1 Confucius Institute Sunday Chinese School Registration Information Packet Below is a list of forms required for the Registration process. Please complete all forms, mail it back to Confucius Institute Office. Office address: MB 1.209, One UTSA Circle, SA, TX, Cover Page Please add this page to the front of the packet and staple together. It will help us with the registration process. The packet will include at least two forms listed below, with the possibility of multiple registration pages (one per students). Registration Required for all students. Correct information is VERY important on the registration form, required for all students. If you would like to order Sunday Chinese school t-shirts, please fill out the bottom portion of the registration form. Waiver The waiver form is required for insurance purposes and we need ONE per student. Volunteer A Hallway Monitor needed for at least twice a semester to ensure the safety of our children. Volunteer is Optional. However, if you would like to lend a hand, please fill out the form and turn it in together with your Registration package (this document). Book Order information can be found on the cover page. All text books are NOT included in the registration fee, and need to be purchased separately. Book Order The Ma Li Pin s text books will be offered to all Heritage Chinese classes, the complete set includes one textbook, two homework books, one summer practice book and one CD. The Better Chinese textbook will be offered to all non-heritage classes, the complete set includes one text book, two homework books. Chinese for Managers will be offered to adult business Chinese classes. Great Wall Chinese will be offered to Adult beginner and Intermediate Chinese classes. If the Participant chooses to drive or to be a passenger in a personally-owned vehicle for any travel associated with the Activity/Trip, my signature below also acknowledges the following: I understand that the Institution assumes no liability or responsibility for the use of such vehicle during any such travel and that the Institution has neither inquired about nor confirmed (a) the driving history, training or licensure of the driver or (b) that the vehicle is covered by a private automobile insurance policy; I understand that the Institution carries no insurance that could cover any damages, injuries, claims or other liabilities associated with the use of such vehicle during such travel; and I understand that any private insurance policy covering such vehicle or the driver personally will be responsible for anydamages, injuries, claims or other liabilities that may arise from the use of the vehicle during any travel associated with the Activity/Trip. I further understand that pictures of my children and me may be taken at the Sunday Chinese School Program. I am also aware that pictures may be posted to a website, and will be used for the purpose of illustrating or advertising the activities of the program. In consideration of Participant being permitted to participate in the Activity/Trip, I hereby accept all risk to Participant s health, including any injury or death to Participant that may result from such participation, which participation includes all travel associated with the Activity/Trip, and I hereby release UTSA, its governing board, officers, representatives, employees and agents from any and all liability to Participant, Participant s personal representatives, estate, heirs, next of kin, and assigns for any and all claims and causes of action for loss of or damage to Participant s property and for any and all illness or injury to Participant s person, including his or her death, that may arise from or occur during Participant s participation in the Activity/Trip, WHETHER CONTRIBUTED TO OR CAUSED BY ANY NEGLIGENCE OF UTSA, ITS GOVERNING BOARD, OFFICERS, EMPLOYEES OR REPRESENTATIVES, OR OTHERWISE. 5 The Confucius Institute at University of Texas at San Antonio Sunday Chinese Culture and Language School Parent Hallway Monitor Sign up Sheet Dear Parents: Thank you for your staunch support for our Sunday Chinese Culture and Language School in the year ! We have had a wonderful and successful year. In the upcoming school year, we will continue our effort to make our Chinese language and cultural education at the Confucius Institute one of the best in San Antonio! At the same time, we will need help from our parents to be the Hallway Monitor for at least twice a semester to ensure the safety of our children. Below is the calendar of our next semester s classes, please list your 1 st, 2 nd, 3 rd and 4 th choice of your participation date. We will adjust based on your choices and try to accommodate your schedule as much as possible. We will consider every parent s availability, and work out a final schedule that is convenient for everybody. Again, thank you for your participation!
