Minutes of the last meeting were not available but will be sent. A request was made that the minutes from the prior meeting be ed to JLMBC members a week or more prior to the scheduled next meeting date. Marcia indicated that staff will endeavor to achieve that goal once Benefits Unit is fully staffed. In the meantime, all JLMBC members will be provided ample time to review and provide input to the meeting minutes before they are finalized and placed on the JLMBC website. Page 1 of 5

Review Annual Cost Projection Report The Segal Company The Cost Projection Annual Report for June 1, 2009 May 31, 2010 was distributed and reviewed by Tom Morrison and Bob Mitchell with committee members and a copy of the report is attached and incorporated by reference. The Cost Projection Report includes the actuarial information of the County Health Plan (self-insured plan), as well as estimated costs for all other insurance plans provided by the County. The information contained in the report are not a guarantee of future rates. Tom Morrison explained carriers like to have six months of actual claims experience before estimating new rates for the new plan year, which would be claims experience through Therefore, actual rates will not be known until late February. Current projected rate increases. Most rates are preliminary unless indicated as final. Kaiser 16% PacifiCare 19% - 21% CHP 0% - Final Delta Dental 11% ING 25% MHN 6% Caremark N/A (3 year rate guaranteed through to June 2011) VSP 0% (2 year rate guarantee through to June 2010) - Final Hartford 0% (3 year rate guarantee through to June 2010) - Final Standard Long Term Disability (Non-safety members)- 0% (2009/10 proposed multiple year rate guarantee)- Final The Kaiser $0 co-pay plan is illustrated, but as of June 2 nd, 2009 this plan will be eliminated. Myrna Dillon from Kaiser offered to a white paper on the integrated delivery of prescription drug costs. This paper will be sent to Segal and distributed to the committee. A new benefit option for Kaiser Chiropractic benefit and rates were reviewed with the Committee. Based upon requests from various employees, the County is offering to add Chiropractic benefit to the Kaiser $10 Co-pay plan. Adding the Chiropractic benefit to the $10 co-pay plan will add a.5% increase to the rates. Q: Can the Chiropractic benefit be added to the Senior Advantage plan? A: There is currently a chiropractic benefit in this plan. The Committee expressed interest in adding this benefit and the County will send out information to the employee associations to add this benefit to the Kaiser $10 co-pay plan. It was indicated the County s interest is that 100% unanimous employee organization agreement will be necessary to add this benefit. Q: Can the Chiropractic benefit be optional for each enrollee? A: No. It is built into the rate for the entire plan option, so has to be made available to all enrollees in the plan. Page 2 of 5

3 4) Kaiser Permanente Capacity for New Members Presentation by Myrna Dillon, Kaiser Senior Account Manager, and Dr. Pappas, Asst. Physician Chief of Medical Center Santa Rosa Dr. Pappas & Myrna Dillon were asked to address the concerns JLMBC members and others have regarding Kaiser s ability to accommodate the possible large migration of employees to the Kaiser plans due to County changes being implemented in June A summary of Dr. Pappas presentation is attached as Addendum No. 1 ( ) and incorporated by reference. Information included the addition of staff services and facility expansion being done to address increase. Kaiser received the highest ratings ever in the California State Advocates Annual Report Card receiving 4 out of 4 scores for both outcomes and patient satisfaction. Kaiser brochures were shared at the meeting. These brochures will be posted on the (JLMBC) website. 5) Update on Retiree Optional Medical Plans AARP An overview of the AARP-United HealthCare Plans were presented at the JLMBC Meeting. These options are being provided in response to the Board of Supervisors direction to find additional low cost options for retirees both in California and nationwide. Q: Will there be education for retirees about how to understand these new plan choices and how to navigate the AARP online services? A: AARP communicates with retirees all the time; this is their main audience/market. They will use all types of mediums to communicate with retirees any perceived complexities of the plans and/or choices JLMBC Meeting Schedule The JLMBC Committee affirmed the 2009 meeting schedule will remain on the third Thursday of every other month starting in February, Time: 1:00 PM 3:00 PM. All meetings are scheduled for the Human Resources Training Room located at 575 Administration Drive, Suite 117C, Santa Rosa. 4 Meeting Recap and Follow-Up Items Approximately 1,100 employees were vaccinated during the employee flu clinic. Request for information regarding what s happening for retirees who may be at risk with regard to the increase in their premiums. Marcia commented there has been some discussion with regard to creating a hardship review committee. Will provide update at next JLMBC meeting. There was a request for feedback on the Kaiser Annual Utilization Report. Marcia explained this report is usually provided after the renewal rates are confirmed; sometime in March. Once available this will be placed on the next applicable JLMBC meeting. Consider having a Kaiser representative present. Question regarding elimination of Dual Coverage (Andre): Q: Can spouses who are non county employees or those not yet impacted by this change (e.g. local 39) double cover their spouses who are county employees? A: Dual coverage only applies to County employees who have a spouse or domestic partner who is also a County employee. A Local 39 member married to a Local 39 member can continue dual coverage, pending negotiation of a new MOU for Local 39. If a Local 39 member is married to a SEIU employee, the SEIU employee cannot be covered by the Local 39 member, as the terms and conditions of employment for the SEIU employee doesn t allow dual coverage. Waivers for Retirees (from Bill Robatka): Q: What are the status/process for retirees and the one time opt out waiver? A: Retirees will be allowed to waive County-offered medical coverage on a one- time basis and may opt back into County-offered medical coverage once. In compliance with HIPAA, the County has been offering recent retirees this waiver but current retirees who retired prior to have not yet been provided this option. Risk Management is planning to incorporate a one-time opt-out for current retirees into the Open Enrollment process in spring If the retiree waives coverage then any and all their dependents or spouse also waives. Hardship Review Committee Kathy Wertz inquired if there was any decision made about establishing a resource for retirees who may be at risk of losing their medical coverage due to the new premium share of cost. Marcia indicated that this resource is being developed as the need arises. With the addition of the AARP United Health plan options, a number of health plan options will be available for retirees within the County s contribution. More information will be provided as the need for this resource develops. Page 4 of 5

5 Coordination of Benefits Andre expressed concerns about the coordination of benefits, in particular the process for HMO s and PPO s and with TriCare, military retiree benefits. Marcia referred JLMBC members to the Summary Plan Document for each health plan outlines Coordination of Benefits for each specific plan. It was also proposed that Segal provide a presentation on this issue at the next JLMBC meeting.
