1 August 31, 2015 Attention: Stockton Unified School District Employees THE ANNUAL DISTRICT OPEN ENROLLMENT FOR HEALTH BENEFITS WILL BE HELD FROM SEPTEMBER 14, 2015 THROUGH OCTOBER 9, 2015 The annual open enrollment for health benefits provides an opportunity for employees to manage their benefit plan elections. These options include: Changing your current medical plan election Adding or deleting spouses or dependent children until the age of 26 Enrolling or re-enrolling in the medical rebate plan (See page 4) All changes made during this open enrollment period will become effective on January 1, Your January pay warrant will reflect the new deduction rates for The District s Annual Health plan costs are effective January 1, 2016 and are enclosed in this packet. If you do not wish to enroll, change plans or add or delete dependents, your present plan election will remain in place through 12/31/2016. Please note all current medical rebate participants must re-enroll during this open enrollment to continue in this benefit option for the 2016 calendar year. This requirement is noted in all bargaining unit contracts. 1

2 Open enrollment elections can be made by completing the appropriate forms at the enrollment sessions scheduled as follows. All enrollment sessions will be held at the STOCKTON HIGH SCHOOL PORTABLE #7 22 SOUTH VAN BUREN STOCKTON, CA SEPTEMBER 14-OCTOBER 9, 2015 The benefits department will be present and available to meet with employees based on the following schedule: First Letter of Employee Date Time Last Name A-M September 14, 16, 18 1:00 pm 4:00 pm A-M September 15, 17 2:00 pm 5:00 pm N-Z September 21, 23, 25 1:00 pm 4:00 pm N-Z September 22, 24 2:00 pm 5:00 pm All employees September 28, 30, 10/2 1:00 pm 4:00 pm All Employees September 29, 10/1 2:00 pm 5:00 pm All Employees October 5, 7, 9 1:00 pm 4:00 pm All Employees October 6, 8 2:00 pm 5:00 pm To ensure the best opportunity for customer service, the following information is being provided to ensure your enrollment visit is productive. DEPENDENT ADDITIONS The following documents are required for the addition of dependents on your benefit plans. We are required to confirm proof of dependency in order to add a child, spouse, or domestic partner to your plan. If you want to change plans, full documentation is required for the employee and all dependents. Children Ages 0-25 Birth certificate, social security card and copy of 2014 tax form, adoption, guardianship or court order documents if applicable. In accordance with the Patient Protection and Affordable Care Act of 2010, married or unmarried children may enroll and remain on your plan until age 26. 2

3 Spouse Marriage certificate or license, Social Security card and copy of 2014 tax forms. Domestic Partner State certificate and completion of the Stockton Unified School District Domestic Partner application, social security card or copy of 2014 tax forms Loss of Coverage Letter from employer indicating the loss of coverage date and appropriate documentation listed above. If you are changing plans, all documents must be provided. Please remember the following: CALPERS does not allow Dual Health benefit coverage for spouse or dependents. When dual CALPERS is discovered, the enrollment that caused the dual coverage will be retroactively cancelled. You may be responsible for all costs incurred from the date the dual coverage began. Example 1-Both husband and wife enroll for benefits and list each other as dependents on each other s plans. (Incorrect) The husband and wife may enroll separately but may not list each other as dependents on each other s plans.(correct) Example 2-An SUSD employee attempts to enroll in a CALPERS plan when he/she is already covered as a dependent under the spouse s plan who works for a different employer who also contracts with CALPERS. (Incorrect) The employee first instructs his spouse to terminate the dependent coverage under the other employer, then enrolls for individual coverage in a CALPERS plan at SUSD. (Correct) Split Coverage-Married employees with CALPERS coverage cannot split their dependents; i.e. one parent must cover all dependents. When split enrollments are discovered, the last enrollment will be retroactively cancelled. Marital Status-In the event a spouse passes away or a divorce occurs, you must notify the Risk Management office promptly. If CALPERS discovers that a divorce has occurred in the past without being reported, the coverage will retroactively cancelled and you may be liable for the claims filed for services for an ineligible dependent. 3

4 The last day for open enrollment is October 9, This is the deadline for the submission of all enrollment forms and documentation. No enrollment forms/documentation will be accepted after this date. MEDICAL REBATE PARTICIPANTS ALL employees currently enrolled in the Medical Rebate program must re-enroll during the open enrollment period and provide the documentation listed below to the enrollment location by 4:00 pm on October 9, This applies even if the enrollment recently was completed as a new or recalled employee. Any documentation sent to the District by mail should be addressed to: Stockton Unified School District, 701 N. Madison St., Stockton, CA and should be received by 4:00 pm, Friday, October 9 th, 2015 at the Risk Management Office. Acceptable documents for the Medical Rebate enrollment are: 1. A copy of the alternative health insurance card, and your 2. Marriage Certificate or a CALPERS notarized Affidavit of Marriage. OR 3. Domestic Partnership-State Certificate and completion of the SUSD Domestic Partner application When selecting or re-enrolling in the Medical Rebate Plan, remember to enroll yourself and your dependents in the dental and vision plans as these plans are cost free. See the medical rebate plan description to confirm if you are eligible for this option. The plan description will be available at the enrollment site. Employees who do not re-enroll for the Medical Rebate Program or submit the required documentation by October 9 th, 2015 will not be able to continue in the program. This timeline is the final date for enrollment. Employees who do not re-enroll for the medical rebate or enroll for a medical plan will not have insurance coverage when January 1, 2016 arrives. All employees who are enrolling for a CALPERS benefit plan must complete and submit an application form. A waiver of coverage is available at the Risk Management office for employees who do not want health insurance, and should be signed no later than October 9 th, For more information, please call the Risk Management Office at , ext for additional assistance. 4

PLAN SUMMARIES OF BENEFITS AND COVERAGES The Patient Protection and Affordable Care Act of 2010 requires the distribution of a summary of Benefits and Coverage for all medical health plans offered by an employer. The Summaries of Benefits and Coverage for all plans offered by the District may be found on the District s website at An excerpt from the CALPERS 2016 Health Benefit Summary has also been enclosed to allow for plan benefit comparison.
