2 Important Notice Concerning Your Rights under the MedCentral Health System 457(b) Top Hat Plan [MedCentral 457(b) Plan] This notice is to inform you that the MedCentral 457(b) Plan will be moving to Fidelity Investments. To ensure that all information is transferred accurately from your current plan account at MassMutual to your new MedCentral 457(b) Plan account at Fidelity, there will be a period of time when you will be unable to direct or diversify investments, or to obtain a distribution from the MedCentral 457(b) Plan. This period, during which you will be unable to exercise these rights otherwise available under the MedCentral 457(b) Plan, is called a blackout period. Whether or not you are planning retirement in the near future, we encourage you to carefully consider how this blackout period may affect your retirement planning, as well as your overall financial plan. The Plan gives you a choice of investment options that allow you the flexibility to build your own strategy to meet your needs both long and short term. After the transition, the MedCentral 457(b) Plan investment lineup may include current options and new investment choices. Depending on which funds you currently invest in, some of your investments may change. If you are satisfied with the way your investments are transitioning to Fidelity, no action is required on your part. An array of services. Fidelity s NetBenefits website is designed so you can quickly and easily set up, monitor, and manage your 457(b) account. It s a new way to help you get, and stay, on course toward your retirement goals. With Fidelity Portfolio Advisory Service at Work, you get active retirement account management. This means that Fidelity s team of investment professionals invest, monitor, and rebalance your account as needed, to adjust to changes in the market or changes to your situation. Fidelity professionals are available to answer any questions you may have about this managed account service. Call (866) for more information. Rebalance notification and automatic rebalance. Get notified via any time your account s investment mix strays from your original strategy. You can also have your account rebalance your assets to stay consistent with the investment strategy you have chosen. Workplace planning and support. Fidelity Retirement Planners are knowledgeable professionals equipped with detailed information about the Plan and the financial know-how to provide advice on investing that will help get you on your way. Your final account statement from MassMutual will be mailed to you. IMPORTANT INFORMATION ABOUT ACCOUNT STATEMENTS In January 2018, your first quarterly statement from Fidelity will be available. A quarterly paper statement will not be automatically mailed to your home, but you will receive an or letter notification when the statements are available. You may request a paper statement of your account at any time. Note that you can change your mail preferences and request that quarterly statements be mailed to your home address. 9 How your investments with MassMutual will transfer to Fidelity When the market closes (generally 4 p.m. Eastern time) on September 7, 2017, the investment options offered through your Plan with MassMutual will no longer be available. As a result, current account balances and future contributions will transfer to the new options offered through Fidelity as shown below. Once the blackout period has ended, which is expected to be during the week of October 1, 2017, all Plan services will be available, including the ability to make investment exchanges within your account.
