1 2018/2019 Sun Prairie Outdoor Farmers' Market Vendor Registration/Application The Sun Prairie Farmers Market (SPFM) operates three markets two outdoor and one indoor. This application is for the two Outdoor Markets only. A separate application is required for the Indoor Winter Market. Please request if interested. The two outdoor markets are: Saturday Outdoor Behind City Hall, 300 East Main Street, Sun Prairie, May 5 through October 27, 7AM to Noon Tuesday Outdoor Parking lot of Saint Vincent de Paul, 1110 West Main Street, May 8 through October 30, 2:30 PM to 6 PM RETURNING VENDORS: You must submit application form, proof of auto liability insurance, copies of all applicable permits and licenses, the first day you plan to sell at the market (if different from opening market date) and vendor fee by the registration deadline of April 16, 2018 NEW VENDOR APPLICATION: You must submit application form, proof of auto liability insurance, copies of all applicable permits and licenses and vendor fee a minimum of two weeks (14 days) prior to your desired start date. Your application will be reviewed by the Market Board and you will be notified of the Board's decision by preferred communication method. VENDOR FEE SCHEDULE: A vendor may apply to participate in any of SPFM's markets as a daily or seasonal vendor. The fee is for one 12' facing vendor stall. See rules regarding additional space. Vendor fees are discounted if a full season is committed to. SEASONAL VENDOR = A VENDOR WHO PLANS TO BE AT THE MARKET EVERY SATURDAY AND/OR TUESDAY FOR THE ENTIRE SEASON Saturday Seasonal Fee = $225 Tuesday Seasonal Fee = $95 Full payment is due prior to or at the first market vendor is approved to participate in. Payment by check will not be cashed until application is approved. DAILY VENDOR = A VENDOR WHO PLANS TO BE AT THE MARKET ONLY ON SPECIFIC DATES Saturday Daily Fee = $15 Tuesday Daily Fee = $5 Aggregate fee may be paid in full prior to or on the first market day selected. Vendor may pay at the beginning of each market attended.

5 SUN PRAIRIE FARMERS MARKET HOLD HARMLESS REPRESENTATIONS AND AGREEMENTS The undersigned acknowledges that he/she has read and is familiar with the Rules governing the Sun Prairie Farmers Market (SPFM). Applicant agrees by his/her signature that the making of this application and the acceptance into SPFM are expressly conditioned upon Applicant's acceptance and continuing compliance with said rules. Applicant hereby releases, waives and discharges SPFM, its Board and Managers from any or all claims of any type or nature, damages, costs and/or expenses (including reasonable attorneys' fees) arising from or in any way connected with this application. Applicant hereby agrees to hold SPFM, the Bank of Sun Prairie, and the City of Sun Prairie harmless from and against any or all claims of any type or nature, damages, costs and/or expenses (including reasonable attorneys' fees). Applicant agrees to indemnify SPFM, the Bank of Sun Prairie and the City of Sun Prairie relating thereto, arising from or in any way connected with the use of the parking lot, or any part of facility thereof by the Applicant, his/her agents, family members, partners, associates, employees, invitees and/or from Applicant's sale of any products at SPFM. Applicant shall be responsible for compliance with all applicable state, county and city laws, regulations, ordinances. I acknowledge I have received the SPFM Rules as part of this application. I have read, understand and agree to comply with said rules.
