If the named insured on an eligible policy chooses to reinstate the cancelled policy in lieu of purchasing a new policy, the named insured must provide a written or recorded statement representing that for the time period between the effective date and time of cancellation and the date and time on which reinstatement is requested no motor vehicle accident involving a household member, accident involving a listed vehicle, or damage to a listed vehicle has occurred. No coverage shall be provided for such accidents or damage if the policy is reinstated.

11 Progressive Direct Insurance Company State of Ohio New Business Effective: December 11, 2009 June 19, 2014 Renewals Effective: January 10, 2010 June 19, 2014 P11-Reinstatement After Cancellation for Non-payment If a policy has been canceled during its term for non-paymentnonpayment of premium, the policyeligible policies may be reinstated if all ofoffered the option to reinstate. If the following conditions are met: A. The policy is "eligible" for reinstatement. A policy is eligible for reinstatement if: 1. Reinstatement is requested by a named insured within 30 calendar days after the cancellation effective date; and 2. any at-fault accident which occurred under the policy during the prior three months, excluding any that occurred in the term ending in cancellation, are closed, and the cumulative amounts paid by the Company for all such accidents, excluding deductible amounts, total no more than $100,000; and 3. The premium is not financed through a premium finance company; and 4. No endorsements adjusting premium were processed after the policy cancellation effective date. B. The named insured on an eligible policy chooses to reinstate the cancelled policy in lieu of purchasing a new policy. C.
