2 Words of Advice and Caution Align to a Target Area Responses provided to Problem Statement, Service Strategy, Service Delivery and Key Activities, and Service Results should clearly align to a target area. Please review your entries carefully. There is no spell-check in the Service Summary. You may be able to enter more than the allotted character limit, but your reviewers will not be able to read beyond the character limit. You can use a word processor to draft, spell check and county characters in your entry. You can then copy that entry from a word processor in to the web application.

4 Key Terms Output Definition: The number, frequency or amount of an occurrence Examples o Number of unduplicated individuals served o Number of hours of service provided o Number of immunizations given o Number of mentors matched Outcome Definition o What occurs as a result of activities o Calculation of the impact of a service/program Examples o % of students with on-time promotion to next grade level o o % of individuals served placed in a job within six months % of patients initially treated for health risk who have reduced that health risks (i.e. patients with hypertension with lowered blood pressure readings, patients with prediabetes with HgA1c levels brought in to normal range) Method for data collection and tracking Definition o The way individuals are counted, calculations are made or results are verified Examples o Attendance sheets o Surveys o Patient database Target Actual Definition: The goal for output or outcome This is usually tied to the program budget (i.e. the organization plans to serve 1,000 clients and therefore the budget for the service is $100,000) Definition: the verifiable results achieved Words of Advice and Caution Be clear on the number of people served with County funding Be clear on the difference between output and outcome If there is no output/outcome data from prior years, please input a zero for each prior year. 5 Budget Summary Key Terms Budget financial plan for how you will accomplish activities and achieve goals Fiscal Year period used for calculating annual financial statements FY14 Actuals Revenue/Income and expenses that tie back to your audited financial statement from fiscal year ending in 2014 FY15 Budget Revenue/income and expenses as approved/adopted/ passed by your Board of Directors for fiscal year ending in 2015 FY16 Budget financial plan for delivering services as proposed in this application Words of Advice and Caution Present clear consistent data. The four cells circled to the left must equal one another. If you submit more than one application, ensure that you provide the same Total Organization Budget for current year in both applications. There is no pre-defined amount of money that will be granted each year. Applications are reviewed and scored by subject matter experts in each target area, and then the applications ranked irrespective of target area. Grant awards range from $15,000 to $1,000,000. Typical first-year grant awards are $25,000 to $50,000.
