The Oak Park community is located eight miles west of downtown Chicago. Local residents tend to be very well educated and have high expectations of the Library and its services. The Library has a diverse collection of over 314,000 items in numerous formats such as books, magazines, CDs, DVDs, videos, audiobooks, ebooks, and databases. Over 886,000 visitors come annually to the Library and they check out over one million items a year. The library s Wide Area Network is mission critical to all library services. The library s Information Technology Department has an excellent track record of near zero service interruptions. We expect as close to zero downtime as possible. If outages are necessary, there is an expectation that projects are extremely well planned and any outages are expected to be scheduled in advance. These expectations shall be incorporated into the service contract and the successful bidder shall be expected to warranty that it can meet these expectations. 2 Project Overview The Oak Park Public Library seeks proposals from qualified professional Information Technology solution providers or consultants (hereafter referred to as consultants ) for Juniper switches and accessories, support, and installation services. Switching Refresh Our goals with the network refresh are to replace end of life switching equipment, test existing fiber optic cables for 10Gbps throughput, implement a 10Gbps backbone, provide 1Gbps to the desktop, implement PoE (Power Over Ethernet) on all ports (to support current and future VOIP phones, wireless APs, and other future PoE devices), and provide a basic level redundancy and ample ports for current needs and future expansion. The library has chosen to replace our existing Cisco switches with Juniper switches. Proposals for switching equipment other than the Juniper models, Dell uplink modules, and Addon fiber optic transceivers listed in the specifications must be pre approved by the Library for functional and reliability equivalency. The current network core is a Cisco Catalyst 4506 switch with 1Gbps multi mode fiber optic backbone. Most of the edge switches are 10/100Mbps Cisco Catalyst 3550s. PoE is currently provided with 3 rd party PoE injectors. Switches are physically located at the Main library MDF (Main Distribution Frame) Computer Room, and four IDFs (Intermediate Distribution Frame). The computer room consists of the network core Cisco Catalyst 4506, HP ProCurve external and DMZ switches, and two Dell PowerConnect 6248 iscsi switches. The four IDFs consist of mostly of Cisco Catalyst 3550s and other 3 rd party switches. Maze and Dole branch locations each have a single IDF and a single Dell PowerConnect 3448P switch. The Juniper switches, listed by physical location, quantity, and part number in the specifications section will replace all existing switches except the Main MDF Dell PowerConnect 6248 iscsi switches. The specifications are designed with future expansion in mind. Our network currently consists of about 14 subnets and 6 VLANs, names are listed in parentheses (Wifi 12, Public 21, Staff 51, Branch Routing 61, Administrative 71, iscsi 400). 4 Support Maintenance and support contracts with Juniper: Juniper J Care 24x7x4 on the core EX4500s only and J Care NBD for all other switches. Spare EX4200 on the shelf for emergencies. Installation Services Consultants will test fiber optic cables and setup, configure, install, and test new Juniper switches. Existing Cisco switches will be removed from racks and brought to the IT workroom. See specifications for a list of deliverables. Unauthorized contact regarding the RFP with other Library employees may result in disqualification. All communications should be sent by to cstacey@oppl.org by the deadline for questions specified in the RFP Schedule of Events with OPPL RFP in the subject. The Library shall respond in writing to written communications and reserves the right, at its sole discretion, to determine appropriate and adequate responses to written comments, questions, and requests for clarification. Where a clarification is necessary, the Library shall issue an electronic amendment to this RFP to all bidders who have registered with the RFP Coordinator. 8 Meeting with library IT Manager/Staff to develop and agree upon implementation plan focusing on minimal service interruption and seamless transition Presentation and acceptance of written implementation plan including dates, times, and assigned tasks of consultants for all phases Assembly, staging, setup, update, and configuration of all switches except spares Implementation and testing of switches using a phased in approach for change control Removal of existing Cisco and other 3 rd party switches/equipment to library IT workroom Training and knowledge transfer to library IT staff Proposal Submission Interested consultants must submit proposals in accordance with the terms and conditions contained herein. All proposals must be received by the final filing date and may be rejected after 10:00 a.m. CST on Tuesday, December 18, Proposals may not be delivered orally or by facsimile transmission. Please submit an electronic PDF copy of the proposal to cstacey@oppl.org. The proposal should be dated and signed by an authorized representative and must contain a statement that the proposal and prices contained in the proposal will remain firm for a period of 90 days after receipt by the Library. Costs for developing the proposals are entirely the responsibility of the applicants and shall not be reimbursed. All proposals submitted to the Library shall become the property of the Library and unsuccessful bidders should not expect to receive back their proposals. Instructions for Submitting Proposals Consultant Information 1. Proposals must include the consultant s complete name and business address, including headquarters and local (if any) office telephone number. The name, mailing address, address and telephone number of the person the Library should contact regarding the proposal. 2. Proposals must include a description of the consultant s organization, including names of principals, numbers of employees, longevity, client base, areas of specialization and expertise, and any other pertinent information that will assist in formulating an opinion about the stability and financial strength of this organization. Also, state how long the organization has been in the business of providing the requested services. 3. Each interested consultant must comply with Illinois Equal Employment Opportunity laws and certify they have a sexual harassment policy in place. 9 comply with all ordinances of the Village of Oak Park, the laws of the State of Illinois and other governmental entities having jurisdiction 4. Proposals must include a complete disclosure if the consultant has defaulted in its performance on a contract during the past five years which has led the other party to terminate the contract, and if so, the identity of the parties involved and the circumstances of the default of the termination. 5. Proposals must include a complete disclosure of any contracts which have not been completed during the past five years, regardless of whether termination was by reason of consultant s default. For any relevant contracts, list the identity of the parties involved and the circumstances which led to the contract not being completed. 6. Proposals must include a list of any lawsuits (whether civil or regulatory, and judicial or administrative in nature) filed against the consultant, its subsidiaries, parent, other corporate affiliates, or subcontractors in the past five years and the outcome of these lawsuits. Proposal Details 7. Proposals must include a brief summary of the highlights of the proposal and its overall benefits to the Library. 8. Proposals must describe the consultant s qualifications, experience, and capabilities that pertain to the RFP. 9. Proposals must include a work plan describing the steps of implementation beginning with testing our existing fiber optic cables to support 10Gb to testing of every switch installed. 10. Proposals must define how much downtime will be necessary, and describe how service interruptions will be prevented or scheduled to allow for a smooth transition. 11. Proposals must certify compliance with the specifications contained herein. 12. Proposals must provide a warranty for the performance of the installation and testing services. Pricing 13. Proposals must include a detailed schedule showing itemized costs for all hardware, support, and services. Please apply all educational, government, and other possible discounts to pricing. 14. The Oak Park Public Library as part of a municipality pays neither Federal Excise Tax nor Illinois Retailers Occupational Tax, and therefore these taxes should be excluded from the quotations. Consultant Qualifications and References 15. Proposals must include a complete list of all key personnel who will provide project management, engineering, installation services, and training. For each person on the list, the following information must be included: job title and years of employment with the consultant and in the relevant trade, role to be played in connection with the RFP Oak Park Public Library: Switching Refresh, Support, and Installation Services Page 9 of 13

10 proposal, address, vendor certifications, relevant experience, other certificates or other relevant achievements. 16. If the consultant is based outside the State of Illinois, or for any reason will not be the only provider of contracted services, any and all potential sub contracted entities, personnel, or other services must be identified in the proposal including the corporate or business entities names and addresses and/or the names, addresses and all other pertinent information of any independent contractors, and copies of all applicable certifications. All subcontractors must also provide the information described in Paragraph Proposals must include at least three references for whom you have provided Juniper switches, support, and services, indicating which services were provided to each customer. Include names, address, addresses and telephone numbers. Evaluation of Proposals All proposals must be received by the final filing date and may be rejected after 10:00 a.m. CST on Tuesday, December 18, The Library retains the right to reject any or all proposals and to waive any technicalities or irregularities in the bidding. Proposals will be opened and reviewed privately by the IT Manager with evaluation/selection/ notification to follow. Proposals will be evaluated upon these criteria (in no particular order): Experience longevity of experience in providing Information Technology solutions in the educational, academic, or library sector Quality of proposal and solution overall quality of submitted proposal, including attention to detail and professional presentation, will be taken into consideration Price the services must be provided in a cost effective manner. References positive feedback from previous clients The contract will be awarded to the proposal that, in the opinion of the Library, offers the best combination of past performance, quality of work, price, and any other factors that the Library considers pertinent, while meeting all required specifications of this RFP. The Library reserves the right to waive any technicalities in any proposal or to reject any proposal or all proposals, or any part thereof. The Library also reserves the right to request clarification of any proposal after all proposals have been received. The Library further reserves the right to interpret the criteria set forth in this RFP as it sees fit based on benefits to the public and the requirements stated in this RFP. 11 Contract Approval The RFP and selection processes do not obligate the Library and do not create rights or claims of entitlement in the apparent best evaluated applicant. Contract award shall commence only after the contract is signed by the consultant and by library officials as required by Oak Park Public Library policies to establish a legally binding contract. Consultant will fill in and use the following INDEPENDENT CONTRACTOR AGREEMENT. 12 INDEPENDENT CONTRACTOR AGREEMENT THIS AGREEMENT entered into by and between,,, Illinois, (hereinafter referred to as the "Contractor"); and the,,, IL, County, Illinois (hereinafter referred to as the " "). WHEREAS, Contractor will be performing various duties as generally described in "Exhibit A" under contract with the said entered into and to be entered into from time to time, which work will be performed on and/or off the premises of the and said Contractor may have subcontractors or one or more employees engaged in the performance of said work. NOW, THEREFORE, in consideration of the mutual covenants and agreements herein contained, and other good and valuable consideration received and to be received, the Contractor hereby agrees: 1. To comply with all applicable laws, regulations and rules promulgated by any Federal, State, County, Municipal and/or other governmental unit or regulatory body now in effect or which may be in effect during the performance of the work. Included within the scope of the laws, regulations and rules referred to in this paragraph but in no way to operate as a limitation are all forms of traffic regulations, public utility and Intrastate and Interstate Commerce Commission regulations. Workers' Compensation Laws, Prevailing Wage Laws, the Social Security Act of the Federal Government any of its titles, the Illinois Department of Human Rights, Human Rights Commission, or EEOC statutory provisions and rules and regulations. 2. To protect, indemnify, hold and save harmless and defend the against any and all claims, costs, causes, actions and expenses, including but not limited to attorney's fees incurred by reason of a lawsuit or claim for compensation arising in favor of any person, including the employees or officers or independent contractors or subcontractors of the contractor or municipality, on account of persona injuries or death, or damages to property occurring, growing out of, incident to, or resulting directly or indirectly from the performance by the "Contractor or subcontractor" hereunder, whether such loss, damage, injury or liability is contributed to by the negligence of the or by premises themselves or any equipment thereon whether latent or patent, or from other causes whatsoever, except that the Contractor shall have no liability or damages or the costs incident thereto caused by the sole negligence of the "." To keep in force, to the satisfaction of the, at all times during the performance of the work referred to above, Public Liability Insurance and Automobile Liability Insurance with Bodily Injury limits of not less than $, Property Damage Insurance with limits of not less than $ and workers' compensation and related insurance coverage at amounts required by statute.
