2 Invest some of what you earn today for what you plan to accomplish tomorrow. Dear Valued Employee: Congratulations! You are eligible to participate in the CentraCare Health 457(b) Eligible Deferred Compensation Plan (457(b) Plan). Eligible employees can defer salary up to $18,000 in 2017 and have these amounts invested in the 457(b) Plan through Fidelity Investments. These pretax contributions would be in addition to, and would be maintained in an account separate from, contributions you may make to CentraCare 403(b) Retirement Savings Plan. Essentially, this means that in 2017 you may be eligible to defer up to $18,000 in your 403(b) account, as well as up to $18,000 in a 457(b) account, for a total of $36,000. (Please note that some employees may also be able to make additional catch-up contributions to the 403(b) Plan if they are age 50 or older). This is a great opportunity for you to maximize your pretax contributions. However, as explained below, eligible employees generally do not contribute to the 457(b) Plan unless they are making the maximum annual contributions to the CentraCare 403(b) Plan. Due to plan requirements, the 457(b) may be offered only to select groups of management and highly compensated employees. Additionally, it is important that you understand that since the 457(b) plan is unfunded, all compensation deferred under the plan, although invested in an account in your name with Fidelity Investments, is legally considered part of the CentraCare Health s general assets and is subject to the claims of the Health s creditors. This could result in the possible forfeiture of the deferred amounts in the unlikely event of CentraCare s insolvency. In contrast, 403(b) account contributions are not subject to the claim of creditors. Accordingly, you might consider exhausting your maximum IRS-allowed contribution to the 403(b) plan before you begin contributing to the 457(b) plan. Prior to enrolling, we would like to expand on an important aspect of the CentraCare 457(b) Plan. Upon termination of employment from CentraCare, the 457(b) Plan states that each participant must make a distribution election and choose to receive their distribution at that time, or defer it to a future date. This election must be made within 30 days of separating from employment. If you do not defer the distribution date, your distribution will be processed 60 days after the date you separated employment. We believe this plan is a great opportunity for you to save more for your retirement. 3 Frequently asked questions about your plan. Here are answers to questions you may have about the key features, benefits, and rules of your plan. Should I enroll if I am not contributing the maximum amount to the 403(b) Retirement Savings Plan? You may want to hold off contributing to the 457(b) Plan unless you are already contributing the maximum annual amount to the 403(b) Retirement Savings Plan. This is because of the 457(b) Plan s unfunded status, the absence of any matching employer contributions, and the Plan s more limited distribution options. How do I enroll in the Plan? Log on to Fidelity NetBenefits at or call the Fidelity Retirement Benefits Line at to enroll in the Plan. How much can I contribute? Each year, your contributions, which are made from your pay on a pretax basis, are limited to a maximum dollar amount. The maximum dollar amount for 2017 is $18,000. This amount is in addition to the contributions you may make to the CentraCare 403(b) Plan. When is my enrollment effective? Your enrollment becomes effective as soon as administratively possible. How is the 457(b) Plan different from the 403(b) Plan? The primary difference is that, unlike the 403(b), the CentraCare 457(b) Plan is a nonqualified plan. This means that by participating in the Plan you become a general creditor of CentraCare and do not have the right to any particular company asset. Your 457(b) account is therefore only as secure as your CentraCare s ability to make the payment. How do I designate my beneficiary? If you have not already selected your beneficiaries, or if you have experienced a lifechanging event such as a marriage, divorce, birth of a child, or a death in the family, it s time to consider your beneficiary designations. You can change your beneficiary designation using the form enclosed in this guide. What are my investment options? To help you meet your investment goals, the Plan offers you a range of options. You can select a mix of investment options that best suits your goals, time horizon, and risk tolerance. The many investment options available through the Plan include conservative, moderately conservative, and aggressive funds. A complete description of the Plan s investment options and their performance, as well as planning tools to help you choose an appropriate mix, are available online at Fidelity NetBenefits. What if I don t make an investment election? We encourage you to take an active role in the CentraCare Health 457(b) Plan and choose investment options that best suit your goals, time horizon, and risk tolerance. If you do not select specific investment options in the Plan, your contributions will be invested in the JPMorgan Smart Retirement Fund Class Institutional Shares with the target retirement date closest to the year you might retire, based on your current age and assuming a retirement age of 65, at the direction of CentraCare. 4 FAQs If no date of birth or an invalid date of birth is on file at Fidelity your contributions may be invested in the JPMorgan Smart Retirement Income Fund Class Institutional Shares. For more information about the JPMorgan Smart Retirement Fund Class Institutional Shares options, log on to What "catch-up" contribution can I make? You may elect to make a last-3-years catch-up contribution of up to double the contribution limit in effect (up to $36,000 in 2017). This contribution may be used in one or more of the three consecutive years prior to the year in which you attain normal retirement age under the Plan. This option gives you an opportunity to catch-up on contributions you could have made but did not make in previous years. See your Employee Benefits Office for more information. When am I vested? Vesting indicates the amount of your account balance to which you are currently entitled under the Plan s rules. You are always 100% vested in your own contributions, as well as any earnings on those contributions, to the CentraCare 457(b) Plan. Can I take a loan from my account? Loans are not available in this Plan. How are my salary deferral contributions and earnings on those amounts taxed? Amounts that you defer plus earnings generally are not taxable for Federal or Minnesota income tax purposes during the year in which the compensation is deferred. However, these amounts (contributions plus earnings) will be taxed as ordinary income when they are distributed to you from the Plan. Unlike 403(b) accounts, there is no penalty for withdrawal before age 59½ or for an unforeseeable emergency. employment with CentraCare, whether due to retirement, disability, death, or otherwise. If you wish to defer immediate receipt of your account, you must make an election within 30 days following the termination of your CentraCare employment. If an election to defer distribution is not made within this 30- day period, Fidelity Investments will automatically distribute the balance of your account to you in a single lump sum payment no later than 60 days after the end of this 30- day election period. Keep in mind that withdrawals are subject to income taxes. Payment of your account will begin on the date selected by you during the 30-day election period. EXAMPLE: Sam s employment with CentraCare terminated when he was 45 and, during the 30- day period following the termination of his employment, he elected to defer distribution until January 15, 2015, when he will be 55. If, upon leaving CentraCare, you accept employment with another tax-exempt employer which maintains a 457(b) plan, then you may choose to transfer their distribution directly to the new employer s 457(b) plan. This would require prior written approval of the new employer, and is dependent on the terms of the new employer s 457(b) plan. Does the Plan provide for hardship withdrawals? Yes, hardship withdrawals are defined as an "unforeseeable emergency" and are subject to review and approval by the Plan Administrator. Generally, an "unforeseeable emergency" means a severe financial hardship resulting from a sudden and unexpected illness or accident involving you or a dependent, loss of your property due to casualty, or other similar extraordinary and unforeseeable circumstances arising as a result of events beyond your control. Can I make withdrawals from my account? You may initiate a distribution from your account any time after you terminate How do I access my account? BlackRock Strategic Income Opportunities Portfolio Institutional Shares VRS Code: Fund Objective: The investment seeks total return as is consistent with preservation of capital. Fund Strategy: The fund normally invests in a combination of fixed income securities, including, but not limited to: high yield securities, international securities, emerging markets debt and mortgages. 14 Investment Options The Russell 2000 Index is an unmanaged market capitalization weighted index measuring the performance of the smallest 2,000 companies in the Russell 3000 index. Hartford International Small Company Fund Class Y VRS Code: Fund Objective: The investment seeks capital appreciation. Fund Strategy: Under normal circumstances, the fund invests at least 80% of its assets in equity securities, including non-dollar securities, of foreign small-capitalization companies. The fund diversifies its investments among a number of different countries throughout the world, with no limit on the amount of assets that may be invested in each country. The fund s investment approach is to invest in equity securities of foreign issuers that the sub-adviser, Wellington Management Company LLP ("Wellington Management"), believes have significant potential for capital appreciation.
