1 FAIRWINDS Credit Union Website Reviews Sweepstakes 2017 Q3 Official Rules Introduction: During the period beginning on Saturday, July 1, 2017 at 12:00 a.m. Eastern Standard Time ( EST ) and concluding on Saturday, September 30, 2017 at 11:59 p.m. EST ( Sweepstakes Period ), FAIRWINDS Credit Union ( Sponsor ) will sponsor the Website Reviews Sweepstakes 2017 Q3 ( Sweepstakes ). Eligibility: The Sweepstakes is open to legal U.S. residents residing within the state of Florida who are not minors at the time of Sweepstakes entry under the laws of their state of residence. Proof of eligibility will be required to claim prize. The following individuals, regardless of their ability to meet the above qualifications, do not qualify to enter or win the Sweepstakes: employees of Sponsor, immediate family members (spouses, parents, siblings, children, grandparents, grandchildren) of employees of Sponsor, employees of FAIRWINDS Insurance Services, LLC, immediate family members of employees of FAIRWINDS Insurance Services, LLC, Sponsor s Board of Directors, and immediate family members of Sponsor s Board of Directors. Prize: Prize is pre-determined and may not be substituted for another prize. Odds of winning will depend on the number of eligible entries received. Sponsor reserves the right to change prize at any time without additional notice. One (1) Grand Prize, with a suggested retail value of approximately $100, will be awarded at the conclusion of the Sweepstakes Period. How to Enter: There are two (2) methods by which you can enter the Sweepstakes. You may enter the Sweepstakes in either one of the following ways: a) Send a handwritten 3 x 5 card with your first name, last name, residential address, daytime telephone number, and address to: FAIRWINDS Credit Union Website Reviews Sweepstakes 135 W. Central Blvd., Suite 1230 Orlando, FL b) Submit a website review from an invitation sent by Sponsor during Sweepstakes Period. Reviews originally submitted before Sweepstakes Period with Sponsor do not qualify for automatic entry into Sweepstakes. Review submission will be automatically entered into Sweepstakes on date of completed and submitted review, regardless of approval or display.

2 One entry per review, per method, during the Sweepstakes Period is allowed. Multiple entries by any one individual through the same entry method may, at the sole discretion of the Sponsor, result in all of that individual s entries being disqualified. No more than one (1) entry per postcard or envelope is allowed. Any attempt to violate these Official Rules will, at the Sponsor s sole discretion, result in disqualification of any or all entries submitted that are not done so in accordance with the Official Rules. Any and all fees associated with any of the above entry methods are the sole responsibility of the individual entering the Sweepstakes. The use of review submission as method of Sweepstakes entry is highly discouraged if reviewer s only motivation to submit the review is to enter Sweepstakes. To enter Sweepstakes without submitting a review with Sponsor, use entry method (a) to mail in a 3 x 5 index card, as described above. General: No purchase necessary to enter or to win. A purchase will not increase your odds of winning. All federal, state, local, and municipal laws and regulations apply. Void where prohibited. Sponsor s decisions regarding all aspects of the Sweepstakes are final and non-negotiable, and may be changed only by Sponsor. Sponsor will not be responsible for typographical, printing, or other inadvertent errors in any materials other than the Official Rules of the Sweepstakes. All entries must be received by 11:59 p.m. EST on Saturday, September 30, 2017 to be eligible. Sponsor will not be responsible for incomplete, lost, late, misdirected, or illegible entries, or for failure to receive entries due to transmission failures or technical failures of any kind, including, without limitation, malfunctioning of any network, hardware or software, whether originating with sender or Sponsor. All entries become property of Sponsor and none will be returned. Sponsor reserves the right to disqualify any entries by persons determined to be tampering with or abusing any aspect of the Sweepstakes. The Grand Prize drawing will be held on approximately Friday, October 6, Prize is subject to change at any time, without notice, at the sole discretion of Sponsor. In the event the Sweepstakes is compromised by a virus, non-authorized human intervention, tampering or other cause beyond the reasonable control of Sponsor which corrupts or impairs the administration, security, fairness or proper operation of the Sweepstakes, Sponsor reserves the right in its sole discretion to suspend, modify, or terminate the Sweepstakes. Should the Sweepstakes be terminated prior to the stated expiration date, Sponsor reserves the right to award prize based on the entries received before the termination date. Sponsor reserves the right to disqualify persons found tampering with or otherwise abusing any aspect of this Sweepstakes as solely determined by Sponsor. Any and all taxes, fees, and other associated expenses, if any, arising from participating in Sweepstakes or claiming Sweepstakes Prize, are the sole responsibility of winners. Winners agree to hold harmless Sponsor from any and all liability, loss, or damages or causes of action arising from or in connection with entrant s participation in or winner s receipt or use of the prize awarded in the Sweepstakes. Sponsor is responsible only for the issuance of prize, not for the use, utility, quality, or otherwise. Once prize is claimed, Sponsor has no further responsibility or liability, stated or otherwise implied, to

3 Winner in regards to Sweepstakes Prize. When claiming prize, Grand Prize Winner may be required to provide a completed W-9 form to Sponsor. W-9 form is required for Sponsor to issue Grand Prize Winner a 1099-MISC form for the Fair Market Value of the prize, as required by the U.S. Government. Grand Prize Winner is required to provide Sponsor with all information necessary to issue a 1099-MISC form including, but not limited to, Winner s full legal name, Social Security number, and address. Winner Selection: Winner will be randomly selected from all eligible entries received during Sweepstakes Period. Random drawing will be conducted by a representative of FAIRWINDS Credit Union, whose decisions are final on all matters relating to the Sweepstakes. One (1) Grand Prize Winner will be selected from all entrants during Sweepstakes Period. Winner Notification and Acceptance: Winner will be notified by telephone. Sponsor will call the winner up to five (5) times that day and the following business day during regular business hours at number provided on entry form, which may vary depending on method of entry, and will leave no messages. Sponsor reserves the right to use any contact phone number provided by Sweepstakes entrant to Sponsor whether on 3 x 5 index card or in Sponsor s current member database. Sponsor is not responsible for incomplete, inaccurate, or illegible phone numbers. Failure to reach winner during these calls may result in disqualification of winner, forfeiture of his or her interest in prize, and selection of substitute winner. Winner will be required to claim prize in person, during regular business hours at the FAIRWINDS Credit Union Corporate Tower (135 W. Central Blvd., Suite 1230, Orlando, FL 32801) within fifteen (15) calendar days of being notified to claim prize. At the sole discretion of Sponsor, exception(s) may be made in the event a randomly selected winner does not live or work within twenty (20) miles of FAIRWINDS Credit Union s Corporate Tower. If an exception is made, and delivery of prize is determined to be an acceptable substitute to winner claiming prize in person, winner will be solely responsible for any and all expenses Sponsor incurs in relation to packing, shipping, or delivering prize. Winner may be required to sign an affidavit of eligibility and a liability and publicity release at the time the prize is claimed. Except as otherwise set forth in these Official Rules, failure of Winner to visit the FAIRWINDS Credit Union Corporate Tower in person and sign and return any required affidavits and releases, or to claim a prize or comply with any term or conditions of these Official Rules, will result in drawing Winner s disqualification and the forfeitures of his or her interest in the prize. Winner may waive their right to receive prize. Prize is non-assignable and nontransferable. No substitutions allowed by winner. Sponsor reserves the right to substitute prize of equal or greater value. Winner is solely responsible for reporting and payment of any taxes on prize. Except where prohibited, acceptance of any prize constitutes

4 winner s consent to the publication of his or her name, biographical information, and likeness in any media for any commercial or promotional purpose, without limitation or further compensation. Prize not claimed by eligible winner in accordance with these Official Rules will not be awarded and will remain the property of Sponsor, or at the Sponsor s discretion may be awarded to another randomly-selected Sweepstakes entrant. Participation: By participating in Sweepstakes, entrants agree to be bound by these Official Rules and the decisions of the Sponsor including publication of name, biographical information, likeness, and photos. Winner agrees to the use of their name, likeness, biographical information, and photograph in all advertising, promotions, and other publicity conducted by Sponsor. Winners List: To obtain the name of the winner, send a self-addressed, stamped envelope to: FAIRWINDS Credit Union, Website Reviews Sweepstakes, 135 W. Central Blvd., Suite 1230, Orlando, FL Requests must be received by October 31, The list will be sent after the selection of winner. Dispute Resolution: Prior to filing any arbitration, the party intending to file such a proceeding shall notify the other party in writing of the existence and the nature of the dispute. The parties each agree that within ten (10) business days of the other party s receipt of such notice, a representative of each party shall meet for a minimum of one (1) eight (8) hour day, or such shorter period of time if the dispute is more quickly resolved, in Orlando, Florida in order to attempt to amicably resolve the dispute. If such informal dispute resolution attempts prove to be unsuccessful, the notifying party may initiate the dispute resolution procedures available as set forth herein. Each party shall bear its own costs associated with the meeting and in no event shall either party be liable to the other for any costs or expenses incurred as a result of such meeting. Except as expressly set forth below, by entering this Sweepstakes, you specifically agree to waive your right to bring a court action and have a jury trial, that any controversy or claim arising out of or related to this Sweepstakes shall be resolved by final and binding arbitration conducted in accordance with the Commercial Arbitration Rules, including the Supplementary Procedures for Consumer Related Disputes, of the American Arbitration Association ( AAA ), and that there will be no class claims in arbitration. Any such controversy or claim (whether a contract, tort, statutory, or regulatory claim or claim based on any other legal theory) initiated by you or the Sponsor against the other, or against any employee, agent, or assign of the other, shall be submitted to and resolved on an individual basis by binding arbitration in accordance with the Federal Arbitration Act. The arbitration proceeding shall be conducted before one arbitrator from the AAA under the Commercial Arbitration Rules, including the Supplementary Procedures for Consumer-Related Disputes, of the AAA. 5 any such arbitration shall be held and conducted in Orlando, Florida. Any court having jurisdiction thereof may enter judgment upon any award rendered by the arbitrator. The arbitrator shall determine the prevailing party, and the costs and expenses of the arbitration proceeding, including the arbitrator s fees, shall be borne by the non-prevailing party, unless otherwise required by law or the Commercial Arbitration Rules, including the Supplementary Procedures for Consumer-Related Disputes, of the AAA. Arbitration claims shall be submitted and resolved on an individual basis, and the arbitrator will not consider or address class claims brought by or on behalf of other persons, or award punitive or any other monetary damages other than the prevailing party s actual damages. The parties acknowledge that violation of these Official Rules would cause irreparable harm that would not be adequately compensable by monetary damages. In addition to other relief, it is agreed that preliminary and permanent injunctive relief will be available to prevent any actual or threatened violation of such provisions.
