3 REQUEST FOR QUALIFICATIONS ARCHITECTURAL/ENGINEERING SERVICES INDEFINITE DELIVERY INDEFINITE QUANTITY CONTRACT The Jefferson Metropolitan Housing Authority (JMHA) is requesting proposals from qualified Architectural and/or Engineering firms for indefinite delivery indefinite quantity contracts. At present, the Jefferson Metropolitan Housing Authority (JMHA) operates 13 sites within Jefferson County, totaling 805 units of public and assisted low-income housing. Each contract will be for three years. No specific contract amount will be set. The amount of each task order will dictate the contract amount for that task. The firm or firms chosen will be assigned tasks relating to A/E services, which may include, but are not limited to, the following: Architectural design, including associated Mechanical, Electrical, Plumbing design. Landscape design. Design work may be in support of demolition, renovations of existing sites or new construction, both interior and exterior. Building types could range from single family to hi-rise and community centers to small business development centers. Design work may incorporate site design, site layout, site revitalization, and urban planning services. Zoning and subdivision services. Services associated with environmentally friendly development techniques and certifications, such as Leadership in Energy and Environmental Design (LEED), EPA Energy Star, Earthcraft, Low-Impact Development (LID), Green Physical Needs Analysis (GPNA) services, and the like. Building condition evaluation services, including Physical Needs Assessments performed to standards promulgated by controlling authorities. Standalone mechanical, electrical, or plumbing engineering services, for existing or new structures or site infrastructure. Civil engineering services. Structural engineering and building evaluation services. Surveying services. Services related to stormwater system design, regulation, and implementation. Environmental assessment, testing, and remediation oversight services, including those related to lead-based paint, asbestos, mold, hazardous substances, and petroleum. Cost estimating services in support of the above services. Project Management and/or Resident Project Representative services, alone or in conjunction with services shown above. 3

4 It is the intention of the JMHA to have at its disposal approved service providers to expedite development and renovation projects which the Authority may wish to initiate. Typically, JMHA will seek the Board s approval for three (3) years for the selected service providers. However, being approved under the proposed contract will not ensure the assignment of any project. Assignments will be based on individual tasks matched to the most suitable service provider under contract. Your performance on the assigned task will dictate whether or not you receive another Task Order. Copies of the solicitation documents will be available for interested firms beginning at 9:00 AM, Monday, September 15, Qualification Proposals, in duplicate, will be received at the Jefferson Metropolitan Housing Authority Administrative Office, 627 North 4 th Street, Steubenville, OH 43952, until 4:00 PM, Thursday, October 16, Additional information may be obtained by contacting Carolyn Swearengen at JMHA s Administrative Office at (740) ext 101. EVALUATION FACTORS NOTE: Please provide specific and direct response to each of the following tabs A E. A. Maximum Points 30 The respondent s demonstrated understanding of the unique and specific requirements of multi-family housing, hi-rise renovation, public housing and affordable housing, including evaluation of such housing and experience with Green Physical Needs Assessments. Demonstrated understanding of urban Design, neighborhood revitalization, and relevant requirements of local, state, and federal authorities (including HUD). B. Maximum Points 20 Demonstrated professional and technical competence and experience as evidenced in terms of cost control, quality of work, and compliance with performance schedules. Please provide the following information for several references that may be contacted regarding A/E s or firms prior work: (Supplying the following information will constitute permission for the Jefferson Metropolitan Housing Authority to make inquiries regarding your company.) 1. Complete name, address, and telephone number of company which contracted the work. 2. Complete name, title, and telephone number of contact person at that company. This person should have knowledge of the work and should be able to answer questions regarding job performance. 4

5 3. Complete description and location of project to identify the particular job done. 4. Dates the work was performed. C. Maximum Points 20 Evidence of the A/E s or firms ability to perform the work, as indicated by profiles of the principals and staff s professional and technical competence and experience, and their facilities. D. Maximum Points 20 Evidence of the respondent s demonstrated experience and competence in concepts of environmentally-friendly development techniques and certifications, such as Leadership in Energy and Environmental Design (LEED), EPA Energy Star, Earthcraft, Low-Impact Development (LID), and similar certifications and programs. E. Maximum Points 10 Professional affiliation with Minority Business Enterprise (MBE) and Women s Business Enterprise (WBE) firms, contractors, and subcontractors. Please describe. Schedule for Submitting Proposals A. Submit proposal by (in duplicate) in an envelope plainly marked: Architectural/Engineering Services for Indefinite Delivery Indefinite Quantity Contract. Proposer s name must also be on the envelope. Note: SCHEDULE OF FEES OR DISCUSSION OF FEES SHALL NOT BE INCLUDED WITH PROPOSAL. PROPOSALS WILL BE SELECTED BASED ON QUALIFICATIONS. FEES WILL BE NEGOTIATED AFTER SELECTION. B. Send proposals by express delivery through USPS, UPS, FedEx, etc. The envelope should be clearly marked Proposal Due by 4:00 PM, Thursday, October 16,

6 Compliance Requirement Documents to be Completed by Firms Awarded a Contract/Task Order Firms that are notified of a contract award will be required to submit the following documents properly executed where applicable. Listed documents become part of the contract. 1. Instructions to Offerors, Non-Construction (HUD-5369-B) 2. Previous Participation Certificate (HUD-2530) 3. Certifications and Representations of Offerors, Non-Construction (HUD-5369-C) Including: o Minority and Women s Business Enterprise Opportunity 4. Corporate Certification (if incorporated) or Partnership Certificate 5. Statement of License Certificate 6. Certification Regarding Debarment and Suspension, or other prohibition from professional practice by any Federal, State, or local agency (HUD-2992) 7. General Conditions for Non-Construction Contracts (HUD-5370-C) Including: o Certification of Payments to Influence Federal Transactions o Equal Employment Opportunity o Section 3 Clause Certificate of Insurance A. Successful A/E or firm must provide, within ten (10) days from execution of first task order, Certification of Insurance for Errors and Omissions sufficient to cover the work performed under this contract; General Liability should be $1,000, and Automobile Liability with a minimum combined single limit of $500, per occurrence; and proof of Workers Compensation as dictated by State regulations, per HUD (1/2014). B. All insurance certificates must have a thirty (30) days written notice of cancellation, list the Jefferson Metropolitan Housing Authority as Additional Insured, list the site/work, and be original copies. 6

7 Standard Form of Agreement Attention is called to the Agreement Between Owner s Representatives and Design Professional, form HUD (1/2014). Respondent should read and understand this document before submitting a proposal. The fee for a Contract/Task Order shall be a firm fixed lump sum, as explained in Article B of form HUD mentioned above, and broken down as requested by Owner. Respondent should also read and understand A/E responsibilities as given in form HUD-5370-C, General Contract Conditions Non-Construction, which are incorporated as part of the contract. 7
