1 BENEFITS AT A GLANCE 2017/04/01 This Benefits at a Glance summary outlines some of the specifics of the benefits that would normally apply to unionized fulltime employees of the following union groups; IAM; USWA Local 7806; UA Local 254, and SPEA. It is a summary only and does not include all details, provisions, exclusions and limitations. In the event of a discrepancy between this summary and the Group Policy, the terms of the group policy (which is retained in Human Resources) will apply. For further details, refer to the specific section outlined in the Benefit Booklet. Queries regarding your health and dental coverage should be directed to Manulife at For information regarding other benefit plans, contact your site HR Services Office. Please be advised there is no COB provision offered for spouses who are both employees at CNL. When both spouses in a Family Plan are employees of CNL, only one employee can carry the benefits, the other must be named a Dependant under their spouse s plan at CNL. Please discuss your options as a Spouse of an CNL employee with your HR Administrator during your enrolment period, prior to making your final decision regarding your coverage. IMPORTANT: Where a reimbursement limit is not specified for any of the Health Care and Dental benefits listed below, reimbursement will be limited to Reasonable & Customary Charges as determined by Manulife. Health Care Benefits Extended Health Benefits Semi Private Prescription Drugs Private Nursing Medical Equipment Accidental Dental Ambulance Vision Other Services Employees are eligible for coverage as of first day of employment. Company pays 75% of the premium and employee pays 25%. Plan pays the cost of eligible expenses in excess of the annual deductible of $25 single and $50 family (deductible applicable to prescription drugs only on a calendar year basis). Semi-private reimbursed at 100%. Generic Price Substitution Arrangement for prescription drugs. When a generic drug exists, the plan reimbursement will be limited to the generic drug price. Only prescription drugs that require a legal prescription by law are covered. Over the Counter medications are not covered unless they are deemed life sustaining. Employees who are enrolled in the Extended Health Care plan have been provided with a ManuScript pay direct drug card. When purchasing prescription drugs (in Canada), employees and/or their dependents should present this card to their pharmacist. If Manulife has all the required subscriber and dependent information in their system, and if the benefit is eligible for reimbursement, Manulife will reimburse the pharmacist directly. In the event that the pharmacist is unable to process a claim electronically, employees can submit a paper claim to Manulife. Private Duty Nursing to an annual per person maximum of $50,000 Speech Pathologist pays reasonable and customary charges for the services of a certified speech pathologist up to a maximum of $500 per person per calendar year. Osteopath or Chiropodist - reimburses to a maximum of $100 per person per calendar year, plus up to $25 for related x-ray examinations. Podiatrist - reimburses to a maximum of $100 per person per calendar year, plus up to $25 for related x-ray examinations. Psychologist Services reasonable and customary charges - maximum of $1,000 per person per calendar year. Massage Therapy - reasonable and customary charges - maximum of $400 per person per calendar year. Physiotherapy / Athletic Therapy - reasonable and customary charges - maximum of $1,000 per person per calendar year. Chiropractor Services - reasonable and customary charges - maximum of $400 per person per calendar year. Naturopath Services reasonable and customary charges maximum of $200 per person per calendar year. Homeopath Services - reasonable and customary charges maximum of $200 per person per calendar year. Vision - reimburses expenses for prescription glasses (including prescription sunglasses) and/or contact lenses, as well as repairs and laser vision correction (also known as laser eye surgery), up to a maximum of $500 per person per 24 months. Laser Eye Surgery Lifetime per person maximum of $2500 Eye Examinations - One routine eye exam every two years for employees and dependents. Hearing Aids - $600 every 48 months per person. To claim reimbursement of eligible expenses, submit a claim form along with your original receipts to Manulife. Claim forms are available from your HR Services office or can be accessed via the intranet under Employee Central / Compensation, Pension & Benefits / Manulife Forms. Page 2 of 8

3 Health Care Benefits Travel Plan Employees are eligible for coverage as of first day of employment. Company pays 100% of the premium cost. Plan pays the cost of eligible medical and hospital expenses during the first 60 days of travel whether traveling for personal reasons or Company business up to a maximum of $1,000,000 per covered person. Travel coverage also applies to eligible dependents who are covered under the employees Extended Health Care (EHC) Plan. ManuAssist employees/eligible dependents traveling outside of Canada who require assistance as a result of an emergency illness or injury should contact ManuAssist at (Canada/US). In other countries use operator to call collect: Dental Plan Employees are eligible for coverage as of first day of employment. Company pays 100% of the premium cost. Reimbursement is based on the amounts allowed under the current provincial Dental Association Fee Guide of the province where the service was rendered. Recalls (checkups) every 9 months for adults (every 6 months for children under age 18). Plan pays 80% of the cost of basic services (preventative and maintenance dental care). Plan pays 75% of the cost of major restorative services (including implants) and certain in-office and laboratory charges to a maximum of $1,250 per person per calendar year. Plan pays 50% of the reasonable and customary charges for orthodontic services rendered on or after April 1, 2008, up to a lifetime maximum of $3,000 per eligible adult and dependent child. Dental claims can be submitted electronically to Manulife if your dentist has the necessary software. Electronic claims are adjudicated on-line in real-time. Manulife can reimburse either the employee or the dentist directly. The Dentist will indicate the applicable reimbursement arrangement on the claim form. If your dentist does not have the necessary software, you can submit a paper claim form to Manulife. Claim forms are available from your HR Services office or can be accessed via the intranet under Employee Central / Compensation, Pension & Benefits / Manulife Forms. 4 Page 4 of 8 Life and Accident Insurance Benefits Basic Life Insurance Coverage effective from first date of employment. Supplementary Life Insurance Company pays 100% of premium cost. Provides coverage equal to 100% of basic annual salary rounded to the next higher multiple of $250, if not a multiple thereof. Coverage ceases upon termination/retirement of employment (unless in receipt of LTDI benefits) Conversion option available upon termination (i.e., option to continue coverage at commercial rates without a medical exam). Coverage effective from first date of employment. Premium cost shared with employee paying 5/6 and CNL paying 1/6 of cost. Provides coverage equal to 100% of basic annual salary rounded to the next higher multiple of $250, if not a multiple thereof. Coverage ceases upon termination of employment. Coverage can be continued at the same group rates if employee is eligible to receive a PSSA pension upon termination of employment. However, for retired employees, benefit coverage reduces 10% for each year the insured is beyond age 65. Conversion option available upon termination (i.e., option to continue coverage at commercial rates without a medical exam). Company continues to pay its share of premium for retired employees. Optional Life Insurance Available to employees and their spouse at group rates in units of $10,000 to a maximum of $250,000 each. Optional Accidental Death & Dismemberment - available through AON Consulting. Optional Accidental Death & Dismemberment benefit also available through this plan. Employee pays 100% of premiums which are deducted through payroll. Applications are available from your HR Services office or can be accessed via the intranet under Employee Central / Compensation, Pension & Benefits / Employee Benefits. Employees can opt to enroll or renew coverage under this plan during the period from October to early December of each year. An Information Notice is issued each fall announcing the availability of the plan. Employee can purchase the single or family plan. Minimum Benefit is $5,000 - maximum benefit is $350,000. Employee pays annual premium directly to the insurer. Coverage is provided on a 24-hour, 365-day basis in the event of accidental death or injury resulting in dismemberment, paralysis, loss of sight, speech and hearing.

5 Disability Income Protection Short Term Disability (STD) Provides employees with 100% basic salary for absences due to illness or nonoccupational injury. New employees are credited with short-term sick days on date of hire and receive an additional credit on April 01 following date of hire. (Amount credited on date of hire and on April 01 following date of hire will vary based on hire date, location and provisions of collective agreement.) On April 01 of each year thereafter, employees receive a credit of 6 days. Unused short-term sick leave credits are cumulative. Cost is fully funded by CNL. Intermediate Term Disability (ITD) Long Term Disability Insurance (LTDI) Provides employees with additional disability income after they have exhausted their short-term sick leave credits. The benefit amount is 75% of basic salary up to a maximum of 26 weeks (182 calendar days). Benefit is reinstated after a return to work of two weeks (10 working days) in the case of relapse or 1 day if a new disability develops. Cost is fully funded by CNL. Coverage effective from first date of employment. Employees who are disabled after their short-term and intermediate term disability benefits run out may apply for LTDI. LTDI benefits provide disability income equal to 66 2/3% of the claimant s predisability basic monthly pay up to a maximum of $24,000 per month. Benefits are payable for up to 2 years if unable to perform the essential duties of own occupation. Benefits are payable beyond 2 years if the claimant is totally disabled from performing the duties of any occupation commensurate to the claimant s education, training or experience. Benefits are offset by income received from PSPP, CPP or other government plans. LTDI benefits are not payable beyond age 65. Benefits are increased to reflect increases in the Cost of Living (COLA) to a maximum of 2%. Coverage ceases at age 65 (or earlier if the employee elects to cancel coverage because he/she has 25 years pensionable service). The premium cost of LTDI benefits are cost shared at 50%. 6 Public Service Pension Plan For Employees of IAM; USWA Local 7806; UA Local 254, and SPEA Hired on or before September 13, 2015 Public Service Pension Plan (PSPP) Employees who are hired on a regular part-time (working an average of at least 12 hours per week), or on a full-time basis for a term of at least 6 months, are required to contribute to the Public Service Pension Plan (PSPP), commencing first day of employment. This plan is governed by the Public Service Superannuation Act (PSSA). This Act, which provides pensions for Public Service employees, has been in effect since January 1, The required contribution rates are determined annually by Treasury Board. For 2017 and for employees who became plan members prior to January 01, 2013, the employee pays 9.47% on pensionable earnings below the maximum salary covered by CPP/QPP, and 11.68% on earnings above CPP/QPP maximum salary. CNL pays 1.01 times the Employee s contribution on pensionable earnings below the maximum earnings of $163,100 allowed by Canada Revenue Agency for 2017, and 7.74 times the Employee s contribution on pensionable earnings in excess of the maximum. For employees who become plan members on or after January 01, 2013, the employee pays 8.39% on pensionable earnings below the maximum salary covered by CPP/QPP, and 9.94% on pensionable earnings above CPP/QPP maximum salary. CNL pays 1.00 times the Employee s contribution on pensionable earnings below the maximum earnings of $163,100 allowed by Canada Revenue Agency for 2017 and 7.74 times the Employee s contribution on pensionable earnings in excess of the maximum earnings. The maximum salary for CPP/QPP for 2016 is $55,300. Page 6 of 8 Participation in the PSPP ceases on January 01 st following the plan member s 71 st birthday. Defined benefit plan; both contributions and benefits are integrated with CPP/QPP. Prior to age 65, the pensions are calculated as follows: 2% X number of years of pensionable service (maximum of 35 years) X average salary for 5 consecutive years of highest paid service. At age 65, or in case of disability, the Public Service Pension is reduced to take into account the CPP/QPP pension. Pensions fully indexed annually to take into account increases in the cost of living since termination date. Return of Contributions (ROC): payable to plan members who leave the Public Service with less than two years of pensionable service. The employee is refunded his/her share of the contributions, plus applicable interest. Employer s contribution remains in the plan, i.e. it is not refunded to the employer or the employee. Vesting normally occurs after accruing 2 years of pensionable service.

7 Public Service Pension Plan For Employees of IAM; USWA Local 7806; UA Local 254, and SPEA Hired on or before September 13, 2015 Pension Benefits for employees who became plan members prior to January 01, 2013: Immediate Annuity: payable at age 60 with at least two years of pensionable service, or at age 55 with 30 years of pensionable service, or at any age if retirement is due to permanent disability and the plan member has two years of pensionable service. Deferred Annuity: payable at age 60. Annual Allowance (reduced pension): payable as early as age 50. Transfer Value (termination of employment before age 50): lump sum value representing the present value of deferred pension; it must be transferred to another registered pension plan or to a locked-in retirement savings vehicle. Pension Benefits for employees who became plan members on or after January 01, 2013: Immediate Annuity: payable at age 65 with at least two years of pensionable service, or at age 60 with 30 years of pensionable service, or at any age if retirement is due to permanent disability and the plan member has two years of pensionable service. Deferred Annuity: payable at age 65. Annual Allowance (reduced pension): payable as early as age 55. Transfer Value (termination of employment before age 55): lump sum value representing the present value of deferred pension; it must be transferred to another registered pension plan or to a locked-in retirement savings vehicle. Survivor Benefit: 1% X number of years of pensionable service X average salary of plan member; children s allowance is equal to one-fifth of the survivor benefit (maximum of four-fifths). Plan members can add eligible prior service in order to increase their pension credits by buying it back or by transferring pension credits under another plan through a Pension Transfer Agreement (PTA).
