1 PEST CONTROL SERVICES REQUEST FOR QUOTE May 19, 2016 GENERAL INFORMATION This is an invitation to submit a quote to provide the Glynn County Georgia Board of Commissioners with Pest Control Services as indicated herein. Quotes will be received at the Purchasing Office, 1725 Reynolds St, 3 rd floor until 2:00 P.M. Tuesday, June 3, There will not be a pre-bid conference. Any questions and/or misunderstandings that may arise from this invitation should be submitted, in writing and forwarded, to the Purchasing Agent, Keri Moreland at kmoreland@glynncounty-ga.gov and copied to the Purchasing Assistant, Lauren Sobala at lsobala@glynncounty-ga.gov no later than 5 business days prior to the bid opening. It shall be the bidder s responsibility to seek clarification as early as possible prior to the time of bid opening. This policy ensures all segments of the business community have access to supplying the goods and services needed by Glynn County. GENERAL DESCRIPTION The Glynn County Board of Commissioners (hereafter referred to as Glynn County) is interested in receiving quotes to provide pest control services for approximately fifty six (56) locations of County owned facilities. The successful Bidder will be required to provide all the material, chemicals, equipment, and labor necessary to facilitate quality workmanship in his performance of this project. Statement Of Work - The successful Bidder will perform whatever tasks are necessary to clear buildings of roaches, ants, rats, mice, silverfish, and fleas. Monthly scheduled treatments will continue for the life of the contract facilitating a non-pest environment. The maintenance servicing will be accomplished so as not to interfere with the Staff's day-to-day operations. This may require servicing after normal working hours. As such, coordinating the monthly service is of utmost importance. Service Call Re-Visit - Should the occasion arise requiring the successful Bidder to re-visit a particular facility, off schedule, the unscheduled re-visit will not be cause for additional charges. QUALIFICATIONS The successful Bidder shall maintain a current Georgia Pest Control Commission license during the life of the contract including any extensions. He must furnish Glynn County proof of this license within fifteen (15) days of award notification. CONTRACTOR'S LIABILITY INSURANCE A. Liability. The Contractor shall maintain such insurance as will protect him from claims under workmen's compensation acts and from any other claims for damages to property, and for personal injury, including death, which may arise from operations under this contract, whether such operations be by himself or by any sub-contractor or anyone directly or indirectly employed by either of them. Certificates of Insurance indicating that the successful proposer has obtained such coverage, shall be filed with the Owner prior to the commencement by the successful proposer of the services. Such certificates shall be in form and substance reasonably acceptable to the Owner, shall indicate that, except in respect to workers compensation insurance coverage and professional errors and omissions, Owner is an additional insured with respect to such coverage, and shall indicate that such coverage is primary and not contributory with any similar insurance purchased by the Owner. The certificates shall contain a provision that the insurer will endeavor, if allowed by the policy, to provide Owner with thirty (30) calendar days notice of nonrenewal, cancellation, or termination of the coverage. If the successful

2 proposer receives a nonrenewal. cancellation, or termination notice from an insurance carrier affording coverage required herein, the successful proposer agrees to notify Owner by fax within two (2) business days with a copy of the nonrenewal, cancellation, or termination notice, or written specifications as to which coverage is no longer in compliance. Failure to comply with any of the provisions relating to insurance coverage herein shall be deemed a material breach if not cured. Certificates of such insurance shall be filed with the Owner. The contractor shall be responsible for providing adequate limits of insurance when working within property owned by railroads, as established by such railroad company. B. Indemnity. To the fullest extent permitted by laws, statutes, rules and regulations, the Contractor shall indemnify and hold harmless the County, Engineer, Engineer s Consultants and the Officers, Directors, Employees, Agents, and other Consultants of each and any of them from and against claims, costs, damages, losses, and expenses, including but not limited to all fees and charges of engineers, architects, attorneys and other professionals and all court costs, arising out of or resulting from performance of the work, but only to the extent caused in whole or in part by negligent, reckless, willful and wanton, or wrongful acts or omissions of the Contractor, its Officers, Directors, Employees, Agents, and anyone directly, or indirectly employed by them or anyone for whose acts they may be liable, regardless of whether or not such claim, cost, damage, loss, or expense is caused in part by a party indemnified hereunder, except that no party shall indemnify any other party or person for their own sole negligence. Such obligation shall not be construed to negate, abridge or reduce other rights or obligations of indemnity which would otherwise exist as to a party or person described in this Paragraph. Comprehensive General Liability -The successful Bidder shall exercise proper precaution at all times for the protection of persons and property. He shall carry approved insurance from insurance companies authorized to do business in Georgia and having an A.M. Best s rating of B+ or better with the following minimums: *The limits of insurance are as follows: CONTRACT PERIODS general liability insurance of at least One Million (1,000,000) Dollars (Combined Single Limit per occurrence) and Two Million (2,000,000) Dollars aggregate; automobile insurance of at least One Million (1,000,000) Dollars (Combined Single Limit per accident for bodily injury or property damage); and Workers Compensation Insurance as will protect potential bidder or offeror from Workers Compensation Acts. Base Year - The base year of this contract, if awarded, will commence upon the receipt of Purchase Order and continue through December 31, First Option Year - The calendar period commencing on January 1, 2017 thru December 31, Second Option Year - The calendar period commencing on January 1, 2018 thru December 31, Third Option Year - The calendar period commencing on January 1, 2019 thru December 31, 2019.

3 AWARD OF CONTRACT The contract will be awarded to that responsible bidder whose quote, conforming to the invitation for bid will be most advantageous to the County; price and other factors considered. Glynn County reserves the right to reject any or all bids and to accept any single item or combination of items in the bid, and to waive informalities and minor irregularities in all bids received. A written award, ed (or otherwise furnished) to a successful bidder within sixty (60) days from date of bid opening shall be deemed to result in a binding contract without further action by either party. Exercising of Option Periods - Thirty (30) days prior to the expiration a contract period, Glynn County will evaluate the County's future needs and determine the necessity for continuing these services. Glynn County's decision to exercise an option period will be provided to the successful bidder, in writing. CONTRACT ADMINISTRATION The Contract Administrator for this invitation and any contract(s) or purchase order(s) or award arising as a result of this invitation to bid shall be Keri Moreland (912) All questions concerning this invitation and all questions arising during the contract period(s) are to be addressed to the Contract Administrator at the following address: CONTRACT TECHNICAL REPRESENTATIVE The Contract Technical Representative is the County's day-to-day manager of the services contracted for. He shall provide the successful Proposer direction and monitor the results within the limits of the contract's terms and conditions. He will decide questions which may arise as to quality and acceptability of services performed. He shall judge as to the accuracy of quantities submitted by the successful Proposer in payment requests and the acceptability of the services which these quantities represent. He will be the point-of-contact for developing contract changes and amendments to be approved by the County and executed by the Contract Administrator. Any technical questions arising, subsequent to contract award, are to be addressed to the Contract Technical Representative at the following address: SITE INSPECTION It shall be the bidder s responsibility to complete a site inspection at their convenience.

4 QUOTE FORM PEST CONTROL SERVICES GLYNN COUNTY BOARD OF COMMISSIONERS 1. The undersigned proposes and agrees, if this quote is accepted, to enter into an agreement with Owner to perform and furnish all Work as specified or indicated in the Contract Documents for the Quoted Price and within the Bid Times indicated in this invitation and in accordance with the other terms and conditions of the Contract Documents. 2. In submitting this Quote, Proposer represents, as more fully set forth in the Agreement, that: (a) (b) Proposer has examined and carefully studied the Plans (if any), Specifications for the work, Deliverables, and has read all Technical Provisions, Supplementary Conditions, and General Conditions, furnished prior to the opening of Bids; that Proposer has satisfied himself relative to the work to be performed. Proposer is aware of the general nature of Work to be performed by Owner and others at the site that relates to Work for which this Bid is submitted as indicated in the Bid Documents. Proposer has correlated the information known to Proposer, information and observations obtained from visits to the site, reports and drawings identified in the Bid Documents and all additional examinations, investigations, explorations, tests, studies and data with the Bid documents. This Bid is genuine and not made in the interest of or on behalf of any undisclosed person, firm or corporation and is not submitted in conformity with any agreement or rules of any group, association, organization or corporation; Proposer has not directly or indirectly induced or solicited any other Proposer to submit a false or sham Bid; Proposer has not solicited or induced any person, firm or corporation to refrain from Bid; and Proposer has not sought by collusion to obtain for itself any advantage over any other Proposer or over the Owner.

12 DRUG FREE WORK PLACE CERTIFICATION In order to have a drug- free workplace, a business shall: Publish a statement notifying employees that the unlawful, manufacture, distribution, dispensing, possession, or use of controlled substances is prohibited in the workplace and specifying the actions that shall be taken against employees for violation of such prohibition. Inform employees about the dangers of drug abuse in the workplace, the business s policy of maintaining a drug-free workplace, any available drug counseling, rehabilitation, and employee assistance programs, and the penalties that may be imposed upon employees for drug abuse violations. As a condition of working on the commodities or contractual services then under Bid, the employee shall notify the employer of any conviction of, or plea of guilty or nolo contendere to, any violation of Chapter 893 or of any controlled substance law of the United States or any State, for a violation occurring in the workplace no later than five (5) days after such conviction. Impose a sanction on, or require satisfactory participation in a drug abuse assistance or rehabilitation program if such in available in the employee s community, by any employee who is so convicted. Make a good faith effort to continue to maintain a drug-free workplace through implementation of this section.
