1 4001 Rodney Parham Road Original Title Page TITLE PAGE This PRICE LIST applies to Long Distance Communications Service ( LDCS ) furnished by Windstream Systems of the Midwest, Inc. and hereinafter referred to as the Carrier, for interstate or international communications. LDCS for interstate communications is furnished between one or more stations in the Operating Territory of the Carrier and one or more stations in the different states in the contiguous United States, Alaska, Hawaii, the District of Columbia, Puerto Rico, the U.S. Virgin Islands, American Samoa, Guam, and the Commonwealth of the Northern Mariana Islands. LDCS for international communications is furnished between one or more stations in the Operating Territory of the Carrier and one or more stations in Canada, Mexico or International Overseas locations. LDCS is furnished by means of the wire, radio, satellite, fiber optics, or any suitable technology or combination of technologies employed by Carrier and/or Carrier's underlying carrier or carriers. LDCS is furnished subject to the availability of facilities, and subject to transmission, atmospheric and like conditions.

4 4001 Rodney Parham Road Original Page REGULATIONS Description of Service SECTION 1 - GENERAL SERVICE OFFERINGS A. Carrier provides interstate communications service to customers for their direct transmission and reception of voice, data and other types of telecommunications between various locations within the continental United States, Alaska, Hawaii, the District of Columbia, Puerto Rico, the U.S. Virgin Islands, Guam, American Samoa, and the Commonweatlh of the Northern Mariannas. Service is available on a full-time basis seven days a week. B. Basic service is provided on the basis of two different service options (Dial-Up and Dedicated). Depending on the option(s) chosen, the customer may select the manner in which calls are originated and terminated. (1) Users who have "Feature Group D equal access" availability are considered dialup service customers. However, when using their "equal access" lines, they simply dial 1 plus the area code and telephone number desired. In "equal access" areas, customers can also dial to reach Carrier's switching equipment. Once answered, the calling procedure is the same as for true dial-up service. Dial-Up service may also be originated when the customer dials one of the exchange telephone numbers of the Carrier. When the call is answered, the customer must key in his authorization code followed by 1 plus the area code and telephone number desired. (2) Dedicated service originates when the customer accesses the Carrier via dedicated facilities between his premises and Carrier's terminal location. When the call is acknowledged, the customer must key in 1 plus the area code and telephone number desired. C. The customer is charged no monthly fee for the first authorization code, only a charge for each individual call placed during the month. Charges for calls are both distance and time sensitive. Please refer to Section D for the schedule of rates and charges. D. Service is generally provided to customers on a subscription basis, which is termed Basic Service. E. The customer is responsible for the charge, local or toll, incurred in accessing the Carrier's switch, said charge to be billed to the originating number, by the appropriate local carrier in the absence of special billing instructions from the customer. F. To obtain Basic Service, the customer, as an "equal access" subscriber, dials 1+. At additional cost, the customer may obtain additional authorization codes or use project codes to identify the users or user groups on his account and to allocate the cost of his service accordingly. The numerical composition of such codes is set by Carrier to assure compatibility with Carrier's accounting and automation systems and to avoid duplication of codes.

Carrier's optional "Call Home" service allows a customer to call back to his "home base", e.g., Lincoln or Omaha, via an 800 number terminating on the carrier's switch. After the witch answers the 800 trunk, the customer follows the same dialing instructions as those for "dial up" service to reach his desired number. H. At additional cost, the customer may obtain exclusive use of a dedicated port on Carrier's switching facility. I. Customers may receive an additional discount on "eligible" 1+ usage through "Special Calling Programs." J. Customers may obtain assistance in determining telephone numbers by calling a Directory Assistance (D. A.) number. As many as two requests may be made on each call to Directory Assistance. The D. A. charge applies whether or not the requested phone numbers(s) can be furnished, e.g., where the requested number is non-published, unlisted, or no record can be found. Those handicapped Customers who qualify for exemptions from local Directory Assistance charges under Local Exchange Company tariffs are exempted from the Directory Assistance charge. This exemption applies only to calls to Directory Assistance which are billed to the handicapped Customer's residence telephone number by The Lincoln Telephone Company. K. At additional cost customer may obtain exclusive use of an in bound 800 number which permits calls made to that number from other locations to terminate via the carrier's switch at the customer's designated premises Limitations on Service A. Service is offered subject to the availability of the necessary facilities and/or equipment and subject to the provisions of this Price List. Carrier reserves the right not to provide service to or from a location where the necessary facilities or equipment are not available. B. Carrier reserves the right to discontinue furnishing service, upon written notice, when necessitated by conditions beyond its control, or when the customer is using the service in violation of the provisions of this Price List, or in violation of the law Surcharges and Fees A. Universal Service Fund (USF) Charge Pursuant to Federal Communications Commission (FCC) orders, carriers may flow through the FCC s USF charge to end user customers, in order to fund the Universal Service Fund. One of the purposes of the USF is to support high cost telephone areas. This charge will be applied monthly and the charge for a given month will be derived by applying the USF percent to the total of the (T) Interstate toll on that month s bill. The USF percentage shall be dictated by the FCC and adjusted periodically in accordance with FCC requirements in CC Docket No (T) B. Payphone Surcharge A surcharge that applies to completed intrastate, interstate and international long distance calls placed from any domestic payphone used to access the Company s services. This includes calling card service, toll-free service, and prepaid calling card service. The Payphone Surcharge is in addition to any other applicable service charges or surcharges. The Payphone Surcharge does not apply to calls placed from pay telephones at which the Customer pays for the service by inserting coins during the progress of the call. Interstate Service Fee SECTION 1 - GENERAL SERVICE OFFERINGS The Interstate Service Fee (ISF) allows the Company to recover costs, such as those associated with provisioning and maintaining customer accounts on the Company s network, as well as interstate and international connection charges. The ISF will be assessed per account on a monthly basis to residential and business customers who subscribe to the Company s long distance interstate service. ISF Monthly Rates Residential, per account $1.97 Business, per account $3.97 D. Bill Copy Fee The Company will charge a fee of $3.00 per bill copy when a customer requests a copy of a previous month s bill. (N) (N)

7 4001 Rodney Parham Road Original Page RATE DETERMINATION - The chargeable time for a call is determined by the duration of the call. Chargeable time includes the initial period plus the additional time involved, if any, and is determined as follows: The per-minute rates and related one-time and recurring charges applicable to Carrier's "Dial up", "Dedicated" and "Call Home" services are as specified in the following rate tables. (Monthly recurring charges may be prorated for partial months.) These rates may be changed upon one day s notice to the Commission Each customer is charged individually for each call placed through the carrier since the last monthly billing. Any portion of a minute of call duration is rounded to the next full minute. Minimum length of a call is sixty seconds unless specified otherwise Time Determination - On all calls chargeable time begins when connection is established between the calling station and the called station, PBX or a mobile radio system for Public Land Mobile Service, Coastal Harbor Service, VHF Service, or Air-Ground Service Disconnect Time - Chargeable time ends when the calling station hangs up." If the called station "hangs up" but the calling station does not, chargeable time ends when the connection is released either by automatic timing equipment in the telecommunications network or by the Carrier operator Customer-Systems - When LDCS is directly connected to a Customer-provided communications system at a Customer's or User's premises, chargeable time for all classes of service begins when an LDCS call terminates in, or passes through, the first Customer Equipment on that Customer-provided communications system. It is the Customer's responsibility to furnish appropriate answer supervision to the point of interface with the LDCS so that chargeable time may begin Dedicated Port - Use, if any, of dedicated port(s) on Carriers switching facility by the customer is subject to a one-time installation charge per port, payable in advance of installation, and a monthly use fee per port per month. These charges are in addition to the other charges in this PRICE LIST. The customer is solely responsible for furnishing and paying other common carriers for the customer's private line connecting the dedicated port to the customer's premises.

8 4001 Rodney Parham Road Original Page DETERMINING APPLICABLE RATES Mainland - For calls originated and terminated in the Mainland, the rate in effect at the calling station at the time the connection is established applies to the entire call All Other Calls - For all other calls, except for calls that use a special access line, when a call is established in one rate period and ends in another rate period, the rate in effect at the calling station for each rate period applies to the portion of the call occurring within that rate period. When a calling station uses a special access line, the rate in effect at the Carrier's Central Office for each rate period applies to the portion of the call occurring within that rate period. When a unit of time is split between two rate periods, the rate applicable to that unit of time is based on the rate period in which it began Time Continuum - Chargeable time for a rate period (e.g., 8 AM - 5 PM) begins with the first stated hour (e.g., 8 AM) and continues to, but does not include, the second stated hour (e.g., 5 PM) DISTANCE CALCULATION - Usage charges for all mileage sensitive products are based on the airline distance between the serving wire center locations associated with the originating and terminating points of the call. The distance between the originating and terminating points is calculated by using the V and H coordinates of the serving wire centers as defined by BellCore (Bell Communications Research), in the following manner: A. Obtain the "V" and "H" coordinates for the serving wire center of the Customer s switch and the destination point. B. Obtain the difference between the "V" coordinates of each of the Rate Centers. Obtain the difference between the "H" coordinates. C. Square the difference obtained in Step B. D. Add the squares of the "V" difference and "H" difference obtained in Step C. E. Divide the sum of the squares obtained in Step D by ten (10). Round to the next higher whole number if any fraction results from the division. F. Obtain the square root of the whole number obtained in Step E. Round to the next higher whole number if any fraction is obtained. 9 4001 Rodney Parham Road Original Page SPECIFIC RATING REGULATIONS - This section contains regulations governing application of the LDCS: in the Mainland; between the Mainland and Alaska, Hawaii, the District of Columbia, Puerto Rico, the US Virgin Islands, Guam, American Samoa, and the Commonwealth of the Northern Mariannas. Rates, regulations and material in this section are for two-point service only Application of Charges - The specific factors used to determine the applicable charges for a call are indicated on the respective rate schedules. The following factors are common to all LDCS rate schedules in this section: A. Initial Period - Initial Period is the initial rate increment of an LDCS call. The specific length of the initial period is one minute unless otherwise indicated. B. Additional Minute - Additional Minute is the rate element used to bill for the chargeable time when an LDCS call continues beyond the initial period. Additional Minute begins when the initial period ends (e.g., with the second minute of a call for which the initial period is one minute). Additional Minute rates apply to each additional minute, or any fraction thereof, that chargeable time continues beyond the initial period. C. "Dedicated" calling usage is accumulated in six second increments. Total usage is then converted to minutes. D. List of Applications 1) Rates for "Dial-Up" and "Dedicated" Service, as described in preceding, are found in 2.5 following. 2) Rates for Basic "One Plus" Service as described in preceding, are found in 2.6 following. 3) Rates for "Call Home" Service, as described in preceding, are found in 2.7 following. 4) Rates for "Special Calling Programs", as described in preceding, are found in 2.8 following. 5) Rates for Dedicated T-1 Access Program, as described in preceding, are found in 2.9 following. 6) Rates for outbound WATS service, as described in preceding, are found in 2.10 following. 7) Rates for inbound 800 services, as described in preceding, are found in 2.11 following. 8) Directory Assistance calls are rated at a flat $0.85 (eighty-five cents) per call. Unless otherwise specified in particular offerings. 9) Time of day references in this PRICE LIST shall be defined as follows: Day = 8 AM to 5 PM Monday - Friday Evening = 5 PM to 11 PM Monday - Friday 5 PM to 11 PM Sunday 8 AM to 5 PM All Holidays Night/Weekend = 11 PM Friday to 5 PM Sunday 11 PM to 8 AM Sunday -Friday 10) Holiday rates shall apply on the following days: January 1, July 4th, Labor Day, Thanksgiving, December 25th.

A. Day Rate Period - 8 AM to, but not including, 5 PM Monday through Friday. B. Evening Rate Period - 5 PM to, but not including 11 PM Monday through Friday and 5 PM to, but not including 11 PM Sunday. C. Night/Weekend Rate Period - 11 PM Friday to, but not including 5 PM Sunday; 11 PM to 7 AM Sunday - Friday; and all day Saturday D. For additional minutes, the rate applicable is that rate which is in effect at the calling station when the additional minute(s) begin. That is, if chargeable time begins during the Evening Rate Period, the Evening rate applies to the initial minute and to any additional minutes that the call continues during that rate period. If the call continues into a different rate period, the appropriate rates from that rate period apply to any additional minutes occurring in that rate period. If an additional minute is split between two rate periods, the rate period applicable at the start of the minute applies to that entire minute. If the computed charges include a fraction of a cent, the fraction is rounded down to the next whole cent (e.g., $1.426 would be rounded down to $1.42) LDCS Options and Contract Services - In lieu of the rates found in Sections 2.4 through following, Carrier, may provide to all qualified Customers similarly situated, certain promotional offerings from time to time and LDCS Option Services and/or Contract Services, subject to the extent of network availability, technical capacity and economic factors. Certain restrictions on the number of Options or Contract Services per main billed telephone number may apply, such as a limit on the number of such options or contracts or the combining of such options or contracts.

This plan is available to switched access customers who have selected the Carrier as the customer s Primary Interexchange Carrier. The customer must subscribe to the plan. No nonrecurring or monthly recurring charges are associated with the plan. The plan discounts are applied to Basic "One Plus" direct dialed Interstate/Intrastate InterLATA calls. The discounts are applied based on the total billed amount of eligible usage. This plan and its discounts do not apply to any other discount calling plans which have been subscribed to. DISCOUNTS Eligible Billed Amount Discount $10.00 to $ % $50.00 or More 30% B. After Seven Calling Plan This plan is only available to existing customers at their existing locations. The After Seven Calling Plan is available to residential switched access customers who have selected the Carrier as the customer s Primary Interexchange Carrier. This plan is furnished for Carrier-provided interstate/interlata Basic "One Plus" direct dialed calls. The plan offers a participating customer the option of paying a fixed monthly charge to obtain up to one hour of calling between the hours of 7:00 p.m. and 8:00 a.m. Monday through Saturday, 8:00 a.m. Saturday to 7:00 p.m. Sunday, and 11:00 p.m. Sunday to 8:00 a.m. Monday. An additional hourly rate, prorated to the actual amount of usage in a partial hour, is billed for calling that exceeds the initial hour. Customers also receive a 7% discount on Basic "One Plus" direct dialed calls made outside the plan calling period. This rate is applicable for all times for interstate calls where technically available. *Monthly Fee: $5.00 Rate per minute: $. 07 Calling Card Calls will be billed at $0.35 per minute with no surcharge. *The monthly fee applies per account, not per line. This plan includes a per minute rate for intrastate calling. D. Residential Bundled Service Plan (Windstream 7 - Bundle) This plan is only available to existing customers at existing locations. The following interstate rate plus monthly fee is designed for residential users who purchase certain bundled service plans. The plans could include Windstream Systems of the Midwest, Inc. local, wireless, Internet, long-distance and paging. This rate is applicable at all times for interstate calls where technically available. *Monthly Fee: $3.00 Rate per Minute:$.07 Calling Card Calls will be billed at $0.35 per minute with no surcharge. *The monthly fee may be included in the bundled price of the plan and may not appear separately on the customer s bill. The monthly fee applies per account, not per line. This plan includes a per minute rate for intrastate calling. (N) E. Residential Flat Rate Bundled Plan This plan is only available to existing customers at their existing locations. The following interstate rate is designed for residential users who purchase certain bundled service plans. The plans could include Windstream Systems of the Midwest, Inc. local, wireless, Internet, long-distance and paging. This rate is applicable at all times for interstate calls where technically available. Rate per Minute:$.10 Calling Card Calls will be billed at $0.35 per minute with no surcharge. F. Residential Calling Plan (Windstream Nickel Sense) This plan is only available to existing customers at existing locations. The following interstate rate plus monthly fee is designed for residential customers of outbound interstate LDMTS. This rate is applicable for all times for interstate calls where technically available. *Monthly Fee: $8.00 Rate per minute: $. 05 Calling Card Calls will be billed at $0.35 per minute with no surcharge. *The monthly fee applies per account, not per line. This plan includes a per minute rate for intrastate calling. (N)

Residential Bundled Service Plan (Windstream Nickel Sense Bundle) This plan is only available to existing customers at existing locations. The following interstate rate plus monthly fee is designed for residential customers of outbound interstate LDMTS. This rate is applicable at all times for interstate calls where technically available. *Monthly Fee: $6.00 Rate per Minute: $.05 Customers must enroll in a Specialized Billing Arrangement prior to becoming eligible for this rate. Calling Card Calls will be billed at $0.35 per minute with no surcharge. *The monthly fee applies per account, not per line. This plan includes a per minute rate for intrastate calling. (N) H. Residential Peak/Off-peak Plan This plan is only available to existing customers at their existing locations. The following flat rates are for residential users of outbound interstate LDMTS. This rate is applicable for the respective times for interstate calls where technically available. Peak Off-peak Rate per Minute $.20 $.10 The times associated with peak hours for this plan consist of 8:00 AM to 6:00 PM Monday through Friday. All other times are considered off-peak. Calling Card Calls will be billed at $0.35 per minute with no surcharge. I. Residential Flat Rate Plan This plan is only available to existing customers at their existing locations. The following flat rate is for residential users of outbound interstate LDMTS. This rate is applicable at all times for all interstate calls where technically available. Rate per minute: $.15 Calling card calls will be billed at $.35 per minute with no surcharge. J. Residential Flat Rate Plan (Windstream 10) The following flat rate is for residential users of outbound interstate LDMTS. This rate is applicable at all times for all interstate calls where technically available. Monthly Fee: $1.95 Rate per minute: $.10 Calling card calls will be billed at $.35 per minute with no surcharge. This plan is only offered in conjunction with a corresponding intrastate plan which includes a per minute rate for intrastate calling.

Windstream 500 Residential Plan This plan is only available to existing customers at existing locations. Windstream 500 is a residential direct-dialed long distance calling plan which includes 500 minutes per month of direct-dialed interstate calling any time of day for a monthly recurring fee with each additional minute billed at a per minute rate. The plan includes a per minute intrastate rate. The intrastate minutes will not be applied to the 500 interstate minutes. (N) The following monthly fee and rates are applicable for all times for outbound interstate LDMTS, subject to availability. Rate Per Minute: $0.10 Monthly Fee: $20.00 Calling Card Calls, Rate Per Minute (No Surcharge) $0.35 L. Residential Calling Plan This plan is only available to existing customers at their existing locations. Residential customers of Direct Dialed LDMTS service may place outbound interstate LDMTS calls at the following flat rate plus monthly fee, where technically available. Rate per Minute: $0.10 Monthly Fee: $4.95 Calling Card Calls will be billed at $0.35 per minute with no surcharge. M. Simple Six Residential Plan This plan is only available to existing customers at their existing locations. Simple Six offers direct dialed intrastate and interstate LDMTS for a monthly fee to residential customers who purchase certain bundled service plans. The bundled service plans could include such Windstream products as wireline, custom calling packages, and long distance. The intrastate per minute rates vary by state. The following monthly fee and rates are applicable for all times for outbound interstate LDMTS, subject to availability. Monthly Fee: $2.00 Rate Per Minute: $0.06 Calling Card Calls, Rate Per Minute (No Surcharge) $0.35 N. Residential Default Plan A This plan is only available to existing customers at their existing locations. Residential customers of Direct Dialed LDMTS service may place outbound interstate LDMTS calls at the following flat rate, where technically available. Rate Per Minute $0.10 Calling Card Calls, Rate Per Minute (no surcharge) $0.35 Customers will automatically be placed on this plan if they do not specifically request any other LDMTS rate plan.

Plan 2 The following flat rate and monthly fee is for residential users of inbound toll-free service. This rate is applicable for all times for all calls where this service is available. Residential Account Code Service Residential Account Code Service is an optional service for direct intrastate and\or interstate long distance calls that offers customers a way to track long distance usage. This Service allows customers the ability to have calls separated and displayed on their telephone bill. Account Code Service requires callers to enter a unique code for completion of a long distance call. The Company s network will authorize calls only if the unique code is entered. Bottom Line New Business Customer Introductory Program This program is offered to introduce the Carrier s service to small and medium sized businesses that are not current customers of the Carrier. This plan and its rates do not apply to any other calling plans which have been subscribed to. When the card balance nears zero during a call, an automated voice announces the remaining time to the customer. Calls are automatically disconnected after the card balance reaches zero. Per minute charge: Maximum of $0.15 W. Calling Card Surcharge Calling card calls placed from a payphone will be charged a surcharge as set forth in Section X. Bottom Line Chamber Advantage This program is offered to businesses that are members of a participating Chamber of Commerce. The Chamber of Commerce commits to the program for a term of two years or longer. Each business member of the Chamber of Commerce, that subscribes to one of the following services; 1. Bottom Line WATS 2. Bottom Line 1+ WATS 3. Bottom Line 800 Dedicated Access Line Program 4. Bottom Line 800 Central Office Line Program will receive a 5% discount on their domestic usage in addition to the respective discounts applicable to the specific plan subscribed to. The participating Chamber of Commerce receives a rebate from the Carrier equal to 2% of the aggregate, net-billed domestic usage of all the participating businesses.

A Customer selecting a term must commit to a minimum monthly usage as depicted below. This minimum usage threshold applies to every monthly billing period during the selected term, and must be paid regardless of the Customer s amount of usage during other billing periods. Customers selecting a term may change plans or terminate the service if Customer provides written notice within ninety (90) days of the initiation of service.
