2 Introduction/Scope of Work The Rhode Island Airport Corporation (RIAC) is seeking proposals from experienced Firms to provide catering services for special events and other meetings held at RIAC. The catering services primarily include the following: Ability to set-up for events/meetings (access to tables/linen, etc.) Ability to provide on-site support staff for events/meetings (when required) Ability to provide a full suite of both food and beverages as needed for events/meetings Ability to clean-up and remove all items in a timely fashion at the conclusion of the events/meetings. This includes providing disposable aluminum (or equivalent) containers for any leftover food to be delivered to a site within RIAC s facility. The number of attendees at any one event could vary significantly (from 10 to as much as 100+) and as such, the Firm must have the ability to be flexible in terms of meeting the different requirements specified. While RIAC typically has these events planned in advance, the Firm must also be flexible in terms of meeting tight deadlines. As the type of event may vary, RIAC reserves the right to select multiple Firms to provide these services. In the case of multiple Firms, RIAC may solicit proposals from multiple Firms for any event/meeting to ensure the highest quality in the most cost efficient manner. Request for Qualifications Requirements Proposing Firm shall submit four (4) printed copies of the proposal to: Office of Procurement Rhode Island Airport Corporation T. F. Green Airport 3 rd Floor 2000 Post Road Warwick, RI Attn: RFQ No Catering Services The proposal must be submitted no later than 2:00 p.m. May 24, 2018, Eastern time. Late submissions will not be accepted. In order to control the dissemination of information regarding this Request for Qualifications (RFQ), organizations interested in submitting proposals shall not make personal contact with any member of RIAC staff or Board of Directors regarding this RFQ. A conference call will be held on May 11, 2018 (1PM) to answer any questions related to the RFQ. To participate in this conference call, please procurement@pvdairport.com to request the call in number no later than May 10, 2018 (11AM). Questions concerning this RFQ should be directed, via , to procurement@pvdairport.com no later than 2:00PM EDT May 11, RIAC will respond to all relevant questions no later than end of day May 14, 2018 via addendum. 3 ( and the State of Rhode Island s Division of Purchasing website ( By submitting a proposal, the proposer certifies that it has fully read and understands the RFQ, has full knowledge of the scope of work to be provided, and accepts the terms and conditions under which the services are to be performed. RIAC reserves the right to reject any and all submittals, to request and consider additional information from submitters, and to reject any and all submittals on any basis without disclosing the reason. The term of the agreement between RIAC and the selected Firm(s) is three (3) years. Based on the Firms selected, RIAC will issue a blanket Purchase Order for each year. RIAC will utilize the Firms on an as needed basis and at RIAC s sole discretion. Submittal & Evaluation Criteria Please ensure the following areas are addressed in your submission and in the order shown. The information you provide should relate to the office within your Firm which will have primary responsibility for the servicing of RIAC's account. Please be sure to organize your response consistent with the format as described below. 1. Organizational Description Identify your principal address. Identify the primary office that will service this account. Identify the Account Executive responsible for your Firm s activities. Identify the total number of professionals who may service RIAC. 2. Experience of Firm Summarize relevant experience and involvement of your Firm and the team members with providing these services. Please include three (3) references. 3. Sample Menus As part of your submission, please provide any sample menus available to RIAC. Additionally, as part of your submission, describe RIAC s ability to customize the menu selection upon request. ADDITIONAL REQUIREMENTS Campaign Finance Compliance Every person or business entity providing goods or services at a cost of $5,000 cumulated value is required to file an affidavit regarding political campaign contributions with the RI State Board of Elections even if no reportable contributions have been made (RI General Law 17-27). Forms may be obtained at Board of Elections, Campaign Finance Division, 50 Branch Avenue, Providence, RI 02904, ( ). Major State Decision-Maker Does any Rhode Island Major State Decision-Maker, as defined below, or the spouse or dependent child of such person, hold (i) a ten percent or greater equity interest, or (ii) a $5,000 or greater cash interest in this business? For purposes of this question, Major State Decision-Maker" means: 3

4 (i) (ii) (iii) All general officers; and all executive or administrative head or heads of any state executive agency enumerated in R.I.G.L as well as the executive or administrative head or heads of state quasi-public corporations, whether appointed or serving as an employee. The phrase "executive or administrative head or heads" shall include anyone serving in the positions of president, senior vice president, general counsel, director, executive director, deputy director, assistant director, executive counsel or chief of staff; All members of the general assembly and the executive or administrative head or heads of a state legislative agency, whether appointed or serving as an employee. The phrase "executive or administrative head or heads" shall include anyone serving in the positions of director, executive director, deputy director, assistant director, executive counsel or chief of staff; and All members of the state judiciary and all state magistrates and the executive or administrative head or heads of a state judicial agency, whether appointed or serving as an employee. The phrase "executive or administrative head or heads" shall include anyone serving in the positions of director, executive director, deputy director, assistant director, executive counsel, chief of staff or state court administrator. If your answer is Yes, please identify the Major State Decision-Maker, specify the nature of their ownership interest, and provide a copy of the annual financial disclosure required to be filed with the Rhode Island Ethics Commission pursuant to R.I.G.L , 17 and 18. In the course of providing goods or services to RIAC, the selected respondent may receive certain personal information specific to RIAC employees including, without limitation, names and addresses, telephone numbers, addresses, dates of birth, account numbers, social security numbers, driver s license or identification card numbers, employment and income information, photographic likenesses, or other personal or financial information (hereinafter collectively referred to as the Personal Information ). The maintenance of the Personal Information in strict confidence and the confinement of its use to RIAC are of vital importance to RIAC. Please include a letter from your president, chairman or CEO certifying that, in the event your Firm is selected: any Personal Information disclosed to your Firm by RIAC or which your Firm acquires as a result of it services hereunder will be regarded by your Firm as confidential, and shall not be copied or disclosed to any third party, unless RIAC has given its prior written consent thereto; and your Firm agrees to take all reasonable measures to (a) ensure the security and confidentiality of the Personal Information, (b) protect against any anticipated threats or hazards to the security or integrity of the Personal Information, and (c) maintain reasonable security procedures and practices appropriate to your Firm s size, the nature of the Personal Information, and the purpose for which the Personal Information was collected in order to protect the Personal Information from unauthorized access, use, modification, destruction or disclosure; and when discarding the Personal Information, destroying it in a commercially reasonable manner such that no third party can view or 4

5 recreate the information, electronically or otherwise. These provisions, which implement the requirements of the Rhode Island Identity Theft Protection Act, R.I.G.L et seq., will also be incorporated into the final contract with the selected respondent(s). 5
