The FundsNetwk platfm is a service provided by Financial Administration Services Limited (referred to herein as FundsNetwk ), which is a Fidelity International Company. When we refer to Standard Life we mean Standard Life Assurance Limited. The FundsNetwk SIPP provided by Standard Life is available exclusively to individual invests, through FundsNetwk. It is not available direct from Standard Life. Both you and your adviser are required to complete this fm. If you wish to go into drawdown, you should complete FSIPP35. If you are a beneficiary transferring in a drawdown fund, you must complete a separate fm (this fm FSIPP30) f each person you inherit from. 2 TES ON HOW TO COMPLETE THIS APPLICATION FORM By filling in this fm you are applying to enter into a contract with Standard Life. Standard Life will provide you with a copy of this fm and the terms and conditions of the contract on request. Until your application has been accepted by Standard Life, their liability is limited to a return of the payments you have made. It is a serious offense to give false statements. The penalties are severe and could lead to prosecution. You must tell us all the material facts. These are facts that a reasonable consumer would consider to be material in such a contract. You must tell us if any infmation changes befe your plan starts. You must give honest and accurate answers to all of our questions. If you do not do this then the terms of your contract may be affected, your plan may be declared void. 6 Imptant Notes on Transferring Benefits 2. Transfers You only need to complete this section if you wish to transfer benefits from another pension scheme policy. You should understand the potential risks of pension transfers, which are summarised in the Key Features Document (FSIPP17). If you are transferring me than 1 plan, please photocopy pages 6 & 13 f each additional transfer and when complete, sign and date page 13 and attach all the pages to this fm. Assets from another pension scheme policy can be transferred in-specie. This means transferring the ownership of the asset instead of its cash value. If you wish to transfer assets in specie, please provide us with a list of the assets you wish to transfer. This will allow us to check that the assets on the list are acceptable to us. If the assets are not acceptable to us, we will advise both your current provider and your adviser. Your adviser will provide you with further advice if this is the case. Please refer to the Key Features Document (FSIPP17) f infmation on the charges f this type of transfer. Transfer payments can be made by BACS, cheque telegraphic transfer and should be made payable and sent to Standard Life. If the transfer payment includes funds in drawdown, a Drawdown to Drawdown Transfer Certificate (SLSIP32A) should be completed by the transferring scheme administrat. We cannot process the drawdown to drawdown transfer until we receive this. We cannot accept a mixture of Flexible and Capped Drawdown in one plan. You may need to open me than one plan. If you do not receive plan documentation from Standard Life within 5 weeks of submitting your transfer application, please contact your existing provider f a progress rept. (Please note; pension transfers can take upwards of 5 weeks on average to complete. (Not all members of the scheme must transfer). Both the ceding and new scheme must be the same f both members and the transfer should be made as one payment to the new scheme. This protects the members rights to a tax-free lump sum over 25 an early retirement age. This protection can only apply to one block transfer. 4. Are any of the assets to be transferred in-specie? 7 General Notes On Payments 3. Member and/ Employer Payments - Regular Single Payment(s) You only need to complete this section if you your employer wish to make a regular single payment(s) If you are self-employed any payments made from the partnership bank account should be reimbursed, as applicable. If you have registered f Enhanced Protection, any version of Fixed Protection, you should not make payments, so you should not complete this section. See GEN658 f further details. Please now go to Section 4. If you recycle a tax-free lump sum you may face a tax charge. Leaflet GEN449 - Recycling of lump sums explains what recycling means and what the tax consequences are. The minimum payment you can make to set up the plan is 300 a month, 3,000 a year a single payment of 10,000. (All figures shown are gross). If you your employer choose to have automatic yearly increases, then these increases apply to ALL regular payments. Single payments must be made by cheque. Remember to enclose the cheque(s) with this application fm. If you have started to take an income from another provider (that is not tax-free cash), you must tell us as it may affect your Annual Allowance. Please refer to to Infmation about tax relief, limits and your pension (GEN658) f me infmation. 3a Details of your Payments You only need to complete this section if you are making regular single payments to your plan. Otherwise go to section 3b. Regular Payments 1. Please state the regular GROSS amount you wish to pay into your plan. 2. How often do you want to make the payments stated above? Monthly Yearly You must make regular payments by Direct Debit. Regular monthly payments must be due on the same day each month. Regular yearly payments must be due on the same day and month each year. The amount you pay will be less than the gross amount that you have agreed to pay. This is because Standard Life reclaim basic-rate tax on your behalf. If you pay tax at the higher additional rate, you can claim the rest of the relief through your tax return. You can make occasional payments at any time. You can stop, restart, increase decrease your payment at any time. You will not be charged f this. 3. Please choose a date f your first regular payment between the 1 st and 28 th of the month. The first payment date must be a date after the date you submit the application fm. If the due date falls on a weekend bank holiday, your account will be debited within two wking days. Depending on which investments you have chosen to make regular payments into, your payments may not be invested into your chosen funds until later in the month. Single Payments 3b If making a single payment please state the GROSS amount. Cheques must be f the NET amount (i.e. less basic rate tax) and made payable to Fidelity. Contribution Controls Contribution controls legislation will apply to all your employer s payments. This means that Standard Life must make a rept to The Pensions Regulat if any payments are not made by your employer. Any payments made in respect of one month must be received by the 19 th of the following month the 22nd f electronic payments. The legal requirements on employers who are subject to contribution controls legislation mean that Direct Debit can only be used as a payment method if we receive payments at the same frequency as you are paid. Single Payments 4. If your employer wants to make a single payment please state the GROSS amount. Your employers cheque must be f the Gross amount and made payable to Fidelity. 8 3d Making Payments through your Employer You only need to complete this section if you will be making payments through your employer by salary deduction. Otherwise go to Section 3e. Salary deduction is where your own net payments are deducted from your salary and passed to Standard Life by your employer. This removes the need f a direct debit instruction on your personal account. 1. Please state the total NET amount of any payments. 2. How often will your employer make the payments? If Other, please give details. 3e Automatic Yearly Increases You only need to complete this section if you wish any regular payments (including existing ones) to increase automatically each year. If there are no regular payments being made, please go to section 3f. If you choose this option, Standard Life will automatically apply the increases on the anniversary of the first payment date, unless you specify otherwise. If the date you choose falls within 3 months of the date Standard Life sets up your plan, Standard Life will not increase the payments until your chosen date in the following year. You can only choose this option if you are paying by Direct Debit. Please note, if you invest in mutual funds then the increase amount will not be automatically invested in the selected mutual funds. Instead it will be placed in the SIPP Bank Account. You must select where you want to invest the increase amount after the increase payment has been collected. 1. We do this by carrying out an online identity check with a reference agency. This is not a credit check and will not affect your credit rating. The agency will verify your identity against public recds and confirm whether you have a credit histy (but will not disclose any infmation to us about your actual browings). The agency will add a note to your credit file to show that an identity check was requested by Standard Life. If successful, a copy of the results will be held by Standard Life to evidence that your identity has been verified. We regret that we cannot offer an alternative unless the online check does not confirm your identity, in which case we will carry out a manual check. Date Page 9 of 15 OFFICE USE ONLY

10 4 How you Want to Pay your Adviser 4a 4. Paying your Adviser and Investing your Payments Please tell us what you want to pay from your plan to your financial adviser. We will use the infmation you provide in this section to pay your financial adviser. By signing the declaration in Section 5 of this fm, you are: agreeing to the payment of the adviser charges documented below, and authising Standard Life to accept all future instructions from your adviser f payment of adviser charges that you have agreed to pay them from your plan. You do not need to complete the infmation in Sections 4a, 4b 4c if: your financial adviser is billing you directly f their services, your financial adviser is not charging you f their services. If either of these two options applies, cross this box and then go to Section 4d. Imptant note Drawdown and/ your tax-free lump sum If you have chosen drawdown, we will calculate Initial Adviser Charges you have chosen to pay your financial adviser on the value of the benefits you are taking befe the tax-free lump sum is paid. This will not reduce the amount of tax-free lump sum you can take. If you are taking your benefits in stages then you should confirm the monetary amount of charges you have chosen to pay. Adviser Charge f Regular Payments (Regular Initial Adviser Charge) Complete this section if you will be making regular payments to your plan and have agreed a charge f advice with your adviser (either as one payment spread over a period of time). Spread payment of Regular Initial Adviser Charge (please complete one of the following options) If you re only making one payment to your adviser, you can enter 1 here eg 100 a month f 1 month. a month f months a year f a quarter f years quarters a half-year f half-years 4b Adviser Charge f Single and Transfer Payments (Initial Adviser Charge) Complete this section if you (and/ your employer) will be making a single payment transfer payments to your plan and you have agreed a charge f advice with your adviser. Initial Adviser Charges will be deducted as each transfer payment is processed once drawdown has been processed. Please state the amount of Initial Adviser Charge per payment as either a monetary amount percentage of that payment. Single payment from you Single payment from your employer Each transfer payment If you are transferring me than one pension and would like to apply different adviser charges to each transfer payment, please provide separate written instructions. 4c Ongoing Adviser Charge Complete this section if you have agreed to pay your adviser f ongoing advice in relation to your plan. The Ongoing Adviser Charge can either be a set amount a percentage of your plan value. 1. Set Amount every month every year every quarter every half-year 2. Where we cannot achieve this we will contact your adviser. As we do not know when the money will be available we cannot guarantee any asset fund prices. When providing your investment instructions please ensure the total investment adds up to 100. When making regular monthly payments to your plan, please ensure that the investment split has been applied to the total monthly amount. Remember that the total monthly amount of your investment may be reduced if your adviser is taking adviser charges. There must be sufficient money within the SIPP Bank Account to pay f your investment instructions. Please also ensure you keep a minimum level of money available in your account to pay f any charges. FundsNetwk recommend that a minimum of 12 months of charges are kept in the SIPP Bank Account. Page 10 of 15 Please Turn Over

11 4d Transfer/Single Payment Investment Instructions Imptant Notes: FUND CODE DETAILS ARE MANDATORY and must be completed in der f us to process your instructions. When completing Fund Name infmation please ensure that you specify either an income an accumulation version of your chosen fund. If you do not include this infmation, your fund choice will default to the accumulation version. You can find details of all investment options including Fund Codes at by asking your adviser. A European Union directive requires fund managers to produce a key infmation document f most funds. This document sets out key infmation about the fund including the objectives and risks of the fund, what it invests in and its charges. It is imptant that you read the key infmation document befe selecting the fund. All key infmation documents where applicable are available at fidelity.co.uk.
