1 TERMS AND CONDITIONS More4U are value-added services that You have access to in addition to Your insurance policy benefits. It does not represent a contract between You and Old Mutual. More4U is brought to You by independent service providers and may be withdrawn, changed or cancelled at any time. You can access More4U if You have an Old Mutual Group Schemes Policy or are a Principal Member or Dependant of a Principal Member of a Burial Society or Group Funeral Solutions Scheme underwritten by Old Mutual. Funeral Support is only available to Life and Funeral Plan policyholders. Growplan or Reversionary Bonus policies do not qualify for More4U. Old Mutual Swaziland ( and Old Mutual Namibia ( policyholders must contact their local branch for information on the value added services that apply to them. You can access these value adds by dialling the More4U helpline on Definitions: You means the Policyholder or the Principal Member of a group policy. Terms and Conditions Page 1

2 EMERGENCY MEDICAL SUPPORT You, your spouse/partner, dependent children and lives covered under the policy can access Emergency Medical Support. A medical emergency means any injury or illness that threatens human life and needs immediate medical attention in order to try and save a life. You will have access to the More4U Emergency Medical Support helpline 24 hours a day, 365 days a year. A More4U operator will be required to verify your personal information. Only the initial emergency medical response will be at no cost to you, any additional medical assistance will for your own account. Emergency Medical Support is subject to the terms and conditions of its independent service provider. EMERGENCY MEDICAL EVACUATION Emergency Medical Support offers you urgent assistance should you find yourself in a medical emergency. When you contact the More4U helpline, the operator will find out the facts of your medical emergency and offer urgent assistance. A qualified medical operator will offer the following: Emergency pre-arrival instructions A professional medical operator will give information or instructions to the person calling to enable them to help the affected person until a medical team arrives at the scene. Emergency Medical Team Depending on the circumstances, an emergency medical team will be sent to the scene by road or by air, whichever is most appropriate. Advanced Life Saving Resuscitation Once the medical team arrives on the scene, the emergency medical team will perform the necessary advanced lifesaving resuscitation and will stabilise the affected person. Transportation of affected person If it is necessary the affected person will be transported to the closest medical facility that can offer medical assistance. Terms and Conditions Page 2

3 TRANSPORTATION TO A MEDICAL FACILITY If the emergency is life threatening, or if the emergency condition requires it, the affected person will be transported by road or air, whichever is most appropriate, to the closest hospital or medical facility that can help with that particular emergency. If the affected person needs specialised care, and the medical facility they were transported to cannot help, then the affected person will be transported by road or air to a hospital or medical facility that can provide the specialised medical assistance. 4 TRAUMA ASSAULT AND HIV SUPPORT You, your spouse/partner, dependent children and lives covered under the policy can access Trauma Assault and HIV Support. A Traumatic Event means exposure to or experience of the following: Disasters (Natural & Unnatural) Death Vehicle accidents Assaults: physical, sexual or psychological, or Being a witness to an event that caused someone harm. You will have access to the More4U Trauma Assault And HIV Support helpline 24 hours a day, 365 days a year. A More4U operator will be required verify your personal information. Trauma, Assault and HIV support is subject to the terms and conditions of its independent service provider. TRAUMA & ASSAULT ASSISTANCE Trauma, assault and HIV support offers access to medical and psychosocial support by trained medical professionals in the event that you experience or are involved in a traumatic event. The More4U operator will assess the facts of your traumatic experience and offer assistance. You could be referred for face-to-face counselling to a medical professional on the More4U network. HIV INFORMATION AND REFERRALS Trauma, assault and HIV support provides support and counselling for You, your spouse/partner, dependent children and lives covered under the Policy, if you are infected or affected by HIV/AIDS. Trauma, assault and HIV support provides You with information, education, support and lifestyle coaching by trained medical and psychosocial professionals. More4U will refer You to appropriate facilities and medical professionals. When You contact More4U, You will be assisted by a trained medical professional who will conduct an assessment to determine the best response to Your situation. Terms and Conditions Page 4

5 SHORT-TERM ANTIRETROVIRAL TREATMENT TO REDUCE THE LIKELIHOOD OF HIV INFECTION If You are exposed to HIV through an accident or rape, Trauma, assault and HIV support provides access to short-term anti-retroviral treatment to reduce the likelihood of HIV infection after potential exposure. The treatment includes the following: Access to telephonic trauma and HIV/AIDS counselling A referral to a medical professional. Three blood tests per incident: the first one immediately after the event, the second at 6 weeks after the event, and the third blood test, 3 months after the event. If the result after the first blood test is positive, no further tests will be done. If You are HIV negative, You are provided with: A 30-day starter pack of anti-retroviral medication A 7-day course of Sexually Transmitted Infections (STI) medication, in the case of rape A morning-after pill to prevent pregnancy, in the case of rape 24-hour access to the HIV counselling centre for information, advice and support. If You are HIV positive, You are provided with: 3 consultations with a medical professional per incident A 7-day course of STI medication A morning-after pill to prevent pregnancy in the case of rape. When You contact More4U, a registered medical professional will assess your situation to determine the most appropriate response. Terms and Conditions Page 5

6 HEALTH AND WELLBEING SUPPORT You and the lives covered under the Policy can access the Health and wellbeing support. Health and wellbeing support is available 24 hours a day, 365 days a year. The More4U operator will be required to verify your personal information. Health and wellbeing support is subject to the terms and conditions of its independent service provider. ANNUAL HEALTH SCREENINGS Health and wellbeing support provides You with access to online information & resources. Health and wellbeing provides health screenings at Your closest medical centre at discounted rates. The screenings will include: Flu inoculation HIV/AIDS counselling and testing Health Risk Assessments(such as blood pressure and BMI screenings) TB Screenings. Terms and Conditions Page 6

7 LEGAL WELLNESS You, your spouse, dependent children and lives covered on the Policy have access to Legal Wellness. Legal Wellness is available 24 hours a day through More4U. Only the first 30 minutes of a face-to-face consultation will be at no additional cost to You. If your consultation is longer than 30 minutes, the additional time will be for your own account. The attorney will inform You when your consultation has gone overtime. The More4U operator will verify your personal information. Legal Wellness is subject to the terms and conditions of its independent service provider. LEGAL ASSISTANCE Legal Wellness provides legal assistance in general South African law such as civil, criminal, labour, conveyancing, tax, financial advice and family law. Legal Wellness is provided by qualified attorneys. More4U will direct your question to a qualified attorney. The attorney will: Provide legal advice Send You a standard legal document Where necessary, refer You to an attorney on the More4U network, closest to You, for a 30 minute face-to-face consultation at no additional cost to you. LEGAL DOCUMENTS Through the legal document offering, You will have access to: A Small Claims Court kit (including guidelines as to whether a matter falls within the jurisdiction of the Small Claims Court) A Domestic Workers Agreement A Maintenance Kit. FACE TO FACE LEGAL CONSULTATION During the 30 minute consultation, the attorney may draft a letter of demand or make one telephone call on Your behalf. If your consultation is longer than 30 minutes, the additional time will be for your own account. The attorney must inform you when your consultation has gone overtime. Terms and Conditions Page 7

8 FINANCIAL WELLNESS You and your spouse/partner have access to Financial Wellness. Financial Wellness is available, through More4U, Monday to Friday from 8am to 5pm. Financial Wellness is not available on Saturdays, Sundays and public holidays. You will have to accept the terms and conditions before Financial Wellness can generate and request a credit report from the Credit Bureaus. Legal fees associated with the management of Garnishee orders will be for Your own account, which means You will have to pay out of Your own pocket. A More4U operator will have to verify your personal information. Your Credit reports will be ed or SMSed to you on a quarterly basis. Financial Wellness is subject to the terms and conditions of its independent service providers and is subject to change, cancellation or withdrawal. FINANCIAL OR CREDIT STATUS MANAGEMENT Financial Wellness offers you access to: Your credit score from each credit bureau Your credit score compared with other South Africans Information on how to improve Your credit score Financial Wellness can provide You with a holistic view of Your current financial habits in order to help You plan and manage Your finances better. ADVICE ON FINANCIAL WELLNESS (INCLUDING CREDIT SCORES AND THE MANAGEMENT OF GARNISHEE ORDERS) Financial Wellness offers you access to a credit coach who can help You: Manage your debt and credit Complete a debt assessment, activity assessment, budget, applying for new credit, and help with the removal of a blacklisting Explain and answer questions pertaining to Your credit report Advise on how to use of tools and services that they offer Analyse debt and restructure existing agreements if required Refer you to affordable attorneys Refer You for debt counselling or consolidation Refer You to Old Mutual s On the Money financial education workshops. 9 Financial Wellness is offered in the following languages: English Zulu Xhosa Tswana Sotho Sepedi Shangaan Venda Afrikaans. MANAGEMENT OF GARNISHEE ORDERS Financial Wellness provides Garnishee Management which will validate the correctness of the Garnishee Order and where possible, will renegotiate the amounts paid to the credit provider. Financial Wellness will contact the relevant HR department, the credit provider, or attorney involved, in order to evaluate all Garnishee orders and assess the best way forward. Once a signed power of attorney is received with copies of all Garnishee Orders under Your name, the credit coach will: Complete a financial assessment with the You and establish details of how the order was served and whether it is lawful. Renegotiate payment terms and finally issue a court order to amend, set aside or rescind the Garnishee Order. Legal fees associated with resolving a Garnishee Order matter, will be for Your own account. Terms and Conditions Page 9

10 FUNERAL SUPPORT You and the lives covered under the Funeral and/or Life Plans can access Funeral Support. Funeral support is available through More4U 24 hours a day. A More4U operator will verify your personal information. You will need to provide: Your name and contact details The deceased s name and identity number The Old Mutual policy number The address where the death occurred. The body will only be transported if the distance between the place of death and the place of burial is more than 50 km. If You use your own money to make funeral arrangements that are available under Funeral Support, you will not be compensated, and More4U will not be liable for these costs. Funeral Support is subject to the terms and conditions of its independent service provider. FUNERAL SUPPORT Funeral Support helps: Transport the body (body repatriation) Interpret policies Obtain a death certificate Refer you to a psychologist or psychiatrist where needed Refer you for counselling where your child has passed away. 11 FUNERAL RELATED DISCOUNT VOUCHERS Discount vouchers for public and private transport; groceries, airtime and funeral related products. Availability: Limited to claim stage Description: Access to discount vouchers for transport, groceries, airtime and funeral related products. You/Your: A Policyholder who qualifies to access discounts under More4U Operating hours: Monday to Friday from 7am to 7pm Saturdays from 8am to 12pm (Excluding public holidays) More4U facilitates access to a range of discount vouchers for: Buses, taxis and flights Groceries and airtime Funeral-related products (for example coffins, tombstones and other funeral related products). The discounts are only available, through More4U, at claim stage. The More4U operator will verify your personal information. Shopping vouchers can only be used at the point of sale. The vouchers cannot be exchanged for cash. Funeral products, on which a discount voucher will be used, must be paid for in full before the funeral. The discount voucher can only be used once. An original voucher must be presented at point of sale for some discount vouchers. Only one voucher may be used per claim. The offer cannot be used together with any other special offer or promotion. The More4U operator will explain to you how to use the discount voucher and will send you an or SMS (within 24 business hours) which either delivers the voucher or the voucher code. In cases where You are required to produce proof of payment before receiving a discount voucher, You will need to send through the proof of payment via fax or , and the discount voucher will be effective within 24 business hours from when they receive the proof of payment. 12 You will be required to identify yourself at point of sale. The discounts are subject to availability and to the terms and conditions of the independent service providers. HOW TO ACCESS THE DISCOUNT VOUCHERS Phone the More4U contact number. Select the appropriate option. A More4U operator will verify your personal information. Request the discount voucher either directly or after requesting information from the More4U operator.
