1 TRAVEL INSURANCE BROCHURE AIG offers Travel Guard insurances, which are available for everyone under 70 years of age living permanently in Poland. Any of these policies do not cover any trip in, to or trough following countries: Cuba, Iran, Syria, Sudan, North-Korea or Crimea region. Please note that this is only general information about travel insurance and you should read carefully the terms and conditions, so you have complete understanding of what your insurance includes. When choosing some of the AIG policies, you confirm that you have read the insurance terms and conditions and you agree the terms and conditions and AIG Privacy policy Travel Insurance includes for example: Medical expenses for travel time illness or accident with no upper limit; Emergency dental care during the trip up to 400 PLN; Emergency assistance services 24/7/365; Reimbursement of cost of ticket and travel expenses for a member of the Insured's family in the event of prolonged hospitalization 300 PLN daily (max PLN per person) Repatriation to Poland with no upper limit; Repatriation of remains to Poland with no upper limit; Accidental death in case of death as a result of an unfortunate accident, arising in the period of up to 12 months from the date of accident, the death benefit is paid up to PLN; Permanent disability, it is paid as a percentage of the sum insured indicated in the insurance document (25% - 100%), up to PLN; Trip cancellation or change of trip costs up to PLN; Flight delayed more than twelve hours maximum per delay 400 PLN; Loss, theft or damage to baggage up to PLN with limit for valuables PLN; Baggage delayed more than twelve hours maximum per person 800 PLN; Legal aid maximum per person PLN. Insurer of this Travel Guard insurance is AIG Europe Limited in United Kingdom, The AIG Building, 58 Fenchurch Street, London EC3M 4AB and reg. no Medical expenses the insurer is not responsible for: events occurred in connection with nervous disorders or mental illnesses; events occurred in connection with being infections or lesions which can be treated in out-patient clinic, pregnancy after the six month; events occurred in connection with recurrences of non-stabilized pre-existing illnesses with a risk of sudden and early aggravation; burial, embalming and funeral costs, unless these are mandatory under local laws; expenses incurred by the Insured without the AIG Alarm Centre provider's prior agreement; costs for meals, hotels, road travel, tolls, fuel, taxis or customs duties except those provided for under the events occurred in connection with actions that may be subject to criminal penalties under the laws of the country in which the Insured is located; medical expenses incurred in the Insured's country of residence; the consequences or recurrence of preexisting sicknesses; medical expenses incurred due to the diagnosis or treatment of a physiological condition (pregnancy) already known prior to the date on which coverage took effect; medical expenses following cases of dorsalgia, lumbar pain, lumbago-sciatica, herniated disc, parietal, intervertebral, crural, scrotal, or inguinal hernias, hernias through the linea alba and umbilical hernias; thermal cures, physical therapy, cost of eyeglasses, contact lenses, prostheses of any kind, routine examinations and tests or check-ups, preventative tests or treatment, examinations and tests in the absence of a covered accident or illness. Luggage the insurer is not responsible for: loss, theft or damage to documents, identity papers, credit cards, magnetic cards, tickets and vouchers for transportation, cash, stocks and securities, keys, skis, bicycles, wind-surfing boards, boats or any other means of transportation, professional equipment, musical instruments, items d'art, antiques, collections, merchandise, eye-glasses, contact lenses, artificial limbs and prosthetic devices of all kinds, clothing or accessories worn by the Insured, perishable goods or food;

2 smoking accidents, as well as damage caused to objects that fall or are thrown into a fire, or burns caused by excessive heat without fire; loss and damage due to normal wear and tear, deterioration or defect of an object; damage caused by moths or vermin, by a cleaning, repair or restoration process, by water or leaks; incorrect use of the object by the Insured or any other person; damage resulting from incorrect packing of the insured object;; when the objects are left unattended in a public place or in unlocked premises to which several people have access, or when the objects are in a convertible vehicle, a vehicle whose windows are not closed or a vehicle whose doors or trunk are not locked; losses due to confiscation, seizure or destruction by order of a government authority. Trip cancellation the insurer shall not be liable if the insured event was caused: consequences of the Insured's previous illnesses or accidents; nervous disorders or mental illnesses entailing hospitalization of 4 consecutive days or less; elective abortions, the consequences and complications thereof; cancellations of trips due to regular medical control and periodical examinations'; cancellations of trips due to the failure to obtain a vaccination as required by the provisions valid in country of destination or country through which the Insured travels; cancellations of trip caused by a failure to present any of the documents required for the trip for any reason whatsoever, except in the cases provided for in the general insurance terms and conditions; Cancellations of trip caused by the carrier or the organizer regardless of the reason therefore. PLEASE NOTE THAT THE INSURED CANNOT APPLY FOR INDEMNITY FROM SICKNESS OF WHICH THE INSURED WAS AWARE OF BEFORE BUYING THE INSURANCE Insurance period The policy is valid for the time period specified in the policy document. Claims and applying a claim Please hold all original receipts and documents, as they might be requested by AIG. In all medical emergencies the insured should contact: AIG Alarm Centre (24/7/365) Policy termination The policyholder who is a consumer within the meaning of the Civil Code has the right to withdraw from the insurance contract within 30 days of informing him of the conclusion of the contract. 3 RULES and REGULATIONS FOR CONCLUDING TRAVEL INSURANCE CONTRACTS ONLINE These Rules and Regulations for Concluding Travel Insurance Contracts Online (hereinafter referred to as the Rules and Regulations) apply to insurance contracts concluded online (on the Internet) between AIG Europe Limited (hereinafter referred to as the Insurer), operating through the medium of Finnair airline website, and individuals, legal persons or non-corporate entities (hereinafter referred to as the Policyholders), to the benefit of individuals (hereinafter referred to as the Insured Persons), for the duration of the Insured Persons trip. 1. INFORMATION CONCERNING THE INSURER 1.1 AIG Europe Limited in United Kingdom, The AIG Building, 58 Fenchurch Street, London EC3M 4AB and reg. no The Insurer provides insurance services on the territory of the Republic of Poland on the principle of freedom to provide services as a branch of the company AIG Europe Limited, based in London, registered with the Register of Companies for England and Wales under the number The Insurer carries on business on the territory of Poland in accordance with the scope of the authorization granted AIG Europe Limited by the Prudential Regulation Authority (previously the Financial Services Authority) and is subject to supervision by the Financial Conduct Authority and the Prudential Regulation Authority. 2. DEFINITIONS 2.1 insurance document a document issued by or on behalf of the Insurer, including an insurance policy/certificate/travel insurance card or any other document confirming the conclusion of the 2.2 insurance period the period indicated in the insurance document, during which insurance cover is provided; 2.3 GENERAL TERMS and CONDITIONS - THE GENERAL TERMS AND CONDITIONS OF AIG available on the website, on the basis of which the insurance contract is concluded; 2.4 website the website, belonging to Finnair, accessible at: through which the User can conclude the 2.5 insurance premium the fee charged for the insurance, calculated on the basis of the selected variant of insurance, number of days, the number of persons insured, geographical area and additional risks, including any discounts and price increases; 2.6 insurance contract is a travel insurance contract concluded on the basis the General Terms and Conditions and the User s application; 2.7 Policyholder a natural person (individual), legal entity or an organizational unit without legal personality, concluding the insurance contract and obliged to pay the insurance premium; 2.8 Insured Person an individual below 70 years of age, on whose behalf the insurance contract is concluded; 2.9 User the person using the website; 2.10 Application the form in the electronic format contained on the website, through which the User fills out the data necessary for the calculation of the insurance premium and the conclusion of the 2.11 Finnair the owner and operator of the website. GENERAL PROVISIONS 3.1 These Rules and Regulations define: a) the types and scope of services provided through the website; b) the conditions and principles for the conclusion of insurance contracts through the website; c) the technical conditions necessary for the use of the website and the conclusion of the insurance contract through it; d) the complaint procedure connected with the operation of the website. 3.2 The Rules and Regulations are made available free of charge on the website in such a way that the Users can download, reproduce and recorded them. 3.3 The Insurer provides the following services through the website (online): (a) providing the User with information about travel insurance offered; (b) enabling the User to calculate the insurance premium; (c) enabling the User to conclude the (d) making the claim-notification forms available to the User. 3.4 Beginning to us the website by the User means the conclusion of a contract for the provision of services by electronic means. The User may at any time leave the website and thereby terminate the contract for the provision of services by electronic means.

THE PROCEDURE FOR THE CONCLUSION OF THE INSURANCE CONTRACT 4.1 The insurance contract may be concluded via the website (online). 4.2 The website is available to Users 24 hours a day, 7 days a week. 4.3 To use the website and to conclude the insurance contract via the website, the User must have access to the Internet and a Web browser installed. To be able to conclude the insurance contract, the User must also have an address. 4.4 The following are the conditions for the conclusion of insurance contracts online: (a) completing the application correctly; (b) submitting the statements required by the Insurer. 4.5 In order to conclude the insurance contract, the User completes the data specified in the application, indicating: (a) the chosen product the type of the (b) the type of the insurance policy from the options available in the application; (c) the variant of insurance; (d) geographical area the destination; (e) the period of insurance; (f) personal details concerning the Insured Person. 4.6 When filling out the application, it is possible to take advantage of the on-screen tips on how a given field is to be completed. The hint is displayed automatically when the character "?" is clicked. 4.7 When filling out the application, the User is kept informed of any irregularities concerning the data entered and the failure to complete the mandatory fields. Prior to submitting the application, the User shall have the opportunity to correct the data entered in the application. 4.8 The User is required to enter valid data in the application, to the best of the User s knowledge. 4.9 Submitting unlawful/illegal content by the User is prohibited. Illegal content shall mean, in particular, the content contrary to the generally applicable provisions of law, offensive or infringing upon the rights of third parties The website does not allow the User to save and keep the data entered in the application. After the User has left the website without completing the procedure for the conclusion of the insurance contract, the data previously entered by the User are automatically deleted For security reasons, the session expires after the lapse of 30 minutes from the last activity of the User on the website and the data entered by the User in the application are automatically deleted Directly after filling out by the User all the fields of the application, the website displays a summary of the content of the completed application, including information on the amount of the insurance premium. The amount of the premium is binding until the expiry of the session on the website Before sending the application, the User must declare that he or she has read and accepts the General Terms and Conditions of Insurance, as well as these Rules and Regulations and the Global Privacy Policy of AIG Europe Limited. The User must also declare that the data which he or she provided are accurate The application completed by the User constitutes an offer within the meaning of the provisions of the Civil Code The payment of the insurance premium is only possible by credit card. In order to conclude the insurance contract, the User provides his or her credit card details for the purpose of effecting the payment of the insurance premium The insurance contract is concluded upon the receipt of the insurance document by the Policyholder, after the Policyholder has submitted the application to the Insurer via the website After the conclusion of the insurance contract, the User receives the insurance document and the General Terms and Conditions, which are sent to the address indicated in the application. 5. COMPLAINT PROCEDURE 5.1 The User has the right to make a complaint in matters related to the operation of the website. 5.2 Complaints should be submitted in writing by sending them either: (a) to the following address: Colonnade Insurance S.A., Marszałkowska Warszawa or (b) to the address reklamacje@colonnade.pl. 5.3 The submitted complaint should contain: (a) first name, last name and the complete address of the person submitting the complaint; (b) insurance document number (if the insurance contract was concluded); (c) the grounds for the complaint.

5 5.4 The complaint will be dealt with within 30 days from the date of receiving the notification by the Insurer.
