1 Date of Issue: HPA CRCE Reference: RADIATION PROTECTION ADVISER CONTRACT It is agreed that a contract exists between the Health Protection Agency, Centre for Radiation, Chemical and Environmental Hazards, Radiation Protection Division of Chilton, Didcot, Oxfordshire, OX11 ORQ (hereinafter referred to as HPA CRCE ) and CUSTOMER NAME whose registered address is REGISTERED ADDRESS (hereinafter referred to as the CUSTOMER ). The annual costs for carrying out the work specified in Part B(a) of the Schedule have been estimated and are based on the charges specified in the Appendix. Scope of Service In respect of the equipment and facilities listed in Part A, HPA CRCE will act as Radiation Protection Adviser (RPA), and will undertake the work indicated below during each 12 month period. The work will be undertaken by Address: Tel No: Contact Name: (a) Basic work: making at least two visits to the CUSTOMER to assess radiation protection standards and provide a report after each visit containing advice and recommendations; providing general advice on radiological protection, legislative requirements, training requirements etc, by telephone or letter; providing advice on immediate restorative action to ensure the safety of personnel after an accident and, where necessary undertake chromosome aberration analysis and/or whole body monitoring for personal dose assessment at no charge; carrying out statutory tests of the radiation monitor(s) listed in this schedule and provide the necessary test certificates as appropriate; carrying out statutory tests of the radioactive source(s) listed in this schedule and provide the necessary test certificates as appropriate; advising on and assisting in maintaining an "Operational File" concerning the CUSTOMER's provisions for radiological safety; providing a brief annual report summarising the work carried out by HPA CRCE. (b) Additional work can include: providing an extra visit or visits to the CUSTOMER to give advice or assistance; providing detailed advice on radiological protection, legislative requirements, etc, including evaluation of plans for new equipment and/or facilities; providing radiological advice or assistance to restore plant and/or facilities in a postaccident situation.

4 APPENDIX TO RADIATION/LASER PROTECTION ADVISER CONTRACT 2011/2012 RATES AND CHARGES (All rates are quoted exclusive of VAT) Hourly Rates The rates in the table apply to the hours spent on carrying out direct work under this contract, other than for unit priced services, including time on customer s sites, written and/or telephone advice and report writing, and the preparation and presentation time for training services.
