this change impact my Dependent Care Flexible Spending Account? No.

5 38. I am an Assignee who is not currently enrolled (or my eligible dependents are not currently enrolled) in Caterpillar-sponsored healthcare coverage. May I now enroll in Caterpillar-sponsored healthcare coverage? Yes. Under the Caterpillar healthcare plans, employees can change their elections mid-year in limited circumstances. One of the circumstances that permits a mid-year change is if a new benefit option is offered or if significant improvements are made to a current benefit option. The Assignee plan administered by Cigna is a significant improvement over the benefit options that were available to you under the Caterpillar Inc. Employee Health, Life, and Disability Benefit Program because the Assignee plan administered by Cigna has no deductibles or out-of-pocket costs when outside of the United States. Therefore, employees (or their eligible dependents) will be permitted to enroll in coverage if they are not currently enrolled, or switch to a different coverage tier to cover dependents (e.g., switching from employee-only coverage to family coverage). However, any changes must be made within 31 days of the effective date of the new plan, so your deadline to make enrollment changes is July 1.
