3 WHAT HAPPENS TO YOUR BENEFITS What happens t yur benefits if yu becme disabled, die, take a leave f absence, leave the cmpany r have anther Qualified Status Change? Yu can find answers t these questins in the pages that fllw. Fr mre detailed infrmatin abut the plans, see the apprpriate summary fr each plan and/r cntact the Emplyee Services Unit (ESU) via AskESU, 4-ESU@travelers.cm r In general, yu must request changes fr yur cverage within 31 days after the event. Changes are generally effective the day f the event. Changes can be apprved nly if the change in benefit cverage is cnsistent with the Qualified Status Change and if the request is received by the ESU within 31 days f the event. In general, a change in cverage will be cnsidered cnsistent nly if the Qualified Status Change results in the gain r lss f eligibility fr benefit cverage under a plan maintained by Travelers r under a plan maintained by yur family member s emplyer. Certain exceptins may apply. Fr example, if yu elect t make pre-tax cntributins t yur Health Savings Accunt (HSA) with Fidelity, yu can change that electin at any time n a prspective basis because such electins are nt subject t any Qualified Status Change rules. See the fllwing pages f this summary fr mre detail n the Qualified Status Change rules. Qualified Status Changes Different Qualified Status Change rules apply t different plans. Please review the relevant sectin f this summary fr the details f hw a specific Qualified Status Change event may impact yur benefits under a particular plan. In general, examples f Qualified Status Changes include the fllwing: Change in legal marital status, such as: marriage, divrce, legal separatin, annulment r death f yur spuse; Change in dmestic partnership status, such as: filing a Dmestic Partner Affidavit and Agreement r filing a Terminatin f Dmestic Partnership; Lss r gain f children, such as: birth, adptin, placement fr adptin r death f a child; Change in emplyment status, such as: terminatin r cmmencement f emplyment by yu, yur spuse r dmestic partner r yur child; Change in wrk schedule, such as: change in hurs f emplyment, cmmencement r return frm an unpaid leave f absence r a strike r lckut which causes yu r yur family member t gain r lse eligibility fr benefits r t pay mre fr benefits; Change in eligibility status f a child, such as: change in a child s eligibility under a plan due t attainment f age, student status, disabled status r change in custdial parent; Change in the eligibility f yu r yur family member fr Medicare, Medicaid r yur state s Children s Health Insurance Prgram (see the Medical summary fr mre details); Change in cverage under a plan maintained by yur family member s emplyer; Change in dependent care expenses fr Dependent Care Spending Accunts (see the Flexible Spending Accunts summary fr mre detail); Change in residence r wrksite, such as: change in yur r yur family member s lcatin f residence r wrksite ut f a health plan s cverage area; and Issuance f a Qualified Medical Child Supprt Order. Yu must cntact the ESU t request a change t yur benefits and cmplete the nline enrllment within 31 days after the Qualified Status Change. Yu must als cmplete the Benefits Affidavit. If yu fail t return the Benefits Affidavit, yur benefit changes may be reversed t yur previus electins. Yu may als be required t prvide additinal dcumentatin supprting yur Qualified Status Change, upn request. Failure t ntify ESU within 60 days f lss f eligibility status will result in yur r yur cvered family member s lss f cntinuatin cverage rights. Travelers will cnsider a failure t ntify Travelers when yu r a family member becmes ineligible as an intentinal misrepresentatin. This may result in a retractive lss f cverage fr yu r yur family member that is nt a rescissin. Travelers Qualified Status Changes (Rev 1-18) 3

4 IF YOU TAKE A LEAVE OF ABSENCE (Including Disability Leave) This sectin f the Qualified Status Changes summary describes what happens t yur benefits and yur respnsibility fr paying premiums, when yu start a leave, while yu are n a leave f absence and when yur leave ends. Generally, leaves f absence fall int five (5) types: Paid FMLA/STD leave this may be either paid Family and Medical Leave Act (FMLA) leave that is cvered by yur paid time ff (PTO), r shrt-term disability leave (and wuld include any paid parental leave yu may be eligible fr). Unpaid FMLA leave this may be either unpaid FMLA leave that starts after yu have exhausted yur PTO, r state paid family leave. Extended disability leave this starts after shrt-term disability leave ends. In general, any leave f absence due t disability is limited t nine (9) cntinuus mnths, starting n the date that yu transitin t LTD leave r unpaid leave (if nt enrlled and determined t be disabled fr purpses f the Travelers disability leave plicy). Even if yur leave extends beynd nine (9) mnths, any cverage that was available during the maximum apprved leave perid will end. Yur extended disability absence cverage may end earlier if yu recver, return t wrk r yur emplyment is terminated during the leave perid. Unpaid persnal leave in certain limited circumstances, Travelers may apprve a persnal (nn-fmla, nndisability) unpaid leave f absence. An appeal leave (during which yu can appeal a determinatin by MetLife under the shrt-term disability r lng-term disability cmpnent f the Disability plan) is cnsidered an unpaid persnal leave fr benefits purpses. Military leave - military leaves f absence generally fall int tw (2) types: Paid military leave this is military leave fr which yu are receiving military differential pay. Unpaid military leave this is military leave that is unpaid because yu are nt receiving military differential pay. Review the Travelers Military Leave plicy fr additinal infrmatin regarding military leave, such as which unifrmed services are cnsidered military service, what happens if yu d nt intend t return t wrk, the maximum length f service, the time perid t return t wrk with reemplyment rights and the effect f dishnrable discharge. Nte: Benefit treatment fr administrative paid leaves are handled the same as Paid FMLA/STD leaves. Benefits treatment fr administrative unpaid leaves are handled the same as Unpaid FMLA leave. Change in leave status: If yur leave changes frm unpaid t paid leave, yu can add r change cverage if cnsistent with the change, but this rule des nt apply t yur spending accunts. IMPORTANT: Yur request fr a change in benefit cverage will be apprved nly if it is cnsistent with the Qualified Status Change and received by the ESU within 31 days f the event. IMPORTANT NOTE ABOUT PREMIUMS: If yu are required t pay any premiums while n a leave and yu fail t d s within 60 days f the date yu are billed fr the premium, cverage will end at the end f the last perid fr which yu paid the last required premiums. Unpaid FMLA leave, extended disability leave, unpaid persnal leave, military leave: Yu have the chice t cntinue r discntinue cverage. T stp cverage, yu must file a written electin t stp cverage within 31 days after yur leave starts. Yur electin t stp cverage generally will be effective n the date yu file it (r when yur leave starts, if later). Nte: During military leave, yu may want t cnsider discntinuing cverage because gvernment cverage may be available. During Yur Leave Paid FMLA/STD leave, paid military leave (if cverage cntinues): Yu must cntinue t pay premiums. Generally, yu pay premiums n a befre-tax basis by yur paycheck. Hwever, if yu d nt have a sufficient amunt in yur paycheck t cver the premium, yu will be required t pay the premiums n an after-tax basis by persnal check. If the Wrking Spuse Subsidy Reductin applies t yu, it will cntinue, but yu may nw qualify fr the smaller Wrking Spuse Subsidy Reductin due t a decrease in yur incme, if yu ntify Travelers f such decrease within 31 days f the start f yur leave. Unpaid FMLA leave, extended disability leave, unpaid persnal leave, unpaid military leave: If yur cverage cntinues, yu must cntinue t pay premiums n an after-tax basis by persnal check. If yu are n unpaid persnal leave nly, the Wrking Spuse Subsidy Reductin will n lnger apply. Fr all ther unpaid leaves, if the Wrking Spuse Subsidy Reductin applies t yu, it will cntinue, but yu may nw qualify fr the smaller Wrking Spuse Subsidy Reductin due t the decrease in yur incme, if yu ntify Travelers f such decrease within 31 days f the start f yur leave. Fr all leaves: If yu fail t make a premium payment, cverage will end at the end f the perid fr which yu paid the last required premium. In this case, yu are nt eligible t enrll during the annual enrllment perid. If yu are n a leave during the annual enrllment perid r at the time f a Qualified Status Change, yu have the same rights as an active emplyee t drp, add r change cverage (except if yu fail t make a premium payment during yur leave). Yu cannt add r change cverage ther than under thse circumstances. When Yur Leave Ends Paid FMLA/STD leave, unpaid FMLA leave, extended disability leave, unpaid persnal leave, military leave (if cverage cntinued): Yur cverage will cntinue withut interruptin, and yu may nt make a new electin upn yur return. Unpaid FMLA leave, extended disability leave, unpaid persnal leave, military leave (if cverage ended): If yur cverage ended at the start f r during yur leave, yur pre-leave cverage will nt be reinstated upn yur return. 6 If Yu Take A Leave Of Absence (Including Disability Leave) (cntinued) Yu may make a new electin within 31 days f yur return t wrk. If n new electin is made during this 31- day special electin perid, yu cannt elect cverage fr the rest f that plan year unless a Qualified Status Change ccurs. Fr all leaves: If yur emplyment ends during r at the end f yur leave, yur cverage will end, at the latest, n the last day f the mnth in which yur emplyment ends. If yur Wrking Spuse Subsidy Reductin was discntinued r decreased due t yur leave, it will resume and will increase t the reductin in effect prir t yur leave, unless yur incme cntinues t qualify yu fr the smaller Wrking Spuse Subsidy Reductin. Life/AD&D Insurance When Yu Start Leave Paid FMLA/STD leave: Yur cverage will cntinue yu cannt elect t discntinue cverage. Unpaid FMLA leave, extended disability leave, unpaid persnal leave, military leave: Yu have the chice t cntinue r discntinue cverage. T stp cverage, yu must file a written electin t stp cverage within 31 days after yur leave starts. Yur electin t stp cverage generally will be effective n the date yu file it (r when yur leave starts, if later). During Yur Leave Paid FMLA/STD leave, paid military leave (if cverage cntinues): Yu must cntinue t pay premiums. Generally, yu pay premiums n an after-tax basis by yur paycheck. In this case, yu are nt eligible t enrll during the annual enrllment perid. When Yur Leave Ends Fr all leaves: Yur cverage will cntinue withut interruptin. Yu may nt make a new electin upn yur return. Yu cannt elect cverage until the next annual enrllment perid (r the ccurrence f a Qualified Status Change). If yur emplyment ends during r at the end f yur leave, yur cverage will end, at the latest, n the last day f the mnth in which yur emplyment ends. Disability Cverage under the STD cmpnent f the plan cntinues thrughut yur paid FMLA/STD leave, unpaid FMLA r military leave until yur emplyment ends (if applicable), but therwise ends when yu start an extended disability leave r unpaid persnal leave. Accrdingly, the fllwing relates nly t the LTD cmpnent f the plan. When Yu Start Leave Paid FMLA/STD leave, extended disability leave: Yur cverage will cntinue yu cannt elect t discntinue cverage. Unpaid FMLA leave, unpaid persnal leave, military leave: Yu have the chice t cntinue r discntinue cverage. T stp cverage, yu must file a written electin t stp cverage within 31 days after yur leave starts. Yur electin t stp cverage generally will be effective n the date yu file it (r when yur leave starts, if later). During Yur Leave Paid FMLA/STD leave; paid military leave (if cverage cntinues): Yu must cntinue t pay premiums if yu were required t pay premiums befre yur leave started (that is, if yur base salary is mre than $50,000). Generally, yu pay premiums n an after-tax basis by yur paycheck. Hwever, if yu d nt have a sufficient amunt in yur paycheck t cver the premium, yu will be required t pay the premiums n an after-tax basis by persnal check. Unpaid FMLA leave, unpaid military leave: If yur cverage cntinues, yu must cntinue t pay premiums if yu were required t pay premiums befre yur leave started (that is, if yur base salary is mre than $50,000). Yu will be required t pay the premiums n an after-tax basis by persnal check. Extended disability leave: Yur cverage will cntinue yu d nt pay premiums fr the perid yu are n leave. Unpaid persnal leave: If yur cverage cntinues, yu must cntinue t pay premiums, even if yur base salary is $50,000 r less. Yu will be required t pay the premiums n an after-tax basis by persnal check. If yu are n a leave during the annual enrllment perid r at the time f a Qualified Status Change, yu have the same rights as an active emplyee t drp r add cverage (except if yu fail t make a premium payment during yur leave). If yur base salary is ver $50,000, yu may add cverage subject t all applicable rules, including evidence f insurability and a requirement that yu be actively at wrk fr added cverage t take effect. Yu cannt add r drp cverage ther than under thse circumstances. When Yur Leave Ends Paid FMLA/STD leave; paid military leave (if cverage cntinued), extended disability leave: Yur cverage will cntinue withut interruptin. Unpaid FMLA leave, unpaid persnal leave, military leave (if cverage ended): If yur salary is $50,000 r less and yur cverage ended at the start f r during yur leave, yur pre-leave cverage is autmatically reinstated withut evidence f insurability. If yur salary is ver $50,000 and yu did nt have cverage befre yur leave r yur cverage did nt change during leave, yu cannt change cverage when yu return t wrk (yur cverage may have changed while yu were n leave if yu made a new electin during an intervening annual enrllment perid r as a result f a Qualified Status Change). If yur salary is ver $50,000, and yur cverage ended at the start f r during yur leave, yur pre-leave cverage will nt be reinstated upn yur return. Hwever, yu may elect cverage within 31 days f yur return t wrk. Yu will be required t submit evidence f insurability in this case, unless yu were n unpaid FMLA leave and yu return t wrk within the time required by law. Cverage will start n the later f the day yu return t wrk r the first f the mnth fllwing the date yur applicatin is apprved (apprval nly applies if yu were required t submit evidence f insurability). If yu d nt elect cverage within 31 days f yur return t wrk, yur cverage is waived (unless yu made a new electin during an intervening annual enrllment perid r as a result f a Qualified Status Change, and that electin becmes effective when yu return). Yu cannt elect cverage fr the rest f that plan year unless a Qualified Status Change ccurs. Fr all leaves: If yur cverage ended during yur leave because yu failed t pay a premium, yu cannt elect cverage until the next annual enrllment perid (r the ccurrence f a Qualified Status Change). If yur emplyment ends during r at the end f yur leave, yur cverage will end, n the day yur emplyment ends. Regular Paid Time Off (PTO) Effect f a Leave n Yur Awarded but Unused PTO Paid FMLA/STD leave: Yu may have used PTO t cver the eliminatin perid (the first week f absence due t disability) under the STD cmpnent f the plan. 9 If Yu Take A Leave Of Absence (Including Disability Leave) (cntinued) Unpaid FMLA leave, unpaid persnal leave: Yu are required t use all f yur awarded PTO prir t taking unpaid leave. If yur unpaid leave cnsists f state paid family leave nly, depending n yur wrk state yu may nt be required t use PTO prir t paid family leave benefits beginning. Extended disability leave: Yur awarded but unused PTO will be paid t yu after fur (4) weeks f leave. If yur leave is less than fur (4) weeks, yur awarded but unused PTO will remain intact. Circumstances When Additinal PTO Will Be Awarded During Leave Or Upn Return t Wrk Paid FMLA/STD leave: PTO is nt awarded unless yu wrk under the STD Return t Wrk Prgram. In this case, PTO hurs are awarded t reflect the hurs yu wrk. These PTO hurs, which are prrated based n the number f hurs yu wrk under the Return t Wrk Prgram, will be available fr use during yur paid leave. When yu transitin frm paid FMLA/STD leave t extended disability leave r when yu return t wrk frm paid FMLA/STD leave (as applicable), yu will be awarded PTO hurs (less any hurs awarded due t wrk under the Return t Wrk Prgram) as if yu had been wrking during yur leave. Military leave (paid r unpaid): PTO cntinues t be awarded as if yu were wrking during yur leave. PTO carryver caps that nrmally apply d nt apply while yu are n military leave. Awarded PTO is paid upn yur return t wrk. Extended disability leave: PTO is nt awarded unless yu wrk under the LTD Return t Wrk Prgram. In this case, PTO hurs are awarded t reflect the hurs yu wrk. These PTO hurs, which are prrated based n the number f hurs yu wrk under the Return t Wrk Prgram, will be available fr yur use during the perid yu wrk under the prgram. Any unused hurs will be paid t yu after yu cmplete the prgram if yu remain disabled. If yu return t wrk fllwing cmpletin f the prgram, awarded PTO will be available fr yur use at that time. Unpaid FMLA leave: PTO is nt awarded during this leave. When yu return t wrk frm leave, yu will be awarded PTO hurs as if yu had been wrking during yur leave. Unpaid persnal leave: PTO is nt awarded during this leave. This is a general discussin f hw PTO interacts with leaves, but it is nt all-inclusive. Fr example, it is des nt discuss intermittent leaves. Travelers will cmply with the FMLA and all ther applicable laws. Purchased PTO When Yu Start Leave Paid FMLA/STD leave: Yur deductins will cntinue -- yu cannt elect t discntinue deductins at any time during this type f paid leave. T stp deductins, yu must file a written electin t stp yur deductins within 31 days after yur leave starts. Yur electin will be effective retractive t the date the leave starts. If yu stp deductins, yur calendar year-t-date deductins (less any time used) will be refunded. Unpaid FMLA leave, extended disability leave, unpaid persnal leave, unpaid military leave: Yur deductins stp. During Yur Leave Paid FMLA/STD leave: If yu are n leave during an annual enrllment perid yu can make an electin as lng as yu remain n a paid leave. Unpaid military leave, unpaid FMLA leave, unpaid persnal leave, extended disability leave: After fur (4) weeks f leave yur calendar year-t-date deductins (less any time used) will be refunded and yur pre-leave deductins will nt be reinstated. Military leave, unpaid FMLA leave, unpaid persnal leave, extended disability leave: If yu are n a leave during an annual enrllment perid, yu cannt make a purchased PTO electin fr the fllwing year. If yu are n a leave at the end f the calendar year, yur unused purchased PTO frm the current year will be refunded regardless f the length f yur leave. Furthermre, any electin fr purchased PTO time yu made during the annual enrllment perid fr the fllwing plan year will autmatically be cancelled. When Yur Leave Ends Paid FMLA/STD leave: Yur return t wrk des nt affect yur purchased PTO. Paid military leave: If yur deductins cntinued, yur return t wrk des nt affect yur purchased PTO if yu return in the same calendar year. Otherwise, yur electin will be waived in the next calendar year. If yu discntinued yur deductins and yu return t wrk within fur (4) weeks frm the start f yur leave and in the same calendar year, yur pre-leave deductins will autmatically resume. Any deductins yu missed during yur leave will be made up frm yur remaining paychecks in the same calendar year. Unpaid FMLA leave, extended disability leave, unpaid persnal leave, unpaid military leave: If yu return t wrk within fur (4) weeks frm the start f yur leave and during the same calendar year, yur pre-leave deductins will autmatically resume. Any deductins yu missed during yur leave will be made up frm yur remaining paychecks in the same calendar year. Fr all leaves: If yu return t wrk after the annual enrllment perid but befre December 31, yu may make a new purchased PTO electin fr the fllwing plan year nly if yu request t be enrlled by December 31 f the current calendar year. 11 If Yu Take A Leave Of Absence (Including Disability Leave) (cntinued) Hlidays Paid FMLA/STD leave: Hlidays are paid during paid leave. A hliday will be cnsidered a disability day and cunted twards the 13- week maximum STD duratin (if applicable). Yu will nt be required t use PTO fr any hlidays that ccur during the eliminatin perid.
