2 REQUEST FOR PROPOSAL (RFP) FOR BEFORE AND AFTER SCHOOL CHILD CARE PROVIDER Notice is hereby given that Coal City CUSD 1 (District) requests proposals for before and after school care providers for the , and School Years. Proposals will be received until 2:00 P.M. local time on Thursday, May 26, 2016 at the office of the Business Manager opened at the address listed below. Baima Street, Coal City, IL (815) The District will base its qualified provider selection on the following criteria: 1. Price per student charged to Parents of qualifying students grades K Experience of provider with respect to child care services 3. Elements of the program Upon review of Proposals received in response to this RFP, the District may enter into a contract with the qualified provider that provides the best overall value to the District. The District reserves the right to reject any and all proposals and to be the sole judge of the value and merit of the proposals offered. Jason Smith Business Manager 2

3 Specification: Qualified providers must: be licensed by the State of Illinois as a Child Care Provider provide criminal background checks for all personnel who will be working on site provide proof of worker compensation insurance for all personnel who will be on site provide liability insurance (see insurance section below) and name the District as an additional insured provide all supplies and materials for the program at the provider s expense comply with all IDPH health requirements and District requirements related to any snacks/food provided during the program provide before and after school care at each of the following locations: Coal City Early Childhood Center, Elementary School and Intermediate School provide before school care for students from 7 AM until start of school day on all school days provide after school care for students from the end of the school day until 5 PM for all FULL school days (note: early dismissals excluded) provide not only supervision of students but also include structured time for students to work on homework as well as organized activities and physical play be cognizant of cost of service to parents of qualifying students and must hold fees constant for the school year acknowledge that the minimum number of participating students per location is 10 (ten) provide a written program plan for evaluation of services. INSURANCE The contractor shall provide and maintain insurance in the amounts outlined below with companies acceptable to the institution. a. Worker s Compensation Insurance 3 Coverage A - Illinois Statutory Limits Coverage B - Employer s Liability $500,000 Limit b. Comprehensive General Liability Insurance Provide the following limits: Bodily Injury/Property Damage $1,000,000 each occurrence $1,000,000 in the aggregate The policy shall include the following coverage 1. Premises/Operations 2. Independent Contractors 3. Products/Completed Operations 4. Contractual Liability Blanket 5. Broad Form Property Damage 6. Personal Injury-Offenses A,B,C, -exclusion C deleted

4 Experience and Professionalism: Please provide a company vitae that includes: A narrative of experience in the business germane to the services being offered A description of company vision, goals and mission A list of references who can speak to the services provided Qualifications of personnel that will support the district during the length of the agreement NO FORMAL RFP OPENING WILL BE HELD. All proposals will be firm for a period of sixty (60) days from May 26, The Board of Education or their designee reserves the right to reject any and all proposals, to waive all irregularities, and to accept that proposal which is considered to be in the best interests of the District. Any such decision shall be considered final. Vendors must submit all certifications required by the District (see attached). In the event that the selected provider cannot provide services as outlined above, the provider will pay the District the tuition for the qualifying students in the program for maintenance of services. 4
