1 Harvey House Fee Schedule Room Use: $300 for first five (5) hours $50 an hour thereafter Catering: Is not provided by the Harvey House Rental Fee (non-refundable): $100 Cleaning Deposit: $100 Two Harvey House Attendants must be present for event. Attendant rates are $10 hr for the first five (5) hours and $15 hr thereafter. Other Items: Must perform pre-event and post event walkthrough Alcohol use must be prearranged with Harvey House Setup must be done prior to event Contact the Harvey House for scheduling Available hours: 9 am to 12 am (After midnight hours possible with prior approval) Must have event insurance Call for Availability

2 AGREEMENT FOR THE USE OF THE HARVEY HOUSE 685 North First Avenue I/We request use of the Harvey House on (date requested) Hours of Event: to Setup and decorating date and time requested: NOTE: The hours as indicated above shall be strictly observed. If it is necessary to extend the time allotted for the event, authorization must be obtained from the Harvey House Leasing Agent before the event convenes. Additional charges may be necessary. The leasing agent reserves the right to terminate this agreement if the contract is not fulfilled, less deposit. All debts and paperwork must be made to Barstow Area Chamber of Commerce and satisfied within two weeks prior to the event. YES NO I/WE, the undersigned, understand that alcohol may not be served and/or consumed on the premises unless a Certificate of Insurance is provided for Comprehensive General Liability with a minimum of $1,000,000 combined single limits for bodily injury or property damage including liquor liability and fire liability naming the City of Barstow (Harvey House) as additional insured. Liquor liability insurance can be purchased separately. Applicant will also ensure that a minimum of two (2) Uniformed Security Guards are present at all times during the event and the undersigned are responsible for the cost of special duty. In order to protect the City of Barstow (Harvey House), and the Barstow Area Chamber of Commerce in the regard, the applicant shall secure, carry and maintain at all times during the term of the agreement, public liability and property damage insurance which names the City of Barstow as an additional insured.
