3 1 INTRODUCTION This Appendix A provides Bidders instructions for completing the two Model Contract(s) included as Appendix B1 for implementation and Appendix B2 for Maintenance and Support. 2 COMPONENTS OF THE MODEL CONTRACT The Model Contracts are contained in Appendix B1 and Appendix B2 and are constructed from the following items. 2.1 State of California Standard Agreement Note to Bidders: Refer to the first page of Appendix B1 and to the first page of Appendix B2 for Model Contract to be filled out and signed. 2.2 Attachment 1 Statement of Work Appendix B1: This section of the Contract includes the draft Statement of Work for the 36 month implementation phase. The SOW will be finalized to incorporate the appropriate RFP provisions from Sections of II, V, VI, and VII after contract award. Appendix B2: This section of the Contract includes the draft Statement of Work for up to five (5) years of maintenance and support plus two (2) optional one (1) year extensions. The SOW will be finalized to incorporate the appropriate provisions from the RFP. 2.3 Attachment 2 General Provisions Information Technology This section of the Contract provides the State s standard General Provisions- Information Technology (GSPD-401IT). The IT General Provisions are also available through the ADA Complaint Version: Attachment 3 IT Purchase Special Provisions This section of the Contract provides the specific Purchase Special Provisions. The ADA Compliant Version: Attachment 4 IT Maintenance Special Provisions This section of the Contract provides the specific Maintenance Special Provisions. The A-2

Appendix B2: This section of the Contract includes the Contractor s completed Cost Worksheets as follows: (Exhibits VII-F through Exhibits VII-G of this RFP). 3 INSTRUCTIONS Bidders are to complete the following sections of the Model Contract as prescribed below. 3.1 Standard Agreement Item 1: Enter the name of the Contractor. Item 2: Prepared by the State. Item 3: Leave blank. The maximum amount of the agreement will be entered by the State. Item 4: Leave unchanged. Complete the Contractor information and sign in the space provided. The person signing on behalf of the Contractor must be authorized to sign on behalf of the Contractor. A-3

5 Include the signed contract with the Final Proposal. IMPORTANT NOTE: The inclusion of costs in any fashion or format in any other place in the Proposal except for the Final Cost Proposal will result in immediate rejection of the proposal. 3.2 Statement of Work Enter contact information for the Contractor Project Director and Contractor Project Manager as specified in section B.4 of the Statement of Work.
