1 RSL SmartChoice TM Short Term Disability (STD) A Grop STD Insrance Soltion for Small Bsiness Plan Benefits and Featres Grop Size 2 to 19 Employees Weekly Benefit Maximm Weekly Benefit $1,500 Garantee Isse 50%, 60%, 66.7% or 70% of weekly earnings 1 Flat amont of coverage for all (not to exceed 70% of weekly earnings) Elimination Period Zero days injry/seven days sickness Benefits Commence First day injry/eighth day sickness Benefit Period Choice of 13 or 26 weeks Pre-Existing Limitation Three months prior/six months after Transfer Pre-Existing Benefit Coverage Type Non-occpational Maternity Benefit Paid as any other illness Partial Disability Benefit Recrrent Disabilities 14 day retrn to work period Coverage is available for all eligible employees who enroll on a timely basis (within 31 days of eligibility) Time covered nder a prior short term disability plan will be credited toward satisfaction of any pre-existing limitation. The benefit for disabilities de to a pre-existing condition in the first six months of coverage will be the lesser of 50% of the STD benefit or $50 for a maximm of five weeks Provides a benefit to disabled employees who retrn to work part time or fll time with restricted dties after being totally disabled for 30 days Contribtions Employers can pay all of, part of, or none of the premim Carve Ots Permitted for 2 or more eligible employees within a class Rate Garantee Initial rate garanteed for 36 months Eligibility Employer Eligibility: Most employers are eligible to participate. A list of ineligible bsinesses is shown on the opposite side of this page. Firms in bsiness less than six months, firms not participating in Social Secrity and firms with employees residing on employer s premises are also ineligible. Employee Eligibility: Eligible employees are those actively working fll time for a minimm of 30 hors per week year rond (non-seasonal) who have satisfied the employer s minimm service reqirement. Eligibility may be modified to inclde part time employees working a minimm of 20 hors per week, provided less than 25% of the eligible employees are working less than 30 hors per week. 1 Definition of Earnings basic salary exclsive of overtime, bonses, and other special forms of compensation. Commission earnings will be based on the average earnings of the prior 24 months. 2 Benefits for grops located in CA, HI, NJ, PR or RI are sbject to a Maximm Weekly Benefit amont of 20% of weekly earnings p to the maximm benefit.
2 Participation Reqirements The following participation reqirements mst be met: 2 eligible employees both mst be insred 3 to 5 eligible employees all bt one mst be insred 6 to 9 eligible employees all bt two mst be insred 10 to 19 eligible employees 75% mst be insred Contribtion Levels Provided all participation reqirements are met, employees may contribte p to 100% of premim. If the employer pays 100% of the premim, all eligible employees mst be insred. Grop Policy Effective Date SmartChoice STD effective date is the first of the month following receipt of all application sbmission materials. Ineligible Bsinesses Definition of Disability An employee is considered disabled if he/she is nable to perform the material dties of his/her job, is nder the reglar care of a physician, and is not performing any work for payment. Recrrent Disability If a disability recrs more than 14 consective days after an employee retrns to active fll time work, a new benefit period begins. Pre-Existing Condition Any sickness or injry for which the insred received medical treatment, consltation, care or services, or took prescribed medicine dring the 3 months prior to the insred s effective date of coverage. A limited benefit will be paid if a disability occrs within the first 6 months of coverage and the disability is cased by a pre-existing condition. The pre-existing condition benefit is the lesser of 50% of the weekly benefit or $50, paid for a maximm benefit period of 5 weeks. The condition will be flly covered if the disability begins more than 6 months after the effective date. 3 RSL SmartChoice TM Long Term Disability (LTD) A Grop LTD Insrance Soltion for Small Bsiness Plan Benefits and Featres Grop Size 2 to 19 Employees Monthly Benefit 60% of basic monthly earnings 1 to a $7,500 monthly maximm Garantee Isse Coverage is available for all eligible employees who enroll on a timely basis (within 31 days of eligibility) Elimination Period Choice of 60, 90 or 180 days Benefit Period To Normal Retirement Age 2 for injry/sickness Own Occpation Definition 36 months Pre-Existing Limitation 12 months prior/12 months after 3 Transfer Time covered nder a prior long term disability plan will be credited toward satisfaction of any pre-existing limitation Pre-Existing Benefit 30% of basic monthly earnings to a maximm of $3,750 payable for 12 months Zero Day Residal Partial Disability/ Rehabilitation Benefit The elimination period can be satisfied by days of total and/or partial disability. Earnings loss is not reqired. After the elimination period, partial disability benefits are payable to disabled employees who retrn to work part time or fll time with restricted dties. Total Disability To be considered totally disabled means that as a reslt of injry or sickness dring the elimination period and for the next 36 months, an insred cannot perform the material dties of his/her reglar occpation. After a monthly benefit has been paid for 36 months, an insred is considered totally disabled if he or she cannot perform the material dties of any occpation that he or she is reasonably qalified based on edcation, experience or training. An insred who is partially disabled will be considered totally disabled dring the elimination period and thereafter. Partial disability means the insred is able to do all of his/her dties some of the time or some of his/her dties all of the time. NOTE: Definitions of Total Disability and Pre-Existing Condition can vary by state. Pre-Existing Condition Any sickness or injry for which the insred received medical treatment, consltation, care or services, or took prescribed medicine dring the 12 months prior to the insred s effective date of coverage. A limited benefit is payable for a disability cased by, contribted to by, or reslting from a pre-existing condition that occrs within the first 12 months from the insred s effective date. The condition will be flly covered if the disability begins more than 12 months after the effective date. Partial Disability/Rehabilitation Benefit This benefit is available to insreds who are partially disabled after satisfying the elimination period. The benefit may be paid ntil the date the maximm benefit period ends. This benefit will eqal the reglar monthly benefit less 50% of earnings received for partial disability employment and less any remaining integration amonts. Partial disability means the insred is able to do all of his/her dties some of the time or some of his/her dties all of the time.
