1 Registration Information Early Bird Registration: Now until May 17 th /17. Deals include single class discount (10% off tuition), multi-class discount (15% off tuition). Both deals apply only if paid in full and does not apply to specialty programs. Please keep in mind we do not hold spaces. You must come in to register as soon as possible to take advantage of these great deals and make sure you get the classes you desire. Now accepting payments with credit cards over the phone for current students. Current students do not need to fill out a new registration form when coming in, just need to verify information. We are currently registering for the 2017/2018 dance year. Please call the studio for exact registration times. We are accepting registrations for: Preschool, Jazz, Hiphop, Ballet, Pointe, Tap, Lyrical, and Acro Fees for 2017/2018 Cost ($) Costume ($) Total ($) Base Cost (1 Class) Add a 2nd Class Add a 3rd Class Add a 4th Class Add a 5th Class Add a 6th Class Add a 7th Class Add a 8th Class Add a 9th Class FREE 50 + Costume *The multi-class discounts apply to single students as well as siblings and parents. **Each additional class after is $ per class for the year. A payment must be made in order to hold the spot in the class. A minimum of onequarter of the total amount owing must be paid to finalize registration. The remaining balance will be divided and placed either on pre authorization debit, pre authorization with credit card, or postdated cheques.

2 Special Notice Regarding Payments: Please keep in mind that only payments made by December 31st, 2017 will be reflected on your 2017 tax receipts from The Catz Meow Dance Education Centre Inc. Each receipt will be ed to you and is considered legitimate for tax receipt purposes. Please keep them for your records. We accept cash or cheque, debit or credit cards here at the Studio. Please make all cheques payable to The Catz Meow. Please note that there is an NSF fee of $45 on returned or declined payments. There will also be no refunds after December 1st, Please inform Catherine of any concerns that you have in regards to your child s dance lessons. 5 Studio Participant Agreement Form 2017/2018 Terms & Conditions 1. I agree that my 1 st Payment will be made on the day of registration. As well as my $10 processing fee if not paying in full and choosing 1 of the 3 payment options. 2. I agree that the pre-authorized debit payment will take place on the 25 th of every month from September 25 th /2017 to March 25 th / I agree that credit card pre-authorization payments will take place on the 15 th of every month from Sept 15 th /17 to March 15 th / I agree that postdated cheques will be given and dated the 1 st of every month from Sept 1 st /17 to March 1 St / I am aware of a $45 nsf fee will be due if any form of payment is returned or unable to process. 6. I agree that if my account is not paid in full by March 25 th, my dancer/s will not be allowed to receive their showcase costume or tickets. 7. I agree that my account is not up to date by March 25/2018 interest charges will apply and by third attempt, a collection agency will be contacted. 8.
