1 ACKNOWLEDGEMENT OF RISK ACCEPTANCE OF RESPONSIBILITY RELEASE OF LIABILITY This document affects your legal rights. You must read and understand it before initiating and signing. DUTY OF PARTICIPANTS: It is recognized that some recreational activities conducted by Fritzlan & Family Guide & Outfitters are hazardous to participants regardless of all feasible safety measures which we can take. All participants shall have a duty to act as a reasonably prudent person when engaging in the recreational activities which are offered by Fritzlan & Family Guide & Outfitters, referred to hereafter as Fritzlan & Family Guide & Outfitters, I hereby covenant and agree not to: a) On any act which shall interfere with the running or operation of Fritzlan & Family Guide & Outfitters when such activities conform to the rules and regulations of the State of Colorado. b) Use any of Fritzlan & Family Guide & Outfitters equipment or facilities or services if I do not have the ability to use such facilities, equipment or services safely without instructions until I have requested and received sufficient instruction to permit safe usage. c) Engage in any harmful conduct or willfully, or negligently engage in any type of conduct which contributes to or causes injury to any person. d) To embark in any self-initiated activity without first informing Fritzlan & Family Guide & Outfitters of my intentions and receiving permission from Fritzlan & Family Guide & Outfitters to engage to such self-initiated activity. ACKNOWLEDGEMENT AND ACCEPTANCE OF RISK: I understand and acknowledge that the activity which I am about to voluntarily engage in as a participant and/or volunteer bears certain known risks and unanticipated risks which could result in injury, death, illness or disease, physical or mental, or damage to myself, to my property or to spectators or other third-parties. I, being aware that this activity entails risks or injuries to myself and a risk or injury to spectators or third-parties as a result of my actions, expressly agree, covenant and promise to accept and assume all responsibility and risk for injury, death, illness, or disease, or damage to myself or to my property arising from participation in this activity. I also agree to pay for any damages caused to others (including attorneys fees and costs) if they are injured or otherwise damaged due to any negligent actions. My participation in this activity is purely voluntary; no one is forcing me to participate, and I elect to participate in spite of the known and unknown risks. RELEASE: In consideration of the services and/or property provided, I, for myself and any minor children for which I am the parent, legal guardian, or otherwise responsible, any heirs, personal representatives, or assigns, do hereby release Fritzlan & Family Guide & Outfitters its principals, directors, officers, agents, employees, and volunteers from any liability and waive any claim for damages arising from any cause whatsoever (except that which is gross negligence). I further agree to reimburse you for all attorney s fees and costs should I bring a legal action against you and lose. ENTIRE AGREEMENT: I UNDERSTAND THAT THIS IS THE ENTIRE agreement between myself and Fritzlan & Family Guide & Outfitters it agents or employees, and that it cannot be modified or changed in any way by the representations or statements of any employee of Fritzlan & Family Guide & Outfitters or by me. 2 DIRECTIONS TO BASE CAMP FROM MEEKER, COLORADO At the North edge of Meeker, turn right on County Road #8 (There is a sign that says Buford & Trapper s Lake) Go 18.3 miles east on COUNTY ROAD #8 There is a sign that says to 7 Lakes Lodge, turn right and cross the river Go 1.5 miles to stop sign. At the stop sign, turn right on the gravel road. Go 7 miles on the gravel road. Camp is located on the left side of the road. ***Note*** Meeker is 39 miles north of Rifle on Highway 13, which is on I-70 approximately 175 miles west of Denver.

Before arriving at the Main Camp, please be aware of the following: You are responsible to weigh and label your gear. You are allowed to bring 150 lbs. total in three army bags.

4 Before arriving at the Main Camp, please be aware of the following: You are responsible to weigh and label your gear. You are allowed to bring 150 lbs. total in three army bags. This is approximately 2 heavy bags matching in weight, with duct tape labels. One lighter bag to include your sleeping bag and boots or some clothing also labeled. This provides one mule per hunter and allows us to safely pack your gear in without harming our mules or your gear. Your gun is not included in the 150 lbs., your gun will accompany you on your riding horse in a scabbard. We have Scabbards available, but it is advisable to bring your own scabbard to fit your gun. Our scabbard might not fit your gun like you would like. Due to the fact that some camps are a couple hours ride in. You are responsible and liable to know all the Colorado Division of Wildlife hunting rules and regulations. This includes point restrictions and sex of the elk or deer. You are responsible to also make sure that proper identification is left on the animal. We will not assume any liability for mistakes made by you or your hunting party. It is our obligation if a mistake is made to notify the Division of Wildlife. For game taken we have a cooler available to our hunters at the cost of $7.00 per day for deer and $10.00 per day for elk. We will take animals to Meeker for processing for a cost of $30.00 per animal. Fritzlan & Family is not responsible for processing errors. Propane is available for the cost of filling the bottle. Water will be packed in for drinking and cooking purposes and camps will be checked every other day. Sometimes everyday but not guaranteed. Fritzlan & Family is not responsible for spoiled or bear eaten meat. It is advised that after you kill your animal and the weather is warm that you get the skin off of the meat and hang the meat up off the ground in game bags to cool. If you urinate around the meat on the ground and leave an article of clothing it seems to help ward off the bears! When you hunt with us we do our very best to get your meat out and taken care of as soon as possible! If you go in two days before the season we will guarantee to have you out at the latest, the day after the season. If you go in the day before the season, we will guarantee to have you out two days after the season at the latest! 5 Acknowledgement of Risks, Acceptance of Responsibility, and Release of Liability PLEASE READ CAREFULLY: Warning: Under Colorado law, an equine professional is not liable for an injury to or the death of a participant in equine activities, resulting from the inherent risks of equine activities, pursuant to section , Colorado Revised Statutes. Warning: There are significant elements of risk in any adventure, sport, or activity associated with saddle and draft animals including but not limited to horses, mules, and burros (referred to herein as activity ), and the use of any equipment herewith. Acknowledgement of Risks: I realize that horses can act unpredictably; that wind, weather conditions, movement, or noise may frighten or cause the animal to move suddenly and/or swiftly; that certain risks are normally involved in riding including but not limited to collisions, falls, and medical injuries and that wearing a helmet is a basic precaution; and that no warranty of any kind, express or implied, is being made as to the habits, disposition, suitability, nature or physical condition of any animal. I also realize that participation in an activity can result in personal injury, accidents and/or illnesses including but not limited to sprains, broken boned, head injuries, paralysis, dismemberment, or death as well as damage to or loss of personal property. Express Assumption of Risks and Responsibilities: In recognition of the inherent risks of the activity which I and any minor children for which I am responsible, will engage in, I confirm that I am (we are) physically and mentally capable of participating in the activity and using the animals and equipment.
