2 What is The Rebellion? The REBELLION is a youth ministry lead by adult, college age & Sr. high volunteers helping Jr. High and Sr. High school students develop unity as a community of teens. By providing a safe place so they can just be themselves without being judged, hang-out, and make friends as they find out who God made them to be through Jesus Christ! Our staff strives to help teens know their value and challenge them to rise above societies Norms of expectation. We are a ministry of changing a generation for Christ! Developing a value system based on Christian principles to shape and guide this generation to be world-changers. A rebellion against apathy, lowlife living, and under-achieving! We are about promoting purity, dignity, honor and a sense of self-worth enveloped in "loving one another" so that we will shape the future of our culture! Our Mission Statement and Purpose for Anchored Our Goal is in Leading this generation into a personal relationship with Jesus Christ and Teaching them to pass a Legacy of Faith in a way that it changes a culture. There are many opportunities for teens to get involved and exercise their Spiritual Gifts. Also, we do Short-term mission trips, camp, once a month adventures as well as ministry of all forms. Our worship team, Fortunes of Judah is always encouraging teens who love to worship to come join! Youth leaders: Do you have a servant s heart? The TOWER Our youth facility The Tower is filled with video games (Xbox, Connect), big screen TV s and a deluxe food and snack bar. We have foosball, ice hockey, Pool table, and a large lounge area to hang-out or do homework or for our all-nighters! Football, basketball, soccer, & volleyball too. KIR Keep it Real Skate Ministry Redemption180 team Our scooter, skate & BMX Park is the best in the community along with dirt jumps for the advanced rider. 4 The Tower Blanket liability release form Release of all claims In consideration for being accepted by YLCC-REBELLION for participation in the tower drop in center, we (I) being 21 years of age or older, do for ourselves (myself) (and for and on behalf of my child-participant if said a child is not 21 years of age or older) do hear by release, forever discharge and agree to hold harmless YLCC- REBELLION and the staff of the Tower and directors thereof from any and all liability. Claims or demands for personal injury, sickness or death, as well as property and expenses, of any nature whatsoever which may be incurred by the undersigned and the child-participant that occur while said child is participating in all Tower sanctioned activities. Furthermore, we (I) [and on my behalf of our (my) child-participant if under the age of 21] here by assume all risk of personal injury, sickness, death, damage and expense as a result of participation in the recreation and work activities involved therein. Further, authorization and permission is hereby given to the church to furnish any necessary transportation, food and lodging for the participant, should the need arise. The undersigned further hereby agrees to hold harmless and indemnify said church, its directors, employees and agents, for any liability sustained by said church as the result of the negligent, willful or intentional acts of said participant, including expenses incurred attending thereto. (If the participant has not attained the age of 21 years): We (I) are the parent(s) of this participant, hear by grant our (my) permission for him (her) to participate fully in said trip, and hereby give our (my) permission to take said participant to a doctor or hospital and hereby authorize medical treatment, including but not in limited to emergency surgery or medical treatment, and assume all personal responsibility for any and all medical bills, if any. Further, should it be necessary for the participant to return home due to medical reasons, disciplinary action or otherwise, we (I) hereby assume all transportation costs. 5 SKATE RAMP LIABILITY RELEASE FORM Release of All Claims In consideration for being accepted by Yosemite Lakes Community Church (YLCC) for participation in the skate ramp on above property, we (I), being 21 years of age or older, do for our selves (myself) (and for and on behalf of my childparticipant if said child is not 21 years of age or older) do hereby release, forever discharge and agree to hold harmless Yosemite Lakes Community Church (YLCC) and the directors thereof from any and all liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the child-participant that may occur while said child is participating in the above described activity. Furthermore, we (I) (and on behalf of our (my) child-participant if under the age of 21 years) hereby assume all risk of personal injury, sickness, death, damage and expense as a result of participation in recreation of the skate ramp at the above named facility. The undersigned further hereby agree to hold harmless and indemnify said church, its directors, employees and agents, for any liability sustained at said church as the result of the negligent, willful or intentional acts of said participant, including expenses incurred attendant thereto. Participant agrees to follow the rules of safety, including providing and wearing of safety gear (helmet is mandatory, knee and elbow pads are encouraged), at all times.
