Provided by UA-PTC 1 times your Annual Earnings, rounded to the next higher multiple of $1,000, if not already a multiple of $1,000.Max is $50,000. Age Of Member VOLUNTARY LIFE INSURANCE DEPENDENT LIFE INSURANCE Fulltime employees that fail to enroll within the first 30 days of employment may be denied insurance by the carrier. Evidence of insurability may be required by The Standard for additional coverages. Fulltime employees that fail to enroll within the first 30 days of employment may be denied insurance by the carrier. Children are covered at 50% of the spouse amount selected. 70 through 74=65% 75 or over=50% Choices: 1, 2, 3 or 4 times your Annual Earnings. 3 SHORT TERM DISABILTY BUY UP OPTION LONG TERM DISABILITY Fulltime employees whose salary is more than $45,000 annually. Effective the 1st day of the month following your hire date. Rates based on employee salary amount. 100% Paid by UA-PTC OPTIONAL LONG TERM DISABILITY Available to employees earning more than $20,000 annually in addition to the Basic Long Term Disability provided by the College. 60% of salary (maximum monthly benefit of $5,000) OPITIONAL ACCIDENTAL DEATH AND DISMEMBERMENT CAFETERIA/125 PLAN A 125 program is an opportunity to increase your spendable income by reducing your taxable income. WageWorks-Flex One (800) Effective the 1st day of the month following your hire date Open enrollment annually in November Health and Dental Insurance Premiums Childcare Unreimbursed Medical You may choose coverage for yourself in $25,000 increments (maximum of $300,000) not to exceed 15 times your annual salary. Family benefits covers your spouse at 60% of the employee amount and each child at 20%. When you authorize PTC to include you in the plan, certain expenses will be withdrawn from your check before taxes rather than after taxes. This lowers your Federal, State, and Social Security withholdings and thereby increases your spendable income. TIAA-CREF (800) Fulltime employees are required to enroll / participate in a Contributory PTC plan. Enrollment required in the first 30 day of employment Employee contributes 6% and UA-PTC contributes 10% Fidelity Fulltime employees are required to enroll / participate in a Contributory PTC plan. Enrollment required in the first 30 day of employment Employee contributes 6% and UA- PTC contributes 10%

The EAP provides confidential short-term individual counseling for employees as well as their family members who experience some form of personal distress. Full-time employees may take a maximum of 8 hours paid Children s Educational (prek-12th) per calendar year. Educational Activity means any school- sponsored activity. A Fulltime employee who serves as a juror or is subpoenaed as a witness to give a deposition in a court or hearing is entitled to receive normal and full compensation in addition to any fees paid for such service. Fulltime employees who are members of the National Guard or any of the reserve branches of the US Armed Forces are granted paid leave at the rate of fifteen (15) working days per calendar year, plus necessary travel time for annual training purposes. Must be paid in the calendar year leave taken. Upon hire Fulltime Faculty members receive; one personal day to use during the fall term and one personal day to use during the spring term. All scheduled academic recesses in lieu of annual leave. Fulltime Faculty Members accrue 8 hours of Sick leave each month from September May. Fulltime 12-month employees Accrued monthly, based on years of Full Time - Arkansas State employment: 0-3 years = 8 hours 4-5 years = 10 hours 6-12 years = 12 hours years = 14 hours Over 20 years = 15 hours SICK LEAVE Fulltime 12-month employees Fulltime 12-month employees accrue eight (8) hours of sick leave monthly
5 HOLIDAY PAY Extra-help employees that are in pay status the day before the holiday and at least 1 hour the day following the holiday will receive prorated holiday pay.
