Prices should be based on awarded proposal (you may combine a sport season but may not book with one vendor for all sports). You will need to complete the reservation form and route for signatures (attached). Please note: Professional Services contracts are no longer required for Charter Bus Services. For budgeted funds: Once you determine which company you will be using: 1) Enter OH Requisition, using item master number ) Write the OH # on the completed reservation form and forward to Purchasing Department (faxed or scanned copies will be accepted). Once the requisition is approved by the Purchasing Department, a copy of the purchase order will be sent to your attention. 3) Book trip purchase order must be approved prior to using any services.

3 For internal funds: Once you determine which company you will be using: 1) Complete internal order and attach the reservation form. Forms must be approved prior to using any services. Do not send reservation forms to the Purchasing Department for Internal Account Orders. 2) Book trip Please note: The District bid replaces Internal Account contracts for charter buses which are no longer required. Arrival back at site: Complete the trip evaluation form and forward to bookkeeper (attached). Notes: There is no need to request a copy of the insurance certificate from the vendor. The Purchasing Department maintains these records. Remember to make arrangement with vendor(s) that have effective certificates of insurance on the date of your trip. If insurance expires before your scheduled trip, you must select a new vendor.
