1 Cognizant Technology Solutions FAQs A. Web Portal Registration How do I register? Simply Visit click Member under the Secure Login box. Choose the login/register button in Members on European, Asia Pacific, Middle East, Africa, Latin American and Caribbean based plans, start here and follow the prompts. Web registration cannot be done before 4 th November 2016 B. E Cards Where can I download it? Simply register on our Aetna web portal first to create your own user name and password. Download the Aetna International App for free from Apps store and login using your user name and password. Upon login, you will be able to view your E card. Please screenshot and store it on your handphone. It is so simple! 1. E-card is available only on smart phones, not on the web portal 2. E-card is available on android and Iphone. No windows app 3. Separate login credentials / registration is required for each member above the age of 18 (ie) Please create separate login credentials 4. A physical card can be requested only at following circumstances by writing to Cognizantservice@aetna.com or calling up Aetna hotline and furnishing the address for delivery of cards a) If the member does not have a windows app / has a non smart phone b) If the member has a planned treatment in India with one of the Aetna network hospitals C. Medically History Disregarded (MHD) What does that mean to me? Cognizant s employee plan is written on MHD basis and we will cover pre-existing conditions suffered by you, subject to the benefits, terms and conditions of your plan. Benefit exclusion 1 will not apply. D. Benefits Limits Does it apply to per member or collectively? The benefit limit that is shown on the benefit schedule applies to each member individually

2 E. Pre-existing conditions & Congenital Anomaly What is the difference? In general, congenital anomaly is any genetic, physical or (bio)chemical (metabolic) defect, disease or malformation (which may be hereditary or due to an influence during gestation) and which may or may not be obvious at birth. Pre-existing condition is any medical condition which has one or more of the following characteristics: Was foreseeable Clearly showed itself You had signs / symptom of You received treatment for To the best of your knowledge, you were aware you had F. Ayurvedic Medicine Do I need referral? No referral is needed for Ayurvedic Medicine and as long as it is administered by a certified practitioner. This benefit is also applicable if you seek treatment in India. G. Co-Insurance Is there any co-pay / co-insurance applicable? No co-insurance is applicable for both outpatient and inpatient treatment. Please note that 20% co-insurance is applicable for routine pregnancy for Managers and below H. Group Leavers Option What does that mean? Employees leaving the company have the option to transfer their employee benefit program to an individual plan. Any transfer will be subjected to a minimum 30% loading on the premium of the individual plan. We will maintain your existing underwriting terms and original join date. I. I have outstanding claims and my treatment date is before 01/11/2016 Which insurer is responsible for the claims? The transition to new insurer will effectively start from 01/11/2016. Hence, any claims with treatment date after 01/11/2016 should be submitted to your selected insurer. J. Outpatient Care benefit What if I have the flu? Check on our member apps (direct settlement provider directory or International Mobile Assistant) for your preferred clinic to visit. If it is your first visit to the clinic, we recommend that you call and arrange for an appointment. Once you reached the clinic, show your electronic member card to the staff, fill in the necessary documentation and obtain cashless settlement for treatments. This is applicable for all treatments below $500. A typical visit to a clinic in Singapore costs $50 - $80 for the common flu.

3 K. Flexible benefit What can I use it for? You can select from 12 options to enjoy this benefit, namely: 1. Gym membership 2. Spas 3. Supplements 4. All sports coaching 5. Baby gym 6. Booking of sports facilities such as tennis court 7. Group classes such as swimming class 8. Durable Medical Equipment, Prosthetic and Orthotic Supplies (DMEPOS) 9. Ayurvedic treatment 10. Vaccination 11. Prescription lenses / frames / contact lenses 12. Health screening Simply go for your preferred option. Obtain an official receipt and submit a claim on our member app or member portal with a picture of the receipt. Routine health check such as regular health screening, cancer screening, cardiovascular examinations, vital sign tests can be covered under Flex benefits L. Routine and Restorative Dental benefit What can I use it for? We will cover for routine examinations, tooth cleaning, normal compound fillings and simple non-surgical extractions. In addition, we will cover repair of crowns, bridge work, dentures, removal of wisdom teeth and more. Please note that orthodontic treatment and dental implants are excluded. Check out the Benefits Schedule for a detailed description of covered items under this benefit. M. Inpatient Care benefit What if I need to have a surgery at a hospital? Please complete 2 forms. (1) Pre-Certification Medical Form will require your basic member details, input and signature from your treating physician the required medical information. (2) Release of Medical Information Form is an authorization to Aetna, so that we can reach out on your behalf to the treating physician or hospital when we need clarification or further information. Upon verifying your treatment details, we will issue a Guarantee of Payment letter (GOP) to both you and the hospital. Kindly give us 5 days notice to prepare the GOP. This will ensure that you don t have to fork out cash for the procedure and focus on your treatment. In the event of an emergency, when you have to be at the hospital for immediate treatment, please engage the hospital staff to contact us directly. If you can t speak to the hospital staff due to your medical condition, anyone who can present your member details can do so on your behalf. We will obtain the necessary information from the hospital and issue the GOP to them.

4 N. Routine Maternity benefit What should I do if my wife is pregnant? Congrats! With a pregnancy, it is common for expectant mothers to have regular pre-natal check-ups and prescribed pre natal vitamins, which is covered under Aetna. When the delivery date draws close (i.e. 2 weeks beforehand), contact us to get a Guarantee of Payment (GOP) letter. This is explained under the Inpatient Care benefit. You can bring along the GOP when your wife is due for delivery as an assurance while a copy will be issued to the provider who accepts it. Based on your co-insurance, you will have to pay a certain % of the treatment costs out of pocket. O. Employee Assistance Programme (EAP) What should I do if I feel burnout at work? You can call the EAP hotline if you feel the need to talk to someone when you feel burnout. The hotline is available 24 hours a day, 7 days a week, 365 days a year. Our professional counsellors can provide support on a wide range of work and personal issues, for example, managing stress, managing life changes, surviving the loss of a loved one etc. P. Pre-Authorization When do I need to obtain it? Planned inpatient or day patient treatment in a hospital Any pregnancy or childbirth treatment (with the exception of routine pre-natal checks) Planned surgery Home nursing charges Planned MRI, PET and CT scans Outpatient psychiatric (if covered under plan) Emergency evacuations Q. MRI, CT & PET Scans What is the benefit limit and do I need pre-authorisation? MRI, CT & PET scans are covered in full up to the annual limits of the plan and you will need to obtain an preauthorisation from us by calling the number found at the back of your card. R. Can I add a new born to my policy if I already have 2 children covered under the insurance scheme? You may delete an existing child and add a new born only if no claims are made by an existing covered child. New borns must be covered within 30 days from birth.

5 S. I used to buy insurance for my parents through Cognizant and pay for this extra cover. Can you please suggest if I can buy Insurance from Aetna in India and what would be the cost? I can provide more details if needed. The India coverage provided is only for families of associates who are hired in India and the associate is currently working abroad. In addition: The India Medical Insurance policy is applicable only if the hospitalization happens in India. This has no impact on your onsite health cover eligibility (if applicable). This policy is governed by the current terms and conditions of employment at Cognizant and is subject to change. Cognizant reserves the right to amend its policies as needed. T. How should I classify my flex claims online/on app when submitting? For online/on app claims, please choose the option Wellness for flex claims. However, the wellness and flex limits will apply.
