4 A MINISTRY OF GOOD WORKS 3 Message from The President The Right Revd Rennis Ponniah Singapore Anglican Community Services A father to the fatherless, a defender of widows, is God in his holy dwelling. God sets the lonely in families, he leads forth the prisoners with singing. (Ps 68: 5-6a) 2014 sees the Anglican community services crossing-over to a new stage of serving our nation of Singapore. We are constantly guided by our ethos; our desire to mediate God s love to people in need, regardless of race, religion or socio-economic status. And we have steadfastly ministered compassion and hope to many disadvantaged and often overlooked in society. Our work has demonstrated certain consistent traits, guided by a God who loves the fatherless and the motherless, the widows, the lonely and those who are prisoners due to their circumstances. From the beginning, we have been pro-active; initiating many services. A moving tale is that of Mrs Francis Thomas, a nurse and wife of the St Andrew s School Principal, starting a clinic in their garage to help the poor villagers of Potong Pasir; a tale of how compassion gave rise to showing love in deeds. And we keep challenging ourselves to keep growing apace in compassion, depth and competency as we do not want to be utilitarian in the provision of our services. Increasing Our Capacity to Serve One of our traits is our desire to increase our capacity to serve. Our immediate priorities are to excel in mental health rehabilitation and provision of care to children and elderly. In this area, the Psychiatric Rehabilitation and Recovery Services have initiated a Community Service Team which aims to plug service gaps and help those who are unaware of how or where to obtain assistance regarding mental health treatment and to educate the public on the importance of mental wellness. Our Employment Support Services have also started the Employment Training Programme that systematically trains clients to be ready for the competitive job market. Our CITY Community Services was invited to open its third Student Care Centre, a testimony to the good work and dedication of the team. It is heartening to see the settling up of the Befrienders Alumni as the work extends beyond the time frame of when the child is in the school. Continual Commitment to Care and Grow Caring for the entire person has been part of the DNA of SACS since the very beginning. Today all our services have chaplains and their associates to care for the emotional and spiritual well-being of the patients, clients and staff. All our services are serving at almost full capacity. Our staff members have conceived many initiatives to improve our service delivery to our client-members; from peer support groups at HCC, SCC and ESS to creative outlets (the Hub at HCC and lunch concerts). PEACE- Connect Seniors Activity Centre and our Family Care Centre continue to deliver the much needed personal touch so needed to the elderly and the women and children who are dealing with family violence. Engaging Heart and Minds SACS will keep striving, desiring the best for our society because of our holistic approach to minister to the body, mind and spirit. Hence we want to have integrity in how we conduct our affairs, mediate compassionate love for our clients and their family, and exercise self-control and commitment to justice and compassion. Our Ministry of Good Works ultimately promotes the well-being of our society. This is SACS calling. So I commit this Annual Report to you to read and be encouraged by how God has grown the services from tiny seeds of compassion since the 1950s. With grateful hearts We are committed to growing the work in the years to come and grateful for the doors which have opened. SACS is looking at setting up a new service at The Hillier, a retail mall and adding a fourth CRSS centre located in central Singapore It is because of the trust and support from Ministry of Health (MOH), Ministry of Social and Family Development (MSF), the National Council of Social Service (NCSS), the Agency for Integrated Care (AIC), grass-root organizations and leaders, other VWOs and corporations that we are able to do what we do. I want to thank and appreciate you for being our partners. I also want to thanks all our volunteers and those who serve on our Management Committee and Board of Directors for the unselfish giving of their time and talent. 5 Psychiatric Rehabilitation and Recovery Services Our core values are Integrity, Teamwork, Professionalism, Innovation and Compassion. And 2013 saw us build on different aspects of these core values as we continue to work together across the six services to improve cooperation and streamlining our provision of services to the community. Hougang and Simei Care Centres Both Hougang Care Centre (2002) and Simei Care Centre (2005) continue to provide quality psycho-social rehabilitation services and care for our member-clients who have mental illness. HCC and SCC complement the continuum of care provided by our sister services CRSS and ESS. HCC has a residential capacity of 178 with SCC s capacity being 156. Our average occupancy has been steadily rising throughout the year from 86% to almost 97%. HCC had 32 clients while SCC had 84 clients enrolled in the Day Rehabilitation Programmes. Together, we are able to impact the mental landscape by helping our members achieve meaningful life roles and regain their life skills so that they can go on to independent living within the community with minimal support. Both Centres have different approaches to the rehabilitation process. HCC has adopted an adapted Clubhouse Model of psychosocial rehabilitation which focuses on the strengths, interests and needs of the individuals. It offers opportunities for members to engage in meaningful activities that help them develop independent living skills so that they can eventually be reintegrated back to the community. SCC incorporates the principles of psychiatric rehabilitation. Each member has an identified case manager who provides both case management and counselling services to the member and the caregiver. In addition, our members participate in our rehabilitation programs, such as psycho-education and life skills training, in order to improve themselves further. New Services and Initiatives Occupational Therapy For this year, SCC welcomed the addition of an occupational therapy team to our services. Consisting of two occupational therapists, the team is working alongside the other professionals in our multi-disciplinary team to provide specialized services to our members. Both our occupational therapists are currently involved in training our residential members in handling their activities of daily living such as personal hygiene and also in preparing our members for employment. The Arts On Jan 13, the Enterprise Hub was officially launched at HCC. The hub provides members who are interested in craft work such as painting, handicraft and sewing to put their skills to good use. The items created by our members will be exhibited and sold at various roadshows. SCC introduced art therapy in June to our members. Art therapy uses the creative making of art as a form of self-expression so as to achieve mental wellness. Currently we have approximately eight members who are involved in the group sessions and a further two members in the individual sessions. Peer Support Groups In Jan 13, we started two Peer Support Groups in HCC - one on Tuesday nights and another on Wednesday mornings. They are facilitated by Mr Harris Ng who has sustained his personal recovery from mental illness for more than three decades. The support groups are meant to inject hope through the positive stories shared by the members so that they can learn from each other to work on their recovery process. SCC has also started Peer Support Groups and currently has three different groups running. Supported Housing At SCC, our residential services have been actively involved in supporting our members when they move out of the centre to stay in the community on their own. For this year, we have started to identify, at an earlier stage, members who can potentially partner each other to stay in rental flats. These paired members are then prepared by being placed in our specially prepared rooms that resemble the layout of a one-room rental flat so that they can learn how to care for each other from start. 4

6 Significant milestones Lunchtime Showcase At SCC, a group of members have made it a point to showcase their talents to the general public on a regular basis by staging lunchtime concerts at the cafeteria monthly. Apart from singing contemporary pop songs and old-time favourites, we have members skilled in playing a variety of musical instruments such as the guzhen, flute, keyboard, guitar and ukulele. Volunteers Since the start of 2013, 12 employees from the Standard Chartered Bank have been coming to volunteer their time regularly with our members in the rehabilitation workshop. Members Reaching Out Our members have been actively reaching out to the underprivileged in the community. For this year, our youth members from the YES Club have actively volunteered with Orange Valley Nursing Home. Through engaging the elderly with songs and games, we hope that our younger members can also be reminded of their social responsibility and be empowered to help others in need. Prisons On 9 May, HCC was approached by Prisons Service to explore referrals from the Prisons Psychiatric Unit (PHU). The PHU provides rehabilitation for inmates who are diagnosed with mental illness. The purpose of this collaboration is to provide accommodation and rehabilitation for those who have no shelter and support upon their release from prisons. This is an important link to help them to continue their psychiatric follow up and also a structured environment to enable them to learn independent living skills. SANH On 14 Oct, HCC started our partnership with St. Andrew s Nursing Home (SANH). This collaboration provides opportunities for our members to have supported employment there. It first started with receptionist duties for four members. There are also other work opportunities such as laundry service, food service and health care assistance in the coming months. Community Rehabilitation and Support Service All the three centres in Bukit Batok, Yishun and Pasir Ris continue to consolidate the roll out of services to the community. CRSS Pasir Ris started their day programme this year with more than 30 registered, and a daily average attendance of 12. In our desire to expand to meet service gaps and needs in the community, we partnered with AIC to initiate a community support team. Community Support Team In order to develop an integrated community mental health programme, a Community Support Team, which is CRSS-led, SACS partnered with AIC to provide mental health support to individuals in the community who are mentally ill or at risk of mental health conditions. This non-clinical mental health programme will form the network of collective support in the community to promote mental health and reduce the mental illness burden in Singapore. The objectives of the CST are to: Increase awareness of mental health in the community through public education, Perform basic recognition of signs and symptoms of mental illness to facilitate early treatment for clients at-risk, Provide basic emotional support to clients and caregivers, Offer liaison services in navigating the mental health system, & Provide crisis management. The service commenced in September 2013 after much deliberations with a case manager in each of the three existing centres being assigned the responsibility of managing responses to referrals and attending to such cases. Overall programme direction is managed by the Head of CRSS Yishun. Staff Mr Tan Mingshen, CRSS Bukit Batok, was awarded a Health Manpower Development Programme grant to attend the PRA Conference in Atlanta, Georgia (USA) and to serve an attachment to Vinfen, Boston, USA. Community Partners CRSS works with many other VWOs and grass root organizations in other to serve the community. Here are some of the partnerships and outreaches to the community: CRSS Yishun in THRIVE with Khoo Teck Puat Hospital. Nee Soon East I Champs Mental Health Awareness Programme. Organised and hosted, together with SAMH and Club Heal, the Bukit Batok East National Day-cum-Hari Raya celebrations on 15 Aug. Guest of Honour was Mdm Halimah Yacob, Speaker of Parliament. Participated in Mental Resilience Day organized by Bukit Batok East Constituency on 20 Oct. Employment Support Services In 2013, the Employment Support Services helped 162 clients in their desire to seek sustained meaningful employment in the competitive job market. Monthly we have organized peer support meetings which are focused on building strong relationships among our clients, providing them with a platform to share experiences and support one another. This enables our clients to have ownership in their recovery process. Employment Support Services held its first graduation on 20th December for all Employment Training Programme participants from Batch 1-4. There was much merriment as they concurrently celebrated Christmas with good food, games and caroling. Employment Training Programme A new programme, funded by Mindset Care Limited, designed to be a structured and holistic training programme, was set up for our clients and their future employers. This programme runs over six weeks with 120 participants. The aim is to help people recovering from mental illness to be equipped through the training sessions with the essential skills and knowledge as part of the preparation to enter the workforce. PRSS Collaborations PRSS organised a series of events at Jurong Spring Constituency to commemorate World Mental Health Day on 6 Oct with special guests: DPM Tharman Shanmugaratnam and Min of State for National Development Mr Desmond Lee. The event pulled together several healthcare VWOs for this community outreach PRRS collaborated with HPB-AIC to commemorate World Mental Health Day on 27 October At Max Expo, ESS was invited to give a presentation; Employment Training, Journeying with Clients by Ms Irene Sng, our ESS, Director. PRRS participated in Enrich Your Mind Learning Festival, organized by AIC and HPB on 27 Oct to commemorate World Mental Health Day, held at the La-Salle College of the Arts. PRSS Staff Training PRSS invited Ms Helen Glover, an Australian mental health care expert, to train our staff in Realising Recovery Relevance in Service Delivery with the objective of ensuring our service delivery is relevant to supporting people in their recovery processes. Her three day workshop (30 Oct 1 Nov) saw participants from all our PRRS services and SACH, Singapore Association of Mental Health and Silver Ribbon. PRSS also invited Professor Hector Tsang, HMDP-ILTC visiting expert from Department of Rehabilitation Services, The Hong Kong Polytechnic University to conduct a three -day workshop from 7-9 November 2013 entitled Integrated Support Employment Training Programme for staff involved in mental health care. This workshop was equally well attended by industry personnel. Professor Tsang s delivery focused on some of the most important principles or concepts undergirding the fundamentals of integrated support employment. He provided the participants with an understanding on his new model of Integrated Supported Employment Program (ISE), which combines Individual Placement Support (IPS) with Social Skills Training (SST). He emphasized the significance of work-related social skills training and enriched participants learning through role play. 6

11 SACS Family Care Centre Introduction The SACS crisis shelter, the Family Care Centre, started in 1986 and has been a place of safe refuge for many women and children who experienced domestic violence and crisis. During their period of stay at the shelter, our dedicated team of nine staff members actively engages the clients to provide them the emotional support and practical help they need to restore their broken spirits, rebuild their confidence and have hope for their future. Our Case Workers also provide counseling for the children and guide them to make sense of their world that has been shaken by their parents decisions. It is a meaningful ministry and it is only made possible by the dedicated volunteers who work closely with the staff to make a difference in the lives of the residents of the shelter. Statistics In 2013, our crisis shelter has served on average 81 clients a month. We have provided services to 123 new clients. All of the volunteers have helped to create a lively community in the centre where residents get to socialize through participating in activities. The positive experiences have rekindled their hope for the future. Our special thanks go to the following in particular: St Peter s Church and Paya Lebar Methodist Church who have been faithfully conducting programmes for the children in the centre. 10

The Serangoon Community Club Youth Executive Committee (YEC) for organising the Science Explorer Camp. The Voluntarius (Singapore) for planning programmes and outings for the children. St Andrew s Cathedral for engaging the children of the centre in various activities. Wesley Methodist Church for organizing the outing to Sembawang Yacht Club with Wesley Skiers Paradise, and also their ministry of the Angel Tree and White Gift s Christmas Project. Renewal Christian Church for their Mother s Day Celebration for our mothers and organizing the S.E.A. Aquarium Outing Kum Yan Methodist Church for their monthly birthday celebration for our residents and organizing a Christmas Party for our residents. The Girls Brigade for organizing the Friendship Day Carnival. St. Andrew s Junior School for organizing the Parent and Child Football Games. The Diocesan Youth Board s SERVE programme interns who did their attachment with us. CCIS Christmas Wish for making us their beneficiaries. CapitaLand for their kind Christmas donations for children in the centre. St. Matthew s Church and Paya Lebar Methodist Church for organizing special Christmas programmes. Partnership with North-East CDC Provided free Chingay tickets to the residents who enjoyed watching the performances. Provided financial assistance to the women so that they need not worry about daily necessities and could focus on resolving their issues. Under the CDC s community employment programme, the Centre was able to provide temporary employment for three residents who eventually managed to find gainful employment. Provided free Moving Aid Services for the residents who are moving to their HDB rental flats. Provided funding for the Centre to engage a facilitator to conduct eight sessions for the women s support group. Enrichment Programmes Money Wags session by WINGS, a local VWO. Dare to Discipline by Focus on the Family Visitors The then Acting Minister of Social and Family Development, Mr Chan Chun Sing visited the Centre on 12 April Eight delegates from Thailand came to Singapore learn more about the management of Family Violence and visited our Centre on 19 June

13 PEACE-Connect Seniors Activity Centre PEACE-Connect Seniors Activity Centre (PCsac) 平安乐龄活动中心 is the community service arm of Holy Trinity Church, and is affiliated to Singapore Anglican Community Services. Situated in the Kampong Glam constituency, PCsac aims to promote purposeful community life among the residents living in the HDB rental units by providing recreational/cognitive activities and support services for the senior residents (above 60 years) living in the HDB one-room/two-room rental units of Blocks 4, 5, 6, 7, 8, 9, 10, 18 & 19 clustered at Kampong Glam. Today, PCsac, with a data base of slightly more than 1700 residents, is known to most residents as the provider of information and assistance; as the hub for senior-wellness and senior-care; as the info-link for senior-care workers from agencies and organisations. Let your light shine before men, that they may see your good deeds and praise your Father in heaven. Matthew 5 v 16 PEACE-Connect celebrated her 10th year of vibrant service in the Kampong Glam constituency. During the year-long celebration, efforts were put together by Holy Trinity Church, the Parent Organisation, the Centre herself and a cohort of benefactor supporters to commemorate this milestone; seven years of service as Shalom SAC and 10 years as PEACE-Connect SAC. As the Centre is dependent on the generous donations from the public, fund-raising efforts via a mail appeal, a Cantonese Opera performance and a Stroll-athon, yielded an income of close to 90,000. The most memorable event in 2013 was none other than the miracle of Granny Choong Foong Lun ( 钟红兰婆婆 ). Let me give her background first, born in 1913, Granny Choong became a Christian in the last decade and was baptized in Holy Trinity Church. A very independent woman, she chose to live on her own in a one-room rental HDB unit even though she had an immediate family of three grandchildren. An assertive woman, she managed to organize her own team of helpers for her daily living activities. A sociable woman, she used the marble table outside the Centre as her day station. In January Granny Choong asked the Centre Manager to celebrate her 100th Birthday on 4 June, and we were prepared to honour her request by seeking sponsorship. 9 April The Joy Truck team from Mediacorp did an on-site recce for their recording at the Centre, and they met Granny Choong and decided to throw her a 100th Birthday banquet on 19 April, as part of The Joy Truck programme, which was scheduled for telecast on her actual birthdate, 4 June. 15 April, Mediacorp started their filming and interviews. During one session, Granny expressed that her two greatest wishes were to be able to give her friends a treat and to receive a big hongbao ( 红包 ). 18 April Granny Choong suddenly fell ill and our in-house doctor went to visit her. Inspite of much persuasion, Granny Choong was reluctant to go to the hospital. 19 April We were surprised that Granny Choong was up early and all ready for her birthday party. All went as planned with Granny Choong eating well, singing loud and cheering all the way. 20 April When we checked on her in the morning, she had fallen ill. The SACH Mobile Clinic Day was at the Centre that day. And God favoured her with perfect timing via a joint visit by MP Mrs Denise Phua, Revd Lim Kek Wah, the Centre Manager and her senior staff, a Mobile Clinic doctor and a friend. 21 April Granny Choong went to the Lord with all her wishes and bonus-wishes granted by God. She had her grandchildren by her bedside, a joint visit by community people she had loved, a banquet for 99 of her village friends and two good-sized hongbaos. The whole episode could only happen because God allowed it as testimony of His Majesty. All of us at Holy Trinity Church and PCsac are blessed and encouraged to know that we are a part of God s divine work at Kampong Glam. We can only proclaim: To God be the Glory. 14 CITY Community Services Befrienders Clubs CITY served over 450 children in our 18 Befrienders Clubs in 17 government primary schools throughout Singapore. The 17 clubs were in Bukit View Primary, East Coast Primary, Eunos Primary, Fengshan Primary, Fuhua Primary, Gan Eng Seng Primary, Horizon Primary, Hougang Primary, Jurong West Primary, MacPherson Primary, Naval Base Primary, North View Primary, North Vista Primary, Seng Kang Primary, Townsville Primary, White Sands Primary, and Xishan Primary. The 18th club was Befrienders Sports Club in East Coast Primary. The two regular camps in our Befrienders program Orientation Camp, and Six2One camp saw a total of 180 participants. School-based Student Care We managed three student care centres - North View Primary School (Kidz Ark), Townsville Primary School (Happy Hearts) and Opera Estate Primary School (Kids Nook), with a total registration of about 230 students. Our contract with Townsville Primary School ended in Jan Holiday outings were organized for the students of the 3 centres in June and December. The students under financial assistance participated in the CCIS (Celebrate Christmas in Singapore) My Christmas Wish programme, and they were brought to Orchard Road to enjoy the Christmas display and collect their presents. Kids Nook, which started operations in January 2013, was officially opened by Ms Indranee Rajah, Senior Minister of State (Law and Education) on 22 July. Befrienders Alumni (BFA) The Befrienders Alumni reaches out to former Befrienders Clubs members who have graduated from Primary school. These young teenagers have joined various secondary institutions but continue to remain friends. The Alumni provides them with a sense of belonging and members are encouraged to return and serve as leaders and role models. We have partnered All Saints Church (English) congregation Youth group, and are grateful to Anglican High School for providing the venue where the BFA can meet regularly. The first BFA program was held on September 28. Activities are held every Saturday afternoon. Programmes include learning sessions in life skills (7 habits of effective people, anger management, money/time management), communication skills (public speaking, presentation skills), use of common computer applications. Active indoor and outdoor sports are regular features. Publications A special edition of Heartprints, commemorating our 5th Anniversary with a report on our journey to date has been published and distributed. Staff Staff strength in CITY was 21 and 5 part time staff of which 8 serve in the Befrienders Group and 18 serve in Student Care. Besides regular staff members, CITY also takes in Tributers and Servers as well as interns from tertiary institutions. Appreciation CITY was formed to build Character In The Young. This is very relevant and needed in today's society. We want to thank our steady and faithful volunteers and our donors, for coming alongside us. As many volunteers have testified, the satisfaction and joy of giving a helping hand, a listening ear, cannot be measured. We are most grateful to our volunteers, Servers and Tributers who have worked alongside us in

16 SAMH Service Review st. andrew s mission hospital 2013 marks SAMH s 100th year of service, and it has been a fruitful year of unprecedented growth and development for St. Andrew s Mission Hospital s (SAMH) respective services, with many initiatives strengthened and launched to serve the needs of Singaporeans. Most notably, SAMH marked this centenary year by launching its newest service (supported by the Singapore Anglican Community Services), St. Andrew s Nursing Home, a psychiatric nursing home providing long-term care for mentally-ill patients. It started operations in late November. On behalf of the Board, I am pleased to provide an update on SAMH and its four community services: St. Andrew s Autism Centre (SAAC), St. Andrew s Community Hospital (SACH), St. Andrew s Lifestreams (SAL), and St. Andrew s Nursing Home (SANH). St. Andrew s Autism Centre (SAAC) Following a two-year process to identify, develop and confirm SAAC s core values, 2013 was the year in which the Centre focused on communicating and cascading its values to all staff. Accompanying this process was the endorsement of SAAC s revised Vision and Mission statement by its Management Committee. SAAC s renewed vision is: To be a light in a dark place, enabling people with autism to lead dignified and meaningful lives. Its mission is: To provide quality education, training, care and support, characterised by Christian love and compassion, to people with autism and their families. The year was eventful for the Centre particularly in its efforts in autism and special needs advocacy, in deepening its relationship with partners and supporters, and its support of SAMH s Centenary Celebrations. Enrolment The total enrolment at the Centre in 2013 was 194 (151 students; 43 clients), an increase of about 14% from the previous year. SAAC s staff strength grew by 9.3% that year, reaching a total of 118 staff by the end of the year. Altogether, six new teaching staff and four new coaches were added to its two respective services: St. Andrew s Autism School (SAAS) and the Day Activity Centre (DAC) which is run by St. Andrew s Adult Autism Services, to support the steady growth of students and adult clients. Events & Milestones Significant events and milestones in 2013 included: World Autism Awareness Week (WAAW) 2013 campaign where SAAC was the organising committee co-chair; ABBA Autism Awareness Charity Run, an inaugural fund-raising and family event; SAMH Centenary Tree Planting and Fun Walk at Elliot Road; NCSS SSTI Showcase and Launch; SAAC Cascading Core Values Workshops; SAAC 8th Anniversary and Thanksgiving; The Purple Parade, an inaugural special needs campaign with SAAC as a key participant; OCBC Christmas Charity Fair to showcase products by DAC s adult clients, as well as the CCA Christmas Fair, with increased support from more sponsors. St. Andrew s Autism School (SAAS) In 2013, teaching and learning structures, and processes were further refined to enable its students with moderate to severe autism to achieve meaningful learning goals. The school also underwent an external review to validate its practices with reference to the MOE- NCSS Quality Assurance Framework for Special Education Schools. Significant achievements and milestones included the first ever participation in the Special Olympics Singapore National Games 2013, which saw seven students winning five medals, as well as the MOE-SAAS Overseas Study Trip on the support of students with severe autism. St. Andrew s Adult Autism Services Day Activity Centre (DAC) We welcomed Mr Raymond Marcel Semaun, who joined the St. Andrew s Adult Autism Services as its Head of Service. The Adult Autism Services further strengthened its staff capacity building with four DAC staff members being certified as Associate Non-violent Crisis Intervention instructors. In October, we marked the official launch of SAAC cafeteria with the President of SAMH and SACS, The Right Reverend Rennis Ponniah, as Guest-of-Honour. The cafeteria is operated by DAC s adult clients in the Food & Beverage and Retail track of the Vocational Skills Training programme. 15

17 Key highlights for the DAC included: The cultivation and provision of small plants as souvenirs for SAMH's Centenary Tree Planting and Fun Walk event by clients in the Horticulture Vocational Skills Training programme. The first ever participation in the Special Olympics Singapore National Games 2013, with three adult clients winning five medals. The placement of one adult client in a supported-employment pilot project at a fast food restaurant. Opportunities to promote products by DAC s adult clients at the inaugural Netherlands Charity Association Fair and The Purple Parade. Parents Support Group (PSG) At the PSG, bold initiatives to lead and encourage parents and families to bond with their child or family member with autism in 2013 included equipping and supporting 33 students/clients and their family members to travel to Phuket, Thailand via air travel. In addition, SAAC organised the PSG Siblings Camp for the third year running, attended by 20 children between eight to 14 years old, with a sibling diagnosed with autism. SAAC also organised the inaugural Siblings Connect, attended by 16 youths and young adults with siblings with autism, providing them with strategies to communicate more effectively with their siblings. A family trip for 52 students and adults clients from the lower income group, as well as a year-end family gathering for all PSG members was also organised during the year. St. Andrew s Community Hospital (SACH) In 2013, SACH continued to expand its range of services (such as the new Dementia Ward, new Sub-acute Ward and expanded Home Care Programme) in response to the needs of the ageing community. SACH also planned for the development of new services through The Integrated Building, which is scheduled to be ready at the end of Workload Statistics SACH s inpatient rehabilitation and sub-acute care are currently being provided through 172 beds across six wards. The average bed occupancy for operational beds was 92% (an increase of 11% compared to 2012), and there were 1,554 admissions (an increase of 15% compared to 2012). Day Rehabilitation Centre attendances increased to 13,551 (an increase of 28% compared to 2012), and Outpatient Clinic attendances numbered 4,309 (an increase of 47% compared to 2012). There were also 4,102 Home Care visits (an increase of 136% compared to 2012). The Integrated Building Optimising Rehabilitation and Independent Living SACH and Changi General Hospital (CGH), as members of the Eastern Health Alliance, have been working together to prepare The Integrated Building (IB), the first-of-its-kind healthcare facility in Singapore focused on optimising rehabilitation and independent living for patients recovering from injury, illness, or surgery. SACH will be operating three wards (96 beds) out of nine wards, as well as relocate the SACH Day Rehabilitation Centre, Home Care and Medical Social Work Departments to the IB. Dementia Ward The Dementia Ward is a 22-bed ward at Level 4 which commenced operations in October It was intentionally designed to provide a calming, home-like, dementia-appropriate environment for the rehabilitation of persons with stroke or fractures, but with concurrent moderate to severe dementia. The project was funded by the NR Mistri Fund, with support from the Community Silver Trust and MOH. The care model is anchored in the concept of person-centred care supported by a dementia-friendly physical and social environment. Rehabilitation (physiotherapy, occupational therapy and speech therapy) and social programmes are specially designed to provide meaningful activities to engage the person so that rehabilitation is optimised. Sub-acute Ward SACH has tied up with CGH s Geriatric Medicine Department to run a 16-bed Sub-acute Ward at Level 6. This will expand to 32 beds in CGH geriatric patients who are past their acute phase of illness, but still require continued medical care, are transferred to this Sub-acute Ward. These sub-acute care patients are managed by a multidisciplinary team of SACH nurses, therapists and medical social workers, with medical care provided by doctors from CGH Geriatrics Department. There is full specialist referral support from CGH available to these patients. The cost of expensive scans and treatments are subsidised by SACH to keep the cost affordable to patients. This collaborative service has helped to increase the scope of expertise at SACH beyond the current strength in rehabilitation, as well as strengthen the sub-acute care capabilities of SACH to cater to increasing numbers of patients who require a subacute inpatient stay at the community hospital. This also benefits CGH in that more of their bed capacity can be channelled towards acute care patients. Adult Home Care Services The SACH Home Care Service (HCS) is a nurse-led, case-managed clinical care programme that coordinates and provides home nursing, home medical and home therapy care. The programme operates from out of three locations - Simei, Kampong Arang (Tanjong Rhu area) and Kampong Glam (North Bridge Road area) and covers major portions of the eastern part of Singapore. In 2013, the programme was expanded to include home therapy and home speech therapy, and to increase home medical capabilities through the collaboration with Family Medicine Clinics and with CGH s Transitional Care Programme (post-discharge support for patients at-risk for readmission). The demand for the programme is growing as there is a concerted effort nationwide to better support the elderly in the community, especially those who are home-bound because of illness or disability. 16

18 Paediatric Inpatient Rehabilitation Service SACH was gazetted by the Ministry of Social and Family Development as a Place of Safety, as well as a Place of Temporary Care and Protection, for paediatric inpatients in SACH is continuing to develop the paediatric inpatient rehabilitation service at Level 10, with plans to increase the current capacity of 10 beds. Hospital Information Technology (IT) In October 2013, SACH implemented Computerised Physician Order Entry (CPOE) in the wards. This enables clinicians and nurses to place laboratory and radiology orders electronically through Computer-On-Wheels (COWs) using the Electronic Medical Records system. This is a major step towards improving the speed and accuracy of clinical ordering of tests, and the access to results and reports. In November 2013, SACH also implemented inpatient Electronic Nurse Charting which enables nurses to electronically chart vital signs and the patients intake and output of fluids. Training and Education SACH participated in the ILTC (Intermediate and Long-Term Care) Quality Festival 2013 organised by the Agency for Integrated Care. SACH had the privilege of chairing the Organizing Committee. For the festival, SACH submitted three poster presentations on hospital quality improvement. One poster - Reducing Patient s Length of Stay in St. Andrew s Community Hospital - was recognised with a bronze award. On 22 and 23 November 2013, SACH co-hosted the Eastern Health Alliance s Annual Scientific Meeting with Changi General Hospital. The Geriatric Fractures & Osteoporosis symposium was held in SACH premises. In addition, the SACH Inpatient Therapy Services won first place for the Best Oral Paper (Research) Competition for their gait-trainer research project. St. Andrew s Lifestreams (SAL) Training Services : Professional & Pastoral Courses Professional Courses A total of 19 professional courses were conducted, of which five were new or revamped courses. As SAL is mindful of the needs of practitioners and caregivers in the community, the courses were organised to augment their knowledge and skills. Many of these professional courses are subsidised between 40% and 60% by the Voluntary Welfare Organisations (VWOs) - Charities Capability Fund (VCF), provided by the Ministry of Social and Family Development (MSF). 19 Counselling Services Counselling In 2013, we provided professional and pastoral counselling to 20 individuals and couples who had emotional, stress, parenting and marital conflict issues. They were mainly referred by the churches, friends and service users. In all, we conducted 48 counselling sessions, amounting to 57 clinical hours in total. Marriage Preparation Programme (MPP) The workshop s primary objective is to help couples better understand and relate with each other, and in turn be able to meet each other s needs. A total of two couples had benefited from this workshop. Each workshop lasted eight hours, covering various issues like personality styles, effective communication skills, conflict resolution, financial management, family planning and sexual needs. You Can Do It This programme deals with Social Emotional Learning. Three Workshops were conducted with a total of 52 participants. St. Andrew s Nursing Home (SANH) St. Andrew s Nursing Home was dedicated as a new service on 18 October 2013, with the Minister of Health, Mr. Gan Kim Yong as a special guest. SANH commenced operations in late November. To date, SANH is the largest and fourth institution providing psychiatric long term care in Singapore. Located at Buangkok Green Medical Park, the 7-storey, 300-bed purpose-built facility will care for aged patients with long-term psychiatric and medical conditions. The philosophical approach to caring for St. Andrew s Nursing Home s patients stems from God s greatest commandment is Love the Lord your God with all your heart and with all your soul and with all your mind. (Matthew 22:37). And the second is Love your neighbour as yourself. (Matthew 22: 39). Accordingly, with an integrated programme of rehabilitation, psycho-therapy, spirituality and evidence-based care approaches, St. Andrew s Nursing Home intends to generate a holistic outcome for its patients. Services Set up by St Andrew s Mission Hospital in collaboration with the Singapore Anglican Community Services, SANH provides skilled nursing and psychiatric care within a comprehensive recovery-oriented framework. Medical Care managing patients with triple diagnoses: psychiatric, general medical and intellectual disability Rehabilitative Care for patients who require occupational therapy, physiotherapy, and psycho-education. Pastoral Care Service to help patients find solace, strength, meaning and purpose towards the journey of recovery. Respite Care, a full-paying service for patients who suffer from chronic psychiatric conditions but do not need active medical treatment and whose caregivers need a period of respite from caring them at home. Skilled and individualised nursing care to support patients in activities of living Admission & Workload Statistics On 25 Nov 2013, SANH opened its doors to admit a total of 120 patients, of which 27 are female, and the rest are male patients. Among this group of patients, the acuity of patients are as follows: - 12% (14) requires 1:2 staff patient care ratio, - 32% (35) needs 1: 4 staff patient care ratio, and - 57% (67) requires 1 care staff to 8 patients. Based on our initial experiences with the first batch of admissions, there are some potential patients who may benefit from the integrated program of psychotherapy, rehabilitation, pastoral care and workplace learning. Our hope is for these patients to be rehabilitated and eventually re-integrated into the community with family support. Volunteerism As patients were mainly from IMH, SANH was introduced to the Yuan group of volunteers who actively volunteer their services and donations in kind to the IMH patients. This volunteer group has approximately 500 volunteers who provides free hair-cut bimonthly, festive goodies and organises parties and festive events for SANH s patients. Fundraising Partners/Supporters The work of SAMH and its respective services have always been blessed by generous fundraising partners and supporters, all of whom have helped us to improve and expand on our respective programmes and initiatives for the community. The Children s Charities Association (of which St. Andrew s Mission Hospital is a member), organises a host of fundraising activities annually; with the combined takings shared among its members. St. Andrew s Autism Centre (SAAC) In 2013, the donation from the Lee Foundation helped to provide the fees and transport bursaries for our needy students and clients in St. Andrew's Autism School and St. Andrew's Adult Autism Services' Day Activity Centre for This helped the families of needy students and clients to defray their fees and transport expenses. 18

St. Andrew s Nursing Home (SANH) An anonymous businessman generously pledged 1m to support SANH s operating expenditure for the next five years and another 200,000 to fund and equip the facility for Fundraising Activities This year, SAMH and SACS jointly held the SACS-SAMH Flag Day 2013 on 7 September. SAMH and its community service, St. Andrew s Autism Centre, also participated in the CCA s Annual Christmas Fair on 7 December 2013 at Ngee Ann City, selling various baked goods and delicacies from top hotels and cafes respectively. SAMH in 2014 We are grateful to the Lord for His provision and resources and guidance in all of the programmes, activities, and initiatives of all of SAMH s community services in 2013, and are thankful for His unwavering faithfulness for the past 100 years. 21 the mission to seafarers The Mission to Seafarers is the world s leading maritime welfare missionary society of the Anglican Church, recognized and trusted by the 1.5 million merchant seafarers. The charity provides for the social and spiritual needs of all seafarers regardless of race, creed or nationality. Currently the mission has a presence and operates in well over 258 ports, in 71 countries around the world. Seafarers are reached and helped through a network of chaplains, staff and volunteers. In Singapore there are three such centers; The Seafarers Care Centre in Jurong Port operated by MtS, and under the auspices of The Maritime and Port Authority of Singapore, a combined Missions Centre operations at Keppel Gate (International Seafarers Drop-In Centre. A third drop-in center at Pasir Panjang terminal is manned and managed by SEACARE. Our Vision is to be valued as God-directed leaders in caring for seafarers through our excellence, innovation, commitment and relevance in providing for seafarers' spiritual, social and practical welfare. The Singapore Branch of the Mission to Seafarers is dedicated to providing quality spiritual and practical care for seafarers through affordable and accessible facilities and through a highly trained and well equipped team of Chaplains and Volunteers. The Branch seeks to expand an awareness of the needs of seafarers, to foster civic and ecclesiastical responsibility in caring for them, to emphasize the role and responsibility of the maritime community as employers and to strengthen a sense of purpose in their seafaring careers. The Mission relies solely on the generosity of benefactors to finance the work. We are therefore, more than indebted to our volunteers and donors who support us with dedication and generosity. In partnership with them we are able to provide an effective ministry to seafarers, whatever their nationalities who call at any of the ports of Singapore. Volunteers Management Committee As a registered charity, The Mission relies on a dedicated team of volunteers to provide management functions for the Mission. Working in concert with the staff, the management committee meets a minimum of 4 times a year to: provide management directions and decisions where appropriate / required review the finances of the Mission and its activities and plan activities, including fund raising activities for the period ahead. In between these meetings, the port chaplain regularly conducts ad-hoc meetings and discussions with the chairman, vice chairman and secretary for matters arising in between the regular meetings which require their attention. Fund Raising Events and Donations During the year under review the following fund raising activities were held: 1. Annual Sea Sunday Appeal amount collected 57, km Ultra-Marathon Run Fund Appeal- netted a total of 43, Donations received from Anglican churches amounted to 95, Corporate donations received- 20, Other sources- 10, On top of the donations, the Mission also receives an annual grant of S40, from the Maritime Port Authority. MtS(Singapore) is very reliant and dependent on the funds and donations raised and received through individuals, churches and corporate donors. Highlights of Ministry Seeking and receiving God s grace and mercy has been a highlight for the year. During a prayer session, it was heart-wrenching to hear of the story of one seafarer who confided how he still loves his wife and chose not to divorce her despite she having three children with another man in his absence from the family. On another occasion, another seafarer was spotted being in distress while visiting our Drop in Centre, after hearing that his wife had opted to divorce him. Staff on hand were able to calm him down, comfort and care for him. Seafarers often pay a high price being away from families. It is only through the grace and mercy of God that we are able to draw alongside, listen, counsel, care and pray for these seafarers who are often lonely and isolated. Seafarers also face dangers at sea. Two Chinese seafarers were injured when their vessel collided with a tanker. This resulted in both of them losing the use of their right leg which was amputated at the knee. One of them had severe damages to the urinary tract which had to be operated on. The doctors have done what they could here to stabilise both the seafarers. They were hospitalized for more than three months, faced isolation and we had been able to become their friend and minister the grace of God and in the process see one of their parents accept the faith. There are opportunities for churches to come alongside in such cases to offer pastoral care and support. The seafarers were grateful that we assisted them in ensuring that they received adequate medical treatment. Another incident involved seafarers on board an arrested vessel who were seeking help with repatriation. Food supplies and water were running low on board the vessel and they were helped and the outstanding matters were resolved with the Union s assistance. Seafarers endure long periods of loneliness and isolation as they spend months away from their homes and families. At sea they are completely cut off from all that we take for granted and in port they are always visitors. They can often face many other difficulties including unsafe working conditions, non-payment of wages or being left stranded without food or money. While the majority of seafarers work in good conditions on well maintained vessels, even on the best ships they can be anxious of their family, suffer serious injury or bereavement, abandonment, face shipwreck, piracy attacks, feel homesick or need spiritual or practical support and we are on hand to draw alongside to offer support. Facilities and Services Offered to Seafarers: In Singapore we have a Central Office in Telok Blangah House and a Seafarers Care Centre in Jurong Port which currently receives just over 300 seafarers a month. We also support the International Seafarers Drop-In Centre at Keppel Gate which is visited by more than 300 seafarers a month. At all these venues seafarers receive a friendly welcome and are able to use internet and telephone facilities to contact their families and friends, be in an environment that is conducive enough to be ä home away from home. In this way we shrink the separation gap between them and their loved ones. This year we were able to visit slightly over 100 ships on average just to be with seafarers who spend most of their time at sea and are not always able to go ashore. As an Anglican Missionary agency whose only income is from donations and grants we rely on the generosity of donors to sustain our ministry. We are deeply grateful to the Diocese of Singapore and the Parishes which have untiringly supported the mission. The Mission needs to raise approximately SGD690, for the Year 2014 as there are vehicle replacement costs to consider. God has been faithful in providing for the needs of the mission and we look to Him with gratitude and confident trust that He will continue to provide for the ministry and walk in the midst of seafarers.
