PROJECT SUMMARY REQUEST FOR QUALIFICATIONS FOR HEALTH & WELFARE BENEFITS BROKER/CONSULTANT Notice is hereby given that the Board of Commissioners of the Port of New Orleans (Board) is requesting the submittal of qualifications from firms licensed to do business in Louisiana (Prime Consultants) who have demonstrated commensurate experience and expertise for providing Health & Welfare Benefits Broker/Consultant Services for three-year contract and two one (1) year options unilaterally exercisable by the PORT at its sole discretion, for a total maximum of three, four or five years for the Health & Welfare Benefits Broker/Consultant Services, subject to all rights reserved to PORT herein. It is the intent of the Port to create an agreement(s) with an insurance broker(s)/consultant(s) in providing the Port with comprehensive and professional insurance services, including the marketing of insurance coverages, as needed. The Port seeks a qualified insurance Broker/Consultant to assist in-house staff with the strategic plan, design, and negotiation of employee/retiree medical, prescription, dental and vision plans, disability plans, life insurance, and other related benefit plans/programs. Deliveries prior to the due date are only accepted Monday thru Friday, excluding holidays, from 8 a.m. to 4 p.m. Failure to submit required information by the deadline will cause disqualification and removal from further consideration on this project. No information may be changed or submitted after the deadline. Appendix A for this RFQ is shown in the RFP Table of Contents. Page 3 of 7

4 PORT CONTACT All inquiries shall be directed to the Procurement Department at by May 8, 2018 at 11:00AM (CST). As necessary, addenda to this RFP will be issued to all Prime Consultants. Prime Consultants are responsible for disseminating pertinent information to subconsultants, if any. Questions and answers and all forms required will be posted on the Board s website at SCHEDULE It is anticipated that a professional services agreement will be awarded by the Board within 45 calendar days after receipt of Request for Proposals. Notice of Award will be issued immediately following the Board s award. The Prime Consultant shall have 14 calendar days after Notice of Award to execute an agreement for professional services with the Board and submit with the signed agreement all insurance certificates. All required certificates of insurance for Prime Consultant and all sub-consultants are due to the Board with the executed agreement. Part II. GENERAL INFORMATION GENERAL INSURANCE REQUIREMENTS Consultant shall provide and maintain at its own risk, cost and expense the insurance summarized herein. Insurance Companies All i n s urance shall be written with insurance companies authorized and licensed to do business in the State of Louisiana and acceptable to Board (Best's rating A-, VI, or better) or mutual insurance pools or associations approved in advance by Board. Self-insurance programs authorized by the Commissioner of Insurance of the State of Louisiana for workers' compensation insurance are acceptable if Consultant provides Board with a certified copy of Consultant's authority to self-insure. Primary Insurance All insurance required in this section shall be primary to any similar insurance that may be carried by Board for its own protection. Furthermore, the insurance certificates of Consultant and all subconsultants or subcontractors working under this Agreement shall state on the insurance certificate the following wording exactly as written herein: Insurance herein is p r i m a r y to any other i n s u r a n c e maintained by Board. Insurance Certificates Before this Agreement becomes effective, Consultant shall furnish to Board's risk manager signed certificates evidencing that it has procured the insurance herein required. Thereafter, Consultant shall furnish such certificates prior to expiration throughout the term of this Agreement. Named Insured Except for workers' compensation insurance and professional liability insurance, Board shall be named as an additional insured on all policies required herein. Furthermore, the insurance certificates of Consultant and all subconsultants or subcontractors working under this Agreement shall state on the insurance certificate the following wording exactly as written herein: Page 4 of 7

5 Port of New Orleans is an additional insured under the general liability and automobile liability c o v e r a g e s as required by written contract. Waiver of Subrogation All insurance policies required herein shall provide that the insurers waive any rights of subrogation they may have against Board. Furthermore, Consultant and all subconsultants and any subcontractors working under this Agreement shall state on the insurance certificate the following wording exactly as written herein: A waiver of subrogation in favor of the Port of New Orleans is granted for general liability, auto liability and workers' compensation coverages as required by written contract. Notice of Cancellation: All policies required herein shall provide for thirty (30) days' written notice of cancellation or material change to be sent to Board at P.O. Box 60046, New Orleans, LA 70160, Attention: Risk Manager. No Representation or Warranty: Board makes no representation or warranty that the insurance set forth in this section will be sufficient to protect Consultant s interests. Deductibles: Consultant shall be liable to Board for any deductible amounts it maintains under the policies required in this section. Combination of Coverages: A combination of primary and excess insurance may be used to satisfy the requirements of this section. Commercial General Liability Insurance During the term of this Agreement, Consultant shall procure and maintain at Consultant's sole cost and expense comprehensive general liability insurance (on an occurrence basis) with limit of liability of not less than one million dollars ($1,000,000) for all injuries or deaths resulting to any one person or from any one occurrence. The limit of liability for property damage shall be not less than one million dollars ($1,000,000) for each occurrence and aggregate. This insurance shall include the "broad form contractual endorsement". Workers' Compensation Insurance - Consultant s h a l l procure and maintain (either as an authorized self-insured or through an authorized insurance carrier) at Consultant's sole cost and expense workers' compensation insurance to protect Consultant from claims under the Louisiana Workers' Compensation Act as well as under the Federal Longshoremen's and Harbor Workers' Compensation Act and Jones Act, if applicable. The limit of liability under the employer's liability section of the workers' compensation insurance policy shall be not less than one million dollars ($1,000,000). Professional Liability/ Errors and Omissions Insurance - Consultant shall procure and maintain errors and omissions/professional liability insurance in the amount of one million dollars ($1,000,000) per claim and two million dollars ($2,000,000) annual aggregate. Consultant shall maintain this coverage in full force and effect for a period of three years after termination of this Agreement. Consultant shall notify Board in writing in the event of any cancellation of coverage. MINIMUM PERSONNEL REQUIREMENTS I. Required: The Consultant firm must be registered to do business in the State of Louisiana at the time of submittal of the RFP in accordance with Louisiana state law with respect to those services. 6 II. III. Required: The Consultant must also employ, on a full time basis, sufficient Louisiana registered professionals to perform any work that requires licensure in the State of Louisiana. Preferred: The Consultant must employ, on a full time basis, at least one appropriately licensed and/or certified professional with at least eight years of experience to serve as lead project manager SELECTION PROCESS AND CRITERIA The Minimum Qualifications Summary of each proposal will be reviewed and rated a pass or a fail. Qualifications not meeting the minimum requirements will be considered non-responsive and not evaluated. DBE/SBE PARTICIPATION The Board encourages the participation of DBE/SBE (Small Business Enterprise) firms on the consultant/team. In order to receive points for participation, firms included in the proposal that have any such status must submit with the bound Qualifications evidence of a current certification from at least one of the following entities that issues such certifications: 1) the SLDBE program run by the City of New Orleans, 2) the State of Louisiana s Department of Economic Development s SBE Hudson Initiative program or 3) the Federal Small Business Enterprise Program. The Board does not have a certification program, but accepts certifications issued only by these public entities. Failure to include such information with Qualifications will be taken to mean that firms have no such current certification. No later submissions of this information will be allowed after submittal of the Qualifications. ETHICS DISCLOSURE The Board calls the attention of all potential bidders and proposers to the Louisiana Ethics Code, La. R.S. 44: _et seq. Those laws prohibit the Board from doing business with any of its current commissioners or those who have served on the Board within the past two years or, from doing business with certain companies with which the following persons are connected COMMISIONERS: Arnold B. 7 PART III. REQUIREMENTS TO RESPOND TO THIS RFQ SUBMITTAL REQUIREMENTS TO RESPOND TO THIS RFQ There are two components required to respond to this RFQ: 1. Written Proposal: one original bound proposal and four bound copies. 2. Flash Drive: A complete electronic set of all documents submitted Any Consultant/Team failing to submit any of the required RFQ information may be considered non-responsive. Facsimiles and submittals of the Written Proposal will not be excepted. EVALUATION CRITERIA OF THE RFQ Listed below, in order of their relative importance, are the criteria that the Consultant Evaluation Committee will use to evaluate the RFQs considering all firms included on the Consultant/Team as stated: I. Experience, both of the Consultant/Team and key individuals in type of work II. Past performance of Consultant/Team and key individuals on similar work III. Conceptual plan of action includes how the Consultant/Team will approach the assigned project(s). IV. Staffing and support experience and expertise, including experience in projects involving comparable issues, tasks, coordination. V. Knowledge of local conditions. VI. Ability to meet the Board s schedule for expeditiously completing assignments. VII. Local representation, including the location of key personnel VIII. SBE/DBE total participation The Consultant Evaluation Committee will be responsible for performing the above described evaluation and selecting consultant teams to respond to a Request for Qualifications. Teams meeting the criteria may be short-listed and invited to respond to a request for proposal. Page 7 of 7
