It is important that you take the opportunity during Open Enrollment to evaluate your personal situation and enroll in the benefit coverage that best helps you manage your health and wellness. 3 Schneider Electric Flexible Benefits Plan Schneider Electric recognizes that each employee has different needs. A healthy single person may have fewer health and dental expenses than an employee with a family. That is why Schneider Electric offers a Flexible Benefits Plan ( Flex Plan ) designed to meet your needs today and in the future. The Flex Plan encourages you to take ownership of your benefits decisions and select the coverage that is appropriate for you and your family. The following benefits are Company-paid: Short-term Disability Basic Life and Accidental Death and Dismemberment (AD&D) Insurance Out-of-province/Emergency Medical You may elect the following coverage for a cost: Health Care Dental Care Long-term Disability Optional Life Insurance for you, your spouse, and/or child(ren) Eligibility You are eligible for the Flex Plan if you: Are actively working at least 20 hours each week (employees working less than 24 hours a week are not eligible for disability coverage), Are continuously employed at least as long as the waiting period, Are a resident of Canada, Have Provincial medical coverage, and Are not a union employee covered by a collective bargaining agreement. Your eligible* dependent(s) may have coverage if he/she: Note: Temporary employees may be exempt from all benefits and should refer to their offer agreement. When Coverage Begins Your coverage will become effective first day of hire for all benefits except short-term disability. To be eligible for short-term disability you must have 3 months of continuous service. Enrolling in the Flex Plan If you are a new hire, you have 31 days from your date of hire to enroll online at You will be responsible for choosing your coverage status, health and dental options, and other optional benefits. If you do not enroll within 31 days from your date of hire, you will be enrolled in Option 1 with Single coverage for both Health and Dental, and excess flex credits will be allocated to the Health Spending Account (HSA). The default for long-term disability is Option % coverage. During the open enrollment period, you have the option to change your benefit elections within specified dates. If you are a Quebec resident and you wish to opt out of health and/or dental coverage, you will be required to provide proof of comparable spousal coverage within 31 days of your date of hire, life event, or during the open enrollment period. If proof of comparable coverage is not received within the specified time frame, you will be in enrolled in the default coverage detailed in this guide. To submit your documentation to PeopleLink, please create a case in the Self-Service portal under category Benefits > Data Entry - Opt out (CA). Note: Open enrollment occurs annually. Is a resident of Canada, and Has Provincial medical coverage *Please refer to the Sun Life booklet for detailed definitions of these dependent(s). Note that if your child is 19 years or older and attending school, a Declaration Dependent Child/Full-time Student form will be requested annually. If the school he or she is attending is out of country, approval from Sun Life is required to maintain coverage. Refer to the How to Enroll job aid to initiate the approval process. 4 Qualifying Life Events If you experience a qualifying life event during the year, you must contact PeopleLink within 31 days of the event in order to make changes to your benefit coverage. PeopleLink will need proof of your life event and the date it occurred in order to open the enrollment tool on for you to make updates to your benefit elections and/or add a dependent. Qualifying life events include: Qualifying Life Event Marriage or common-law partner for 1 year or more New child (birth or adoption) Divorce or separation Spouse involuntarily loses coverage (Does not include spouse electing to change or cancel coverage) Documentation Required Marriage certificate Signed, notarized declaration form Mail that shows a date of at least one year ago, with the common- law partner s names and the member s address Birth certificate or hospital child registry (must be an official document with the child s name and date of birth) Legal adoption document (must show employee as legal guardian) Separation certificate Divorce certificate Letter written jointly by member and spouse/commonlaw partner indicating the date of separation Confirmation from spouse s employer with coverage termination date Reminder: You should review all your coverage and ensure your beneficiary designations are up-todate for life insurance and pension benefits. If you add a dependent after 31 days following a qualifying life event, you will have to complete a Health Questionnaire Dependent Group Coverage form and receive approval from Sun Life for your dependent to obtain coverage. However, you can remove an eligible dependent at any time, without proof of an event. Refer to the How to Enroll job aid for more details on enrolling in your benefits. Employee and Family Assistance Program (EFAP) As part of our Wellness program, you or your eligible dependents can call Homewood for confidential assistance at any time during your employment with Schneider Electric. 5 Health and Dental You and Schneider Electric share the cost of health and dental coverage. Schneider Electric will provide you with flex credits to help pay for your coverage. Health Coverage Type Option 1 Option 2 Option 3 (Default Plan) Prescription Drugs* Pay Direct Card 50% 80% 90% Generic drugs If there is a lower-priced generic drug that is equivalent to a brand name drug, the eligible amount the Plan will reimburse you is based on the cost of the generic drug Prior Authorization For certain drugs, you will need to obtain a prior authorization from Sun Life in order for your claim to be eligible for reimbursement. You can obtain a list of drugs under this program by accessing Supplementary Health 50% 90% 100% Semi-private Hospital 0% 90% 100% Out-of-province/Emergency Medical Coverage Vision Care (i.e., lasersurgery, prescription glasses, contact lenses) 100%; $1,000,000 lifetime maximum for you and each insured dependent 100%; $1,000,000 lifetime maximum for you and each insured dependent 0% 90% $250 maximum every 12 months for each dependent child under age 18 and every 24 months for you and all other insured dependents 100%; $1,000,000 lifetime maximum for you and each insured dependent 100% $300 maximum every 12 months for each dependent child under age 18 and every 24 months for you and all other insured dependents * Dispensing fee cap: Flex Plan pays 100% of the dispensing fee up to $6.50 per prescription. If you choose the lowest level of coverage (Option 1), your flex credits will cover the monthly cost. If you have flex credits left over, you can transfer your excess flex credits to: Your Health Spending Account (HSA) to pay for expenses that qualify as a medical expense under the Income Tax Act (Default) The Group Registered Retirement Savings Plan (RRSP). If you select this option, you must complete your enrollment through Great-West Life. (Refer to page 10 for enrollment details.) If you choose higher levels of coverage (Options 2 or 3), your flex credits will pay part of the monthly costs. You will be responsible for paying the difference through payroll deductions. Important Things to Consider When Making Your Election This is your opportunity to protect yourself and your family from the high cost of health and dental claims. Make sure you elect the coverage you would need if an accident or illness were to occur. Will you need Supplementary Health Care coverage such as massage therapy or chiropractic treatments? If yes, keep in mind the claim is subject to a reimbursement level (% coverage per claim) as well as a calendar year maximum. See page Q-2 of your Sun Life booklet for details. You are locked into your option (including opt out) until either the next open enrollment period or qualifying life event, provided you are not on a leave of absence. Do you have a spouse that has coverage? Did you know you can coordinate coverage between the two plans? For more details see page T-6 in your Sun Life booklet. If you are on a leave of absence, such as disability or personal leave, you can t make a change to your coverage until you return to work. When making your election, ask yourself what coverage you would need in the event that you become disabled. Keep in mind that you will not be able to change coverage while you re on a leave of absence, even if you no longer have coverage through a spouse s plan. Have you read your Sun Life booklet? 6 Health and Dental (cont.) Dental The Dental Plan covers dental care for you and your covered dependent(s). It includes preventive care like exams and cleanings. Options 2 and 3 cover more services and have a higher annual limit. 7 Health Spending Account (HSA) If you have excess flex credits you can enroll in an HSA. It can cover expenses that aren t covered by your group health or dental benefits. Please refer to your Sun Life booklet for details regarding what can be claimed, per the Income Tax Act. Contributions You have the option to deposit your excess flex credits into an HSA Eligible Dependent(s) You, your spouse, and any other dependent (e.g., a child, grandchild, or parent) for whom you are claiming a tax deduction in the taxation year in which the expenses were incurred Eligible Expenses Any expense that qualifies as a medical expense under the Federal Income Tax Act Plan Year January 1 December 31, 2016 Claim Submittal Deadline 60 calendar days from December 31 for prior plan year expenses Expiration Funds must be claimed within two benefit years, or you lose them. Sun Life Financial In this example, the employee has elected Option 1, Family Coverage for medical and dental. You will have $96.64 in excess flex credits deposited into your 2016 HSA each month. Activity Claims/HSA Balance Time Line Claims Submitted HSA 2016 balance deposited $96.64 per month* ($1, per year) 2016 HSA expenses $1,000 Purchased and submitted during 2016 (eligible out-of-pocket medical and dental claims submitted for reimbursement) HSA balance as of December 31,2016 (after submitting 2016 expenses) $ Vision claim incurred on December 27, 2016 (submitted towards 2016 HSA balance) 2016 HSA balance after submitting the vision expense 2017 medical expense (incurred on August 10, 2017) * Your flex credits are available when they are posted to your HSA on a monthly basis. $100 Submit February 29, 2018 (Deadline to submit claim is within 60 days of the end of the plan year.) $59.68 Remaining HSA balance to be used for 2017 eligible expenses $59.68 Submit claim towards 2016 HSA balance. (If not incurred by December 31, 2017, this amount is forfeited. You have until February 29, 2018 to submit 2017 claims). Out-of-province/Emergency Medical Coverage (Company-paid) This covers you and your eligible dependent(s) for all services and supplies eligible under your group benefits plan, if you have a medical emergency while you re travelling outside the province where you live. Note: If you opt out of medical and/or dental coverage, you will only have Out-of-province/Emergency Medical Coverage. Short-term Disability (STD) Insurance (Company-paid) STD Insurance provides income protection if you become disabled and are unable to work for five or more consecutive business days due to a non-work related illness or injury up to 26 weeks. You automatically receive coverage based on your years of service. The amount of your STD payment is determined based on your years of service and is shown in the chart below: Tax Status Waiting Period Benefit Period Monthly Employee Cost STD income is taxable Three months of continuous service Up to a maximum of 26 weeks (based on your years of service) None. This benefit is Company-paid Acclaim Ability Management, Inc. *The maximum benefit payable is equal to what is payable as a maximum under the Employment Insurance benefit. Please review the Short-term Disability policy for detailed information including your role and responsibility and how to file a claim within the first 10 days of disability. Long-term Disability (LTD) Insurance (Employee-paid) LTD insurance ensures a source of continuing income for disability lasting longer than 26 weeks. You have the option to elect LTD coverage at either 66.67% (default) or 60% of your pre-disabililty monthly base earnings up to $11,000. Benefit Amount Option 1: 60% of monthly earnings to maximum of $11,000 Option 2: 66.67% of monthly earnings to maximum of $11,000 (Default Plan)* Tax Status Nontaxable; sales tax applies in Ontario and Quebec Definition of Disability 24 months own occupation; any occupation after 24 months Elimination Period 26 weeks Benefit Period To the end of the month in which you reach age 65 Termination of Insurance 65th birthday, or date of retirement, if earlier Monthly Employee Cost 60% Coverage: $0.599 per $100 of benefit 66.67% Coverage: $0.650 per $100 of benefit Example Calculation Employee with a monthly base salary of $7,166 (annual base salary $86,000/12) elects option 2 (66.67%). Monthly benefit is $4,779 ($7,166 * 66.67%). Monthly premium is $31.06 ($4,779/100 *$0.65). Sun Life Financial *Please note if your annual base earnings are $220,000 or greater, you would not benefit from electing 66.67% coverage level due to the $11,000 monthly maximum benefit. 8

9 Life and Accidental Death and Dismemberment (AD&D) Insurance Life and AD&D Insurance provides financial protection in the event of a death or accident. This benefit is Company-paid; however, it is considered taxable income for tax purposes Sun Life Financial Optional Employee/Spouse Life Insurance (Employee-paid) To enhance your Basic Life coverage, you may elect Optional Life Insurance coverage to provide an additional benefit to your beneficiary(ies) in the event of your death. You may also elect Optional Spouse and/or Child Life Insurance to provide a benefit to you if your spouse and/or child(ren) should die. Note: Premiums for Employee/Spouse coverage are based on age and tobacco-user status. These premiums will increase January 1 of the following year if you and/or your spouse move into a new age bracket. Coverage for employee/spouse Optional Life Insurance is subject to approval by Sun Life. Group Registered Retirement Savings Plan (Group RRSP) When you enroll in the Group RRSP, the Company makes RRSP deductions from your salary and remits them on your behalf over the course of the year to Great-West Life (the Company s RRSP provider). Your immediate tax liability is reduced, since the tax you pay is based on your salary after your RRSP contribution. By contributing through payroll deductions, rather than contributing after you are paid, your net income increases providing you with more cash throughout the year.
