4 Employees appointed to full-time positions in Merit classifications are eligible for participation in the following benefit programs, unless otherwise indicated: RETIREMENT Iowa Public Employees Retirement System (IPERS) (800) Eligibility: All City Employees Defined benefit Employee contribution is 5.95 percent of gross wages. Employer contribution is 8.93 percent of gross wages. Number of years to vest: 7 or age 65 Normal retirement age: 65 or rule of 88:(age + at least 30 years of service=88) Municipal Utility Retirement System Administered by Vanguard Eligibility: Non-temporary employees with at least 10% of salary funded by utility revenue. Defined contribution Employee contribution is 5.4 percent of gross wages. Employer contribution is 7.93 percent of gross wages. Number of years to vest: Immediate Normal retirement age: 55 or later 4

6 DENTAL INSURANCE DELTA DENTAL Premium incorporated into health insurance premium. (800) DELTA DENTAL I (available with both Alliance Select and Blue Advantage Health Insurance) Deductible: $50/$150 Maximum benefit: $750 Coverage at 100%: cleanings, oral evaluations, fluoride applications, X-rays, sealants Coverage at 80%: cavity repair, tooth extraction, root canals Coverage at 80%: periodontal services, crowns, inlays, dentures, bridges DELTA DENTAL II (only available with Blue Advantage Health Insurance) Must remain on the plan for a minimum of 3 years. Deductible $50/$150 Maximum benefit: $1,250 Coverage at 100%: cleanings, oral evaluations, fluoride applications, X-rays, sealants Coverage at 80%: cavity repair, tooth extraction, root canals. Coverage at 50%: periodontal services, crowns, inlays, dentures, bridges Coverage at 50%: orthodontics-dependents to age 19 (maximum of $1500) LIFE INSURANCE The City provides life insurance coverage, including accidental death and dismemberment for all benefited employees. Coverage is effective on the first month following 30 days of employment. Full-time employees receive $50,000 in coverage. LIFE INSURANCE SUPPLEMENTAL Voluntary Benefit Information available from Human Resources LONG TERM DISABILITY INSURANCE Eligibility: Insurance becomes effective the first of the month following one year of eligible employment. IPERS: the City provides coverage for all full-time employees. Utility Retirement: employees participating are required to enroll in the disability program and to pay the full premium cost through payroll deduction. 6

8 FLEXIBLE SPENDING ACCOUNTS Employees may elect to have pre-tax salary dollars deducted from their payroll to pay for monthly health insurance premium, non-covered medical, dental, vision, and/or dependent care expenses. Plan Year: Calendar year Medical Reimbursement Acct: Minimum - $ Maximum - $2, Dependent Care Reimbursement Acct: Minimum - $ Maximum - $5, Claims are reimbursed: Carryover (medical only): Weekly Carry over up to $500 from one plan year to the next Premium Only Plan: your insurance payments are taken out of your paycheck before taxes are with held. Dependent Care Reimbursement Account: covers day care, home care, or nursery care expenses for children under age 13 and eldercare. Medical Reimbursement Account: covers deductibles, co-payments, vision care, dental care, prescription drug expenses and other eligible expenses. HEALTHY 4 LIFE The Healthy 4 Life individualized health incentive program is designed to motivate actionable healthy lifestyle behavior change that results in improved health risk status of the participants in the program. The program is self-directed by the participant, but involves the direction and support of the City Health Promotion program. Healthy 4 Life provides a financial reward linked to benefits upon successful completion of the program. Success is determined by measurable changes in health status that occur over a one-year time frame. Alternatives will be provided for those in which it is unreasonably difficult to achieve the standard. Successful participants are rewarded through a subsidized premium for health insurance. All full-time or part-time employees covered by the City health insurance plan are eligible for the program. For additional requirements, eligibility, process, criteria and incentive information, contact the Health Promotion Coordinator. 8
9 DEFERRED COMPENSATION The 457 program is a pre-tax savings program limited to government employees (and some not-for-profit organizations). Each 457 participant develops his or her own portfolio and investment strategies. Current limit is $18,000. Since participation in a 457 program is optional, you may enroll any time during your employment with the City. TUITION ASSISTANCE AND EMPLOYEE DEVELOPMENT An employee must have completed the six-month probationary period in order to be eligible for participation in the educational assistance program. Courses must be jobrelated, either to the employee s present position or one within the normal career progression from the employee s present position. (Please see policy for further details.) Educational assistance shall not be considered an employee right but shall be granted at the discretion of management. VACATION Employees accrue vacation on a monthly basis: 1 st through 6 th year of employment: 10 days per year 7 th through 13 th year of employment: 15 days per year 14 th through 23 rd year of employment: 20 days per year 23 rd and subsequent years of employment: 25 days per year Vacation may be accumulated up to one and one-half the annual rate. SICK LEAVE Employees accrue sick leave on a monthly basis at the rate of 8 hours per month with unlimited accumulation. Employees may use up to 24 hours of accrued sick leave, per fiscal year, for occasions which require the employee to care for a member of their immediate family who is incapacitated due to illness or injury, or who has examinations with a physician or other health care provider. 9
