2 GETTING SET TO MAKE YOUR ENROLLMENT DECISIONS Priyah, Josh, Vanessa, and Marco are back, and they are getting set to make their enrollment decisions. First, let s see what the four have learned about the new Bind medical plan so far: It s mobile-friendly With Bind, you can use the Bind app or website to get the details you need fast check your coverage for specific procedures, request a virtual visit, find copay costs, and more. There s an out-of-pocket maximum Sometimes the frequency of your plan use and the associated costs vary from year to year. If you have a year of high medical expenses, there s an out-of-pocket maximum that protects you against significant medical costs. Free preventive care Like our other medical plans, all preventive care is covered at 100%, which means there is no cost to you. Core and prescription drug coverage Core coverage in the Bind plan ensures the care most people need is always covered. That means when you or any family member on your plan needs to see their primary doctor, get a prescription filled, have a baby, or go to the emergency room, they re covered by Core coverage. Add-In coverage If you or any covered family member has a medical issue that requires a planned procedure outside of Core coverage, such as knee replacement surgery, you can add coverage for that specific procedure. Add-In coverage can be purchased at any time, which means you re only paying for the coverage you need when you need it. Health Care Flexible Spending Account (FSA) compatible If you enroll in the Bind plan, you can also contribute to a Health Care FSA if you want. This type of account provides you a pre-tax option to pay for your healthcare expenses, including the copays associated with the Bind plan. With these features in mind, how does Bind compare to the other medical plan options for Priyah, Josh, Vanessa, and Marco as they think about their personal situation? BACK 2 NEXT

5 MEET JOSH Married with two sons Active family On-the-go Back pain issues needing possible surgery Josh has been working as a Senior Manufacturing Director in Boulder, Colorado for 10 years. He s married with two very active sons, ages 8 and 10, who are in a traveling soccer league. Josh knows that finding great medical coverage is an important part of maintaining his family s sporty lifestyle. Here s what he considers: Sometimes a bad flu season means many doctor visits for the family. Josh is always on the go. If he and his doctor decide to go forward with the procedure next year, Josh can buy the Add-In coverage right before the surgery happens. He doesn t need to decide during open enrollment if he ll have back surgery, so he s comforted knowing he can add coverage at any point during 2019 should he need it. Last year, Josh s family had a lot of medical expenses that added up quickly. He knows that getting the full picture means both looking at paycheck costs and estimating the potential costs of care. And, he doesn t have to pay all of this amount at once. Because Josh chooses a high value provider, his Add-In coverage includes an additional copay to the provider of $150 and a set number of ongoing, pre-tax payroll deductions of $100 for 26 paychecks, so Josh can pay for the Add-In coverage over time.
