1 Public Works Menlo Park Municipal Water Water Connections Guidelines & Application October 2017 There will be additional fees to install a new water meter or to upsize an existing water meter. Submit your application and plans to the following: By Submit to water@menlopark.org with the subject WATER CONNECTIONS Service Address In Person: Submit to the front desk staff at the City s Administration Building (701 Laurel St., Menlo Park) with ATTN: Water Connections. These guidelines were developed to assist property owners in the Menlo Park Municipal Water service area to obtain a new meter service connection, or to upsize an existing meter service connection. These only apply to properties located within Menlo Park Municipal Water. Applicants with properties not served by Menlo Park Municipal Water should contact their respective water provider directly. To determine your water provider, visit click on the map, select the search icon, and enter your address. WHAT TO SUBMIT 1. An application, see attached (one application per parcel). 2. A copy of the stamped and approved Menlo Park Fire District plans. The approved Fire District plans must show the following: a. Location and size of the existing meter and the new proposed meter. b. Location and size of the existing water service line and the new proposed water service line (water service lines connect the water meter to the water main). c. Fire flow information, including the date of the fire flow test, used to design the fire sprinkler system. NOTE: Fire flow tests are valid for 12 months only, from the date of the fire flow test to the date the City receives a completed Water Connection Application and fire sprinkler plans approved by the Menlo Park Fire District. d. For residential properties two stories or less, either a passive-purge fire sprinkler system OR a backflow prevention assembly. If a passive-purge fire sprinkler system is installed, it must be clearly noted on the approved Fire District plans. If a backflow prevention assembly will be installed, show location, size, model, and type. a. The backflow prevention assembly must be approved by the University of Southern California s Foundation for Cross-Connection Control, installed per City Standards, tested by an approved backflow prevention tester listed on the San Mateo County Environmental Health s list of Certified Backflow Prevention Testers, and tagged with a County tag. Assemblies must be tested by the owner upon installation and annually thereafter. b. Prior to the City initiating water service, the owner must test all backflow prevention assemblies and submit passing test results to water@menlopark.org. c. San Mateo County Environmental Health manages the City s Cross-Connection Control Program. 2 DESIGN GUIDELINES 1. All work shall be done per City Standards. 2. The City will provide the following services as part of the water connection: a. Provide meters (all sizes). b. Install meters 2 size and smaller. c. If the water service line (the water line between the water main and the meter) will be replaced: May provide the wet tap connection to the water main depending on pipe material. Provide a materials list that the contractor must purchase for meters 3 size and larger. d. Shutdown of water main, if needed. e. NOTE: If City services are provided outside of business hours (evenings or weekends) that require Water Maintenance staff to be available, additional costs may be incurred and will be included in the Fees described below. 3. If the water service line (the water line between the water main and the meter) will be replaced, the owner has the option to hire their own Class A contractor to perform the work within the public right-of-way or request the City obtain quotes and select a contractor. The contractor shall provide the following services as part of the water connection: a. Trenching b. Install water service line c. Install water meter d. Purchase and install items from the City-provided materials list (for water service lines 3 size and larger) e. AC paving and slurry-seal f. Striping, if applicable g. Concrete work (i.e. sidewalk, curb, gutter), if applicable FEES If the owner chooses to hire their own contractor, the City must approve the contractor before any work is done. Submit the following information for City approval: Contractor name, address, and contact information Class A License # Certificate of Insurance The certificate must name the City of Menlo Park as an additional insured, and minimum insurance limits must be met. See below for insurance requirements. 1. If you are upsizing to a larger meter, the City will include a Water Capacity Charge credit based on the existing meter size. Water capacity charges can be found in the City s Master Fee Schedule for the current fiscal year. 2. Fees include: a. Encroachment Fee b. Water Capacity Charge for the new meter (and credit for old meter), if applicable c. Fire Service Fee, if applicable d. Contractor construction cost, if applicable e. Engineering/Administration Fee Water meter and supplies Staff time to review/process your application and install the water meter

3 25% of construction cost for new meters 3 size and larger if the water service line is replaced. f. Additional costs if City services will be provided outside of business hours (evenings or weekends) that require Water Maintenance staff to be available. ONCE THE APPLICATION IS SUBMITTED 1. City reviews the application and contacts the owner with questions, if any. 2. City meets the contractor in the field to verify project and materials needed, including: a. Verify location, size, and material of the existing water main. b. Confirm distance between the existing water main and the new meter. 3. City obtains contractor quotes, if applicable. 4. City sends applicable fees to applicant for payment. 5. Applicant pays fees. 6. Once applicant pays the fees, the contractor obtains the Encroachment Permit, and the City issues a Notice to Proceed. 7. The City verifies that the backflow prevention assembly, if applicable, has been installed and tested, and receives passing test results from the applicant. 8. The City conducts inspections on the open trench, water service installation, backfill, compaction, and final repaving. 9. The City approves the project and updates water account information. Insurance Requirements The following minimum insurance coverage is required in order to perform work within the City of Menlo Park. 1. Commercial/General Liability Insurance minimum coverage: $1,000,000 per occurrence, $2,000,000 aggregate. 2. Automobile Liability Insurance minimum coverage: $1,000,000 for each occurrence combined single limit, or $1,000,000 for any one person and $1,000,000 for any one accident, and $300,000 property damage 3. Workers Compensation and Employers Liability Insurance minimum coverage: Limits as required by the Labor Code of the State of California employers liability limits of $1,000,000 per accident 4. Professional Liability Insurance protecting the contractor against claims arising out of the negligent acts, error and omissions of contractor pursuant to the agreement, minimum coverage: $1,000,000 combined single limit. Said professional liability insurance is to be kept in force for not less than one year after completion of services described in the contract. The City of Menlo Park shall be named as additionally insured on contractor s insurance policy or policies. Upon notice of contract award, contractor shall provide the City with a certificate of insurance coverage evidencing the above coverage limits, including a copy of all declarations of exclusions, prior to execution of contract. To the full extent permitted by law, contractor agrees to defend, indemnify and hold City, its employees, agents and officers harmless from any and all claims, damages and liability in any way occasioned by or arising out of the contractor s negligent performance of services under this agreement, breach of contract or construction defects arising out of contractor s work.
