1 Submission of life certificate in respect of Retired staff/ Family pensioners 1. Submission of Life Certificate in respect of the retired staff pensioners and family pensioners falls due on 1 st November 2017 for further release of pension from the month of November 2017 onwards. 2. The branch wise list of staff pensioners/ family pensioners drawing pension is made available in intranet. Branches may access the same through Downloads -> HO-PAD -> Branch_wise_list_of_staff_pensioners. Pension paying branches are requested to take a print out of the same for information and follow-up purpose. 3. A format of Life Certificate furnished as Annexure-I, is also made available on the same path. Pension Disbursing Branches are advised to note that: 3.1 The Life certificate in respect of staff pensioner and family pensioners has to be submitted by the Pension disbursing branch in the prescribed format after certifying that the pensioner/family pensioner is alive. 3.2 No additional documents/photographs should be insisted from the pensioner/family pensioner. 3.3 In respect of staff pensioners/family pensioners who are not in a position to visit the branch due to disability/illness, arrangements shall be made by the Pension Disbursing branch for visit of a branch to the place of local residence of such staff pensioners/family pensioners for certification. 3.4 The Life Certificate has to be furnished in duplicate and Branch s have to return one copy to the pensioner as acknowledgment for having submitted to Staff Pension Cell, PAD-HO. 4. All Family Pensioners have to submit Non Marriage/Re-Marriage declaration in the Format enclosed as Annexure-II and Son/daughter drawing Family Pension has to submit Employment/Non-employment Certificate in the format enclosed as Annexure-III

Branches are advised to furnish a scanned copy of the duly completed Life Certificate, Non Marriage/Re-Marriage declaration and Employment/Nonemployment Certificate in respect of the concerned pensioner/family pensioner to our mail id pension@corpbank.co.in. Last date for receipt of certificates/declaration has been fixed as , and payment of pension shall be stopped from in the cases where Certificate/declaration is not received. 4 Annexure-II Declaration by Family Pensioners (To be submitted once in a year in November along with Life Certificate) CERTIFICATE OF NON - MARRIAGE/ RE-MARRIAGE (In case of Widow/Daughter eligible for Family Pension) I hereby declare that I am not married/i have not been married during the past one year. 1. :. 2.Name of the Family Pensioner: 3.Employee Number.. 5 Annexure-III Declaration by Family Pensioners (To be submitted once in a year in November along with Life Certificate) EMPLOYEMENT/NON- EMPLOYEMENT CERTIFICATE (In case of Son/Daughter eligible for Family Pension) I declare that I have not received any remuneration for serving in any capacity in an establishment of the central government or a State Government or government undertaking or from local fund till date.
