1 June 25, 2015 REQUEST FOR PROPOSAL RP The Gwinnett County Board of Commissioners is soliciting competitive sealed proposals from qualified service providers for the Provision of Custodial Services at Various Gwinnett County Facilities on an Annual Contract with four (4) options to renew for the Department of Support Services. Proposals must be returned in a sealed container marked on the outside with the Request for Proposal number and Company Name. Proposals will be received until 2:50 P.M. local time on Tuesday, July 21, 2015 at the Gwinnett County Financial Services - Purchasing Division 2 nd Floor, 75 Langley Drive, Lawrenceville, Georgia Any proposal received after this date and time will not be accepted. Proposals will be publicly opened and only names of submitting firms will be read at 3:00 P.M. A list of firms submitting proposals will be available the following business day on our website A pre-proposal conference is scheduled for 10:00 a.m. on Tuesday, July 7, 2015 at the Gwinnett County Purchasing Office, DOFS Birch Conference Room. All service providers are urged to attend. One unbound original and four (4) copies should be submitted. All suppliers must submit with bid, a bid bond, certified check or cashier's check in the amount of five percent (5%) of the total bid. Failure to submit a bid bond with the proper rating will result in the proposal being deemed nonresponsive. Successful supplier will be required to meet insurance requirements and must provide a $50,000 Fidelity Bond, submit a one hundred percent (100%) performance bond and a one hundred percent (100%) payment bond. Insurance and Bonding Company should be licensed to do business by the Georgia Secretary of State, authorized to do business in Georgia by The Georgia Insurance Department, listed in the Department of Treasury's Publication of Companies holding Certificates of Authority as Acceptable Surety on Federal Bonds and as acceptable reinsuring companies. The bid bond, payment bond, and performance bond must have an A.M. Best rating of A-5 or higher. 2 RP Page 2 PROVISION OF CUSTODIAL SERVICES AT VARIOUS GWINNETT COUNTY DEPARTMENT OF WATER SOURCES FACILITIES ON AN ANNUAL CONTRACT I. INTRODUCTION The Gwinnett County Board of Commissioners is soliciting proposals from qualified service providers for comprehensive custodial services for 30 facilities owned and operated by the Gwinnett County Government. These facilities are listed and described below in Section II.A Scope of Services/Locations and Cleaning Schedules. The services will be on an annual contract basis, with four (4) one (1) year options to renew. The Scope of Services is described in Section II.B Scope of Work, and more detail on service requirements and standards are contained in other paragraphs of Section II. Proposal Submission Requirement is detailed in Section III. Contract award will follow the procedures outlined in Section IV. II. SCOPE OF SERVICES A. Locations and Cleaning Schedules A listing and general descriptions of the facilities covered by this proposal are provided below. The areas for some of the County facilities listed are gross square footages, so the net areas cleaned actually will be somewhat less than the amounts noted. If the requested services only cover a portion of a facility, that portion of the building is noted. Also, if certain areas within a facility require different cleaning efforts or different schedules, those discrepancies also are noted; and the service areas are separated for pricing on the Proposal Fee Schedule. Finally, the frequency of cleaning, and the time of day specified for cleaning, are noted for each facility. 1. Buford Human Services Center 2755 Sawnee Avenue Buford, GA Size: 24,415 sq. ft. One Floor Characteristics: Waiting rooms, dining room with small kitchen, meeting and activity rooms, offices, medical exam rooms, and lab space. Flooring is approximately 75% VCT and ceramic tile, 25% carpet. Cleaning Schedule: 5 Days, Evening 2. Norcross Human Services Center 5030 Georgia Belle Court Lilburn, GA Size: 53,637 sq. ft. Two Floors Characteristics: Waiting rooms, transaction areas, dining room with kitchen, meeting and activity rooms, children s classrooms and bathrooms, offices,

3 RP Page 3 medical exam rooms and lab space. Flooring is approximately 24,600 sq. ft. carpet, 25,400 sq. ft. VCT, and includes some ceramic tile, EQF and sealed concrete. Cleaning Schedule: The cleaning scope and schedule for the Norcross Human Services Center is somewhat fragmented, and will require three separate pricings, which are described below: a. Entire Facility minus Norcross Tag Office (approximately 50,163 sq. ft.) Days, Evening. b. Norcross Tag Office (3,474 sq. ft.) Days, Daytime. c. Day Porter Services for Entire Facility Days (Monday through Friday) 8:00 A.M. - 5:00 P.M. 3. Courtrooms within Gwinnett Justice and Administration Center (GJAC) 75 Langley Drive Lawrenceville, GA Size: This work element includes only the courtrooms within the referenced facility. The building contains 22 courtrooms, which inclusive of entry vestibules have approximately 34,500 sq. ft. The courtrooms are spread over three floors in one wing of the facility. Characteristics: All carpet flooring, fixed bench spectator seating, jury boxes with post mounted chairs, significant amount of wood trim and millwork, and some fabric wall covering/panels. Cleaning Schedule: 5 Days, Evening. cleaning must conclude by 9:30pm 4. Gwinnett County Central Services Facility - - Public Health Functions 455 Grayson Highway Lawrenceville, GA Size: 45,922 sq. ft. (Within a larger building) Characteristics: Lobbies, large waiting rooms and transaction areas, offices (enclosed and open plan), numerous exam rooms (w/cabinets and sinks). Flooring is a mix of VCT and carpet. Cleaning Schedule: 5 Days, Evening 5. Gwinnett County Central Services Facility - - Records Management Offices/Warehouse, Common Building Corridors, Break Room/Restrooms 455 Grayson Highway Lawrenceville, GA Size: 35,302 sq. ft. (Within a larger building) Characteristics: Records Management offices/workrooms w/ floors a mix of carpet and VCT (3,116 sq. ft.); Common building corridors, break room, restrooms (7,484 sq. ft.) includes significant amount of VCT flooring. Records Management Warehouse (24,702 sq. ft.) is 75% occupied by storage racks, flooring is sealed concrete. Cleaning Schedule: Offices/Workrooms, Corridors, Break Room/Restrooms 5 Days, Daytime. Records Warehouse - - Monthly, Daytime.

The flooring in the branches is typically carpet with some tile in entry areas, ceramic tile in restrooms and VCT in break rooms and storage areas. Lawrenceville Tag Office (Within Gwinnett County Government Annex) 750 South Perry Street Lawrenceville, GA Size: 9,400 sq. ft. Characteristics: On first floor of above referenced facility. Waiting lobbies with transaction areas, offices, and call center. Flooring is a mix of VCT and carpet. Cleaning Schedule: 4 Days, Daytime.

Characteristics: On second floor of above referenced facility. Includes medical/exam rooms, offices, training room, specimen toilet rooms, break area and miscellaneous spaces. Flooring is a mix of vinyl and carpet. Cleaning Schedule: 5 Days, Evening 26. Georgia Department of Drivers Services/Lawrenceville 310 Hurricane Shoals Road Lawrenceville, GA Size: 10,344 sq. ft. Characteristics: Large public waiting room with transaction counter, offices and general office work areas. Flooring is VCT in public areas and carpet in the office areas. Cleaning Schedule: 5 Days, Evening 27. Gwinnett County Courts Annex (Recorders and Juvenile Courts 115 Stone Mountain Street Lawrenceville, GA Size: 77,400 sq. ft. finished space; Four Floors but only three built out. Characteristics: Lobbies and waiting areas, transaction areas, 6 courtrooms, 6 judges chambers, general offices (enclosed and open plan) meeting and conference rooms, classrooms, and prisoner holding areas. Flooring finishes have not been quantified but are primarily carpet with significant amounts of porcelain tile, VCT and EQF. Cleaning Schedule: Routine Services 5 days, 6:00AM 9:00AM Day Porter Services 5 Days (Monday through Friday) 9 A.M. 2 P.M. 28. One Justice Square 446 W. Crogan Street Lawrenceville, GA Size: 99,400 sq. ft. Characteristics: This building is a fully occupied facility with a high level of public traffic. Numerous offices (open plan and enclosed), lobbies and waiting areas, transaction counters, conference rooms, high density filing, supply rooms, break rooms, and public restrooms. The office areas are predominantly carpet with some VCT and traditional linoleum. Restrooms are tile. Public areas, elevator lobbies, and corridors include some marble tile on first floor, but carpet in all other locations. Cleaning Schedule: 5 Days, Evening

8 RP Page Gwinnett County Senior Services Center 567 Swanson Drive Lawrenceville, GA Size: 20,000 sq. ft. Characteristics: The front half of the building is typical office space with both open plan areas and enclosed offices, conference rooms, filing and storage areas, restrooms and a break room. Most flooring is carpeting. The back half of the facility is a large kitchen operation that is used for preparing foods for the senior services dining programs. It includes large freezers and coolers, dry storage, an open food preparation and cooking area, some office support and restrooms. Cleaning Schedule: 5 Days, Evening 30. Georgia State Patrol Offices 1645 Pleasant Hill Road Duluth, GA Size: 2,535 sq. ft. Characteristics: This space is actually rented by the County to provide a temporary location for a Georgia State Patrol unit. The space is typical office space, both open plan and enclosed. Flooring is mostly carpet. Cleaning Schedule: 5 Days, Evening B. Scope of Work 1. General The Service Provider shall furnish all labor, supervision, materials, equipment, cleaning products, hand soaps, paper goods, and trash liners necessary to perform satisfactorily the work of the contract. The services shall include all functions normally considered a part of janitorial work. All services are to be provided for the public entrance ways and interiors of the specified buildings. All services (except specified day time cleaning and day porter services) shall be accomplished during periods of the day that will not interfere with the building occupants or the operations of the facilities. The County reserves the right to add or delete locations and services during the contract period at a comparable rate to be mutually agreed upon by the County and the Service Provider. 2. Hours of Operations Routine daily cleaning for all facilities except those as specified differently shall be conducted Monday through Friday after 7:00 P.M.

9 RP Page 9 Routine daily cleaning for the Library Branches will be carried out seven days a week according to the following schedule: Monday, after 8:00 P. M. Tuesday, after 8:00 P. M. Wednesday, after 8:00 P. M. Thursday, after 8:00 P. M. Friday, after 6:00 P. M. Saturday, after 5:00 P. M. Sunday, after 5:00 P. M. Routine daily cleaning of the Gwinnett County Central Services Facility - - Records Management Offices, Common Building Corridors, Break Room, and Restrooms is to be carried out five days per week in the daytime hours as scheduled by the Department of Support Services Housekeeping Supervisor. Routine daily cleaning of the Courts Annex is to be carried out five days per week between 6:00 A.M. and 9:00 A.M. Routine daily cleaning of the GJAC Courtrooms is to be carried out five days per week between 6:00 P.M. and 9:30 P.M. The cleaning crew must exit the building by 9:30 P.M. Routine daily cleaning for the five Tag Offices will be carried out four days a week on the days of operation of the individual Tag Offices. Cleaning services will be provided only while Tag Office staff are present between the hours of 8:00 A.M., and 6:00 P.M., as can be scheduled. The Records Management Warehouse is to be cleaned on a monthly basis. These cleanings will be in daytime hours on days and at times arranged with the facility occupants and the Department of Support Services Housekeeping Supervisor. The monthly cleaning tasks for all facilities required under the General Cleaning Specifications below are to be deep cleaning efforts through which the Service Provider can furnish, and the County obtain, more comprehensive and detailed cleaning than is allowed for in routine daily efforts. As such, the County will expect the Service Provider to provide extra staff and/or time for these cleaning tasks and to schedule them, with prior notification to Department of Support Services Housekeeping Supervisor, on Saturdays or on evenings when County oversight can be provided. The monthly cleanings will be grouped and staggered in such a manner that facilitate this County oversight. All work completed must be acknowledged with service completion forms to be submitted by the 1 st week of the next month. If scheduled on Saturdays the monthly cleaning still shall be a part of basic services; and there shall be no additional charge.

10 RP Page General Cleaning Specifications All areas in which the Request for Proposal requires cleaning shall be maintained to meet the standards and expectations determined by the Gwinnett County Department of Support Services. These specifications describe in general the types and frequency of routine cleaning efforts expected in various areas of the buildings. Inadvertent omission of a cleaning detail that would be considered a typical janitorial service does not preclude that item from being included in the required services. a. Public Entrance Ways - - Exterior i. Daily Empty and dispose of trash from all wastebaskets and ash urns located in plazas or walkways within 30 feet of the main entrance doors. Remove and dispose of any trash in planted areas adjacent to said walkways and plazas. Thoroughly sweep hard surfaced plazas and walkways from entrance doors to points 30 feet from the doors. b. Public Corridors, Lobbies, Elevator Interiors, Waiting Rooms, Courtrooms, Conference Rooms, and Classrooms. i. Daily Empty waste containers and dispose of trash. Replace liners if soiled. Where the County s Recycling Program is in effect, empty designated recycling containers and dispose of materials in designated containers as directed by County representatives. Spot clean all glass doors, storefront glass, and other interior glass panels or walls. During flu season (November February) disinfect all surfaces touched with hands. Spot clean aluminum and stainless frames and panels. As applicable, sweep and damp mop quarry/porcelain tile or vinyl composite tile (VCT) flooring. As applicable, vacuum carpet. Include runners and walk-off mats. ii. Weekly Change liners on all trash containers and damp clean containers as needed. Clean and sanitize drinking fountains. Dust and/or damp clean counter tops and exposed desk/table tops or other horizontal surfaces on furniture, millwork, or architectural fixtures.

11 RP Page 11 Spot clean to remove obvious scuff marks, smudges, dirt, and finger prints from wood and metal doors, door frames, door push/pull plates or handles, painted and vinyl walls, laminated millwork and cabinetry, and wood finished millwork, rails or panels. Clean and spray buff VCT floors. iii. Monthly Remove cobwebs and gathered dust from wall corners and ceiling areas (accessible from 6 ft. ladders). Clean (and polish as needed) all counter tops and exposed desk/table tops or other horizontal surfaces on furniture, millwork, or architectural fixtures. Clean wood and metal doors, door frames, door push/pull plates or handles, painted and vinyl walls, cove base, laminated millwork and cabinetry, and wood finished millwork, rails and panels. Vacuum all upholstered furniture (seating and upholstered panels). Clean all glass doors, storefront glass, and other interior glass panels or walls. Clean aluminum and stainless frames and panels. As applicable, clean and polish quarry/porcelain tile, linoleum, or vinyl composite tile using products and equipment as required by flooring manufacturers and standard industry practices to maintain material s appearance. Dust window blinds and window sills. c. Public and Private Restrooms i. Daily Clean and polish all metal, glass and porcelain fixtures including toilet bowls, urinals, sink basins, vanity surfaces, mirrors, towel dispensers, waste containers, and miscellaneous fixtures or accessories. Clean and sanitize all seats, handles, flush rings, drains, and overflow outlets. During flu season (November February), disinfect all surfaces regularly touched by hands. Empty waste containers and dispose of all trash. Replace liners. Replenish all dispensers for paper towels, toilet paper and hand soap. Sweep and then mop floors thoroughly with disinfectant cleaner. ii. Weekly Damp wipe and spot clean all restroom stall partitions and doors. Spot clean to remove obvious spots, stains and splash marks from semi-gloss painted walls, vinyl wall covering, and/or tile walls.

12 RP Page 12 Spot clean to remove obvious scuffmarks, smudges, dirt and fingerprints from doors, doorframes, door push/pull plates or handles, and light switches. Clean and sanitize latter two items. Clean toilet bowls and urinals with sanitizer. Maintain a liquid level in all floor drains with an approved disinfectant. iii. Monthly Thoroughly wash and sanitize all restroom stall partitions and doors with germicide detergent. Thoroughly wash and sanitize all semi-gloss painted, vinyl and ceramic tile walls with germicide detergent. Clean tile floors with powered cleaning floor scrubber. d. Offices, Open Work Areas, Senior Center Activity Areas, Open Library Areas (Stacks and Reading Areas), and Miscellaneous Work Areas (other than Exam Rooms and Laboratories). i. Daily Empty waste containers and dispose of trash. Replace liners if soiled. Where the County s Recycling Program is in effect, empty designated recycling containers and dispose of materials in designated containers as directed by County representatives. Spot clean all glass doors, storefront glass and other interior glass panels and walls. During flu season (November February) disinfect all surfaces touched with hands. As applicable, sweep and damp mop vinyl composite tile (VCT) As applicable, vacuum carpet in corridors, conference/meeting rooms, open library areas, and other public or high traffic areas. ii. Weekly Change liners in all trash containers and damp clean containers as needed. Dust and/or damp clean counter tops, and exposed desk/table tops or other horizontal surfaces on furniture, millwork or architectural fixtures. In the Library Branches, staff periodically will clean materials off tables, desks and carrels to facilitate better cleaning of surfaces. No specific time is assigned, but this will be approximately weekly. A written note for the cleaning crew will be left one day prior to the clearing of materials. When materials are so cleared, thoroughly clean all exposed tables and desktops. (Do not remove books or materials from tops of tables or book ranges. Library staff will handle as described above.)

Clean wood and metal doors, door frames, door push/pull plates or handles, painted and vinyl walls, laminated millwork and cabinetry, and wood finished millwork, rails and panels. Vacuum all upholstered furniture (seating and upholstered panels) Clean all glass doors, storefront glass, and other interior glass panels or walls. As applicable clean and polish vinyl composite tile or linoleum using products and equipment as required by flooring manufacturer s and standard industry practices to maintain materials appearance. Dust window blinds and window sills. e. Staff Lounges, Kitchens, Senior Center Dining Rooms i. Daily Empty waste containers and dispose of trash, Damp clean containers and replace liners. Where County s Recycling Program is in effect, empty designated recycling containers and dispose of materials in designated containers as directed by County representatives. Clean lounge counter tops and backsplashes. Clean sink basins and surface clean appliances such as, vending machines, microwaves, refrigerators and coffee makers. Clean dining table tops and damp wipe seats and backs of chairs. Spot clean all glass doors, storefront glass, and other interior glass panels or walls. During flu season (November February) disinfect all surfaces touched with hands. Spot clean aluminum and stainless frames and panels. As applicable, sweep and damp mop quarry/porcelain tile, EQF, or vinyl composite tile (VCT) flooring, including in kitchen areas.

Weekly Spot clean to remove obvious scuff marks, smudges, dirt, and fingerprints from wood and metal doors, doorframes, door push/pull plates or handles, painted and vinyl walls, laminated millwork and cabinetry, and wood finished millwork, rails and panels. Clean and spray buff VCT floors. iii. Monthly Remove cobwebs and gathered dust from wall corners and ceiling areas (accessible from 6 ft. ladder). Thoroughly clean and sanitize all lounge counter tops, back splashes, sink basins, and applicable surfaces. Clean wood and metal doors, door frames, push/pull plates or handles, painted and vinyl walls laminated millwork and cabinetry, and wood finished millwork, rails and panels. Thoroughly clean and sanitize all dining table tops and seats and backs of chairs. Also clean legs on tables and chairs. Clean all glass doors, store front glass, and other interior glass panels and walls. Clean aluminum and stainless frames and panels. As applicable, clean and polish quarry/porcelain tile, EQF, linoleum or vinyl composite tile using products and equipment as required by flooring manufacturers and standard industry practices to maintain material s appearance. Dust window blinds and window sills. f. Medical Examination Rooms and Laboratories (In Human Services Centers and Central Services). These areas require the use of latex gloves during all cleaning procedures. Do not remove items from hazardous biological waste containers. i. Daily Empty general waste containers, dispose of trash, and replace liners if soiled. Clean counter tops, backsplashes and sink basins. Sweep and damp mop vinyl composite tile (VCT) During flu season (November February) disinfect all surfaces touched with hands. ii. Weekly Change liners in all general trash containers and damp clean containers as needed. Spot clean to remove obvious scuff marks, smudges, dirt, and fingerprints from wood and metal doors, door frames, door

15 RP Page 15 push/pull plates or handles, painted and vinyl walls, laminated millwork and cabinetry, and wood finished millwork, rails or panels. Clean and spray buff VCT floors. iii. Monthly Remove cobwebs and gathered dust from wall corners and ceiling areas (accessible from 6 ft. ladder). Thoroughly clean and sanitize all counter tops, back splashes and sink basins. Clean all other laminated millwork surfaces. Clean wood and metal doors, door frames, door push/pull plates or handles, painted and vinyl walls, cove base, and wood finished millwork, rails and panels. Vacuum all upholstered furniture. Clean and polish vinyl composite tile using products and equipment as required by flooring manufacturer and standard industry practices to maintain material s appearance. Dust window blinds and window sills. g. Cleaning of Records Management Warehouse The Service Provider will clean the Records Management Warehouse on a monthly basis on days to be coordinated with the Department of Support Services. The monthly cleaning shall include the following tasks: Vacuum/sweep and damp mop all exposed areas of flooring (sealed concrete). Dust and wipe clean all empty storage shelves and tops of shelving units. h. Day Porter Services at Norcross Human Services Center and Courts Annex In addition to the regular or routine cleaning services provided for herein, the Service Provider shall provide building attendant or day porter services for the Norcross Human Services Center and the Courts Annex. These services are to be provided for the Norcross Human Services Center Monday through Friday from 8:00 A.M. to 5:00 P.M and for the Courts Annex Monday through Friday from 9:00 A.M. to 2:00 P.M. The day porter shall provide services to include policing the main entrance areas (including exteriors within 30 feet of entrance doors) and public areas of the buildings for trash and spills, emptying public trash cans as they become full, cleaning and restocking public and staff restrooms as required, providing intermittent dusting, mopping and vacuuming of floors, and cleaning/polishing glass and metal surfaces to keep a neat appearance within the buildings.

16 RP Page 16 C. Green Cleaning i. Requirements and Standards for Floor Cleaning and Maintenance. Carpet shall be vacuumed with commercial grade machines at the frequencies noted above. The vacuuming equipment shall meet the standards spelled out elsewhere herein. Where vacuuming is specified, the Service Provider shall include vacuuming under desks, tables and chairs; and behind doors. The Service Provider shall sweep and damp mop all vinyl composite tile (VCT) daily. In exam rooms and laboratories, VCT floors shall be mopped with a disinfectant. The Service Provider shall spray/buff all VCT weekly and shall strip and rewax all VCT floors every six months as part of its basic services. The Service Provider shall sweep and damp mop all quarry/porcelain tile daily. For weekly and monthly cleanings, the Service Provider shall clean the floors using products and equipment as required by flooring manufacturers and industry practices to maintain material s appearance. At least monthly the Service Provider shall clean ceramic tile floors in the restrooms with a powered scrubber. The County requests that the Service Provider present as a part of its proposal a program for Green Cleaning. Green Cleaning is defined as cleaning to protect health without harming the environment. The County asks that the Service Provider describe a cleaning program, inclusive of equipment, chemical choices, policies and procedures that minimize the impact of the cleaning process on the health of occupants and protect the environment as a whole. The selected Service Provider will be expected to implement its proposed program. This program should include specific chemical cleaning products, paper products, trash can liners, and powered janitorial equipment needed to meet Green Cleaning objectives. Proposed products and equipment should be based on requirements and recommendations of the U.S. Green Building Council (LEED for Existing Buildings Rating System), Green Seal, Environmental Choice (CCD Standards) and the Carpet & Rug Institute (CRI Green Labeled Standards). The County operates a waste recycling program at many of the facilities included in these services. Where such a program exists, the Service Provider shall strictly adhere to guidelines established by the County for separation of recyclables from other waste materials. D. Equipment, Materials and Supplies 1. The Service Provider will supply all vehicles, equipment and tools needed to perform the cleaning services as described herein. The County reserves the right to inspect any and all equipment and tools to be used by the Service Provider. The Service Provider also shall provide all chemical cleaning products, soaps and other supplies needed to perform the cleaning services as described herein. The Service Provider shall provide the County with a listing of these products and

17 RP Page 17 supplies together with information on where and how they will be used as well as material safety data sheets (MSDS) on any and all chemicals. The County reserves the right to review these lists and reject the use of any equipment or materials it deems to be substandard or unsuitable. The use of products containing phosphates, chlorine, or ammonia is prohibited. The Service Provider shall provide all consumable cleaning materials, such as sponges, cloths, mop heads and dusters. The Service Provider shall provide all hand soap, paper towels, toilet tissue and trash can liners required at the serviced facilities, except at the 15 Branch Libraries. The County will provide soap, paper supplies and liners for these latter facilities on a schedule to be agreed upon by the Department of Support Services and Service Provider. 2. In provision of all of the above products, supplies and equipment, the Service Provider will submit documentation to demonstrate compliance with the Green Cleaning program elements accepted by the County. 2. The County will designate and provide limited storage space in each serviced facility for the Service Provider to maintain a stock of paper goods, trash liners and cleaning tools, products or materials to be kept on site. The County will provide for electrical power and water at all facilities to be cleaned. E. Service Provider s Personnel and Supervision 1. The County s preference is that the Service Provider utilizes employees with direct employee/employer relationships with the Service Provider. Nevertheless, we realize that many custodial services firms use sub-contracts or combinations of sub-contracts and directly hired personnel to provide their services. Thus, these types of arrangements will be acceptable. In such cases, the prime Service Provider shall provide the names of key sub-contractor companies or individuals and include descriptions of their experience and capabilities in the housekeeping field and explanations of their relationships with the prime Service Provider. In all cases, the Service Provider shall make very clear in its proposal the hiring practices, training, quality standards, and operating rules and regulations that will apply to all staff inclusive of management, supervisors and field staff. If subcontractors are utilized, the Service Provider shall maintain administrative control over all such companies and/or individuals and shall be knowledgeable of and responsible for resource allocations, scheduling and work quality. 2. As a part of the proposal and contracting process, the Service Provider shall sign a Contractor Affidavit and Agreement that verifies its compliance with the Illegal Reform Enhancements for 2013 and O.C.G.A , stating affirmatively that as a company it is participating in a federal work authorization program (any of the electronic verification of work authorization programs operated by the United States Department of Homeland Security or any equivalent federal work authorization program to verify information on newly hired employees, pursuant to the Immigration Reform and Control Act of 1986

18 RP Page 18 (IRCA), P.L ), in accordance with the applicability provision and deadlines established in O.C.G.A Compliance with these E-Verify provisions is an essential part of the Service Provider s agreement with the County. Moreover, if the employees of the Service Provider were hired prior to E- Verification procedures described above, the Service Provider still will be held responsible for insuring that its employees are legal resident and authorized workers with the United States; and the Service Provider shall provide to the County the Department of Homeland Security Form I-9, Employment Eligibility Verification for any persons who will be providing services to the County under the terms of this proposal. 3. Upon award of these services, the Service Provider shall submit to the County the names and personal information on all employees assigned to these custodial services. The information presented shall meet the requirements of Paragraph F.1 below and shall be adequate for the County to determine the employees work eligibility and to perform criminal record checks on the employees. No person shall be assigned to work in a County facility until clearances of eligibility and criminal history have been obtained. The County also reserves the right to prohibit any employee from assignment to these services if it determines that such prohibition is in the County s best interest. The Services Provider shall notify the County within one business day when a change has occurred or is needed in the personnel assigned to these services. The Service Provider shall allow the County 72 hours to clear new personnel once adequate documentation has been submitted by the Service Provider. The Service Provider also shall notify the County within one business day when there is a change of staff between buildings. 4. Service Provider shall require its employees to comply with the instructions herein pertaining to conduct in delivery of services. Service Provider and its employees shall observe all building policies, rules, regulations, and schedules as established and required by the County. All employees of the Service Provider shall wear uniforms that identify the company providing services, and the employees shall wear County issued identification cards when in service. Identification cards must always correspond to the persons wearing them; they shall not be swapped between personnel. No smoking by Service Provider employees shall be allowed in any of the facilities covered by these services. Service Provider s employees shall not disturb papers on desks, open file drawers or cabinets, or use telephones, computers or other office equipment. Service Provider s staff shall not unplug equipment from electrical outlets. The Service Provider will be liable for correction of problems created by the disconnection of power. If Service Provider s personnel must move furniture to conduct cleaning services the personnel must restore the furniture to its original location. Service Provider shall staff its services so that there is always at least one staff person present at a service delivery site who speaks, reads and interprets the English language fluently. All supervisors, team leaders, and day porters must be fluent in English.

19 RP Page The Service Provider shall provide adequate supervision for all janitorial services performed. The Service Provider shall identify all supervisors in writing to the County and shall maintain this information on a current basis. The phone numbers for supervisors shall be made available to the Department of Water Resources. Designated supervisors shall be available by phone at all times that the custodial services are in progress. All supervisors shall speak, read, and interpret English fluently. F. Security and Building Access 1. The Service Provider shall furnish the Gwinnett County Department of Support Services a list of all staff that will be working in the facilities, with an indication of which staff will service which buildings. Prior to authorizing any of these personnel to work in County facilities, the County shall require the Service Provider to submit for each employee documentation that enables the County to assess the employee s work eligibility and criminal history. The Service Provider must provide the following for each employee for which it is seeking authorization and clearance: a) A copy of the Department of Homeland Security I-9, Employment Eligibility Verification for the person for whom authorization is sought. This form shall be the form submitted by the employee at the start of his or her employment with the firm. b) Copy of an unexpired and acceptable Document of Identity as listed in the I-9 Form Instructions regarding Employment Eligibility Verification. c) (If Applicable) A copy of an E-Verify Program form for the person for whom authorization is requested. This form must have the notation Resolved/Authorized in the Resolution Section. This requirement applies to employees hired since January 1, d) A signed County Consent Form authorizing the County to run a criminal history check on the person for whom authorization is requested. If these materials are not provided in full, the Service Provider will be notified that the subject employee cannot be considered. If the review of information indicates fraudulent documents or a criminal history, the subject employee also will be rejected. For all employees who provide complete documentation and receive security clearance, the County will provide authorization to work under the contract and will issue County Identification/Access Cards with photographic images. The employees must wear the identification/access Cards whenever providing services on County property. The Service Provider shall insure that Identification/ Access Cards are returned to the County when individuals leave the company s employ, and when the Service Provider s services end. 2. The Service Provider will be required to maintain a logbook at each facility serviced. This logbook will be used for sign-in and sign-out purposes, and to

20 RP Page 20 indicate any facility maintenance issues of which the County needs to be kept aware. 3. The Service Provider shall not permit access to the facilities it services by anyone other than its employees. The Service Provider shall under no circumstances allow children or unauthorized personnel to accompany staff while staff is performing its custodial services in the facilities. 4. All of the Service Provider s staff who will access any of the facilities with security alarm systems will be issued keypad entry code numbers and pass codes for the applicable facilities. The Service Provider shall make every effort to insure staff is trained to avoid creating false alarms. (Nevertheless, if upon entering a facility, an employee accidentally activates an alarm, the employee shall call the security monitoring company, identify himself, and give the building s account number, phone number and the pass code as requested by the security monitoring company. This process will enable the company to cancel the false alarm). The Gwinnett County Department of Support Services will provide appropriate access cards and keys to the Service Provider for access to all facilities covered under these services. The Service Provider shall maintain control of these access cards and keys at all times, and insure that they are not duplicated for any reasons. Service Provider shall report lost keys and access cards to the Gwinnett County Department of Support Services in writing detailing the circumstances surrounding the loss within twenty-four (24) hours. Upon termination of the contract for services, the Service Provider shall return all keys and access cards to the Department of Support Services within twenty-four (24) hours. The Service Provider shall not give these above referenced codes, pass codes, or keys to anyone except its employees who will be performing services on the County s buildings. G. Damages to County s Property Any damage to the County s facilities or to equipment, furniture or finishes therein caused by the Service Provider or its employees, whether by neglect, negligence or improper performance of its services, shall be repaired at the Service Provider s expense to the County s satisfaction. III. PROPOSAL SUBMISSION REQUIREMENTS A. General Requirements 1. Individuals, firms and businesses seeking an award of a Gwinnett County contract may not initiate or continue any verbal or written communications regarding a solicitation with any County officer, elected official, employee or other County representative without permission of the Purchasing Associate named in the solicitation between the date of the issuance of the solicitation and the date of the final contract award by the Board of Commissioners. Violations will be reviewed

21 RP Page 21 by the Purchasing Director. If determined that such communication has compromised the competitive process, the offer submitted by the individual, firm or business may be disqualified from consideration for award. This is to insure that all prospective respondents have the same level of knowledge of the project as well as insuring the additional data is made available to all proposers. 2. All questions or requests for additional information shall be addressed to Cindy Stokes, Contracts Manager, Gwinnett County Purchasing Division at or by facsimile , 3. One (1) unbound original (marked as the original ), four (4) bound copies, and one (1) disc in Adobe PDF format of your proposal should be submitted. Proposals shall be submitted in a sealed envelope/package. Within the sealed package, the cost proposal must be enclosed in its own envelope, marked Cost Proposal on the outside of the envelope and sealed to keep it separate from the other proposal components. All copies of the proposal should be identified with the proposal number, date of opening, and the proposing company s name. All copies of the proposal must be identical. The full cost of proposal preparation is to be borne by the proposing firm. Proposals must be signed in ink by a company official who has authorization to commit company resources. 4. Proposals shall be submitted in a sealed envelope/package. Envelope/package shall be addressed to Gwinnett County Purchasing Division, Gwinnett Justice and Administration Center, Second Floor, 75 Langley Drive, Lawrenceville, Georgia and shall be identified with the proposal number, date of opening and company name on the outside. 5. Proposals submitted are not publicly available until award by the Gwinnett County Board of Commissioners. All proposals and supporting materials as well as correspondence relating to this RFP become property of Gwinnett County when received. Any proprietary information contained in the proposal should be so indicated. However, a general indication that the entire contents, or a major portion, of the proposal is proprietary will not be honored. 6. Gwinnett County reserves the right to reject any or all proposals, in whole or in part, to negotiate changes in the scope of services and to waive any technicalities as deemed in its best interest. 7. Gwinnett County Request for Proposal documents are posted on the Gwinnett County web site in PDF format for interested parties to download. It is also the sole responsibility of all interested parties to download any applicable addenda for a solicitation via the Gwinnett County web site. 8. After the pre-proposal conference, all prospective service providers will be given the opportunity for independent site visits at each facility. Prospective proposers may visit the sites on July 8 and July 9 from 8:00A.M. 5:00P.M. Should

22 RP Page 22 questions arise during site visits being done independently, please do not ask individual Facility Coordinators, Branch Managers or other personnel as they may not answer the questions accurately. No organization or proposer is to discuss any aspects of this Request for Proposal with any Gwinnett County employee without approval of the Purchasing Division s representative. This is to insure that all prospective respondents have the same level of knowledge relative to this proposal as well as insuring the additional data is made available to all proposers. Gwinnett County Purchasing Division is not responsible for any information, whether verbal or written, regarding this Request for Proposal from any source other than the Gwinnett County Purchasing Division. Any additions/changes/clarifications to this Request for Proposal are not to be considered valid unless in writing in the form of an addendum. B. Content of Proposal Submission The Service Provider s Proposal shall be responsive to the specific range of issues described in this Request for Proposal. Service Providers are asked to read the Request for Proposal carefully to insure that they address the specific requirements of this Request and submit all requested information. Proposing Service Providers will be evaluated and scored based on the information provided in their proposals as it relates to this Request. Any additional information provided by the proposing firms will not be considered and is discouraged. The Service Provider s Proposal shall be organized in the order and format described below. Each proposal shall include the following information. 1. Background and Experience of Company Provide a narrative description of the company s history, purpose, range of services, resources, financial stability, and past and current business activities. In the latter area, describe in detail the company s experience in the custodial services field. Indicate the numbers, types and sizes of facilities previously and presently serviced; and indicate the scopes of services provided by the company and the annual contract values. Also describe any special capabilities of the company, such as resources, programs or practices that set it apart from other firms. 2. References Provide five (5) references for custodial services contracts presently underway or completed within the past three (3) years. References must be for service on individual facilities of not less than 25,000 sq. ft. and whenever possible should show the company s ability to handle multiple properties at once, with cumulative areas up to or above 500,000 sq. ft. References must indicate the client, the facility or facilities included in the contract, the size(s) of the facilities, the full range of services provided, the contract time frame, and the most recent annual contract cost.
