1 LOCATION: REQUEST FOR QUOTE FOR SURFACE WATER, GROUNDWATER, AND METHANE MONITORING SERVICES AND LANDFILL COVER INSPECTION AT THE CATE ROAD LANDFILL FOR GLYNN COUNTY BOARD OF COMMISSIONERS (Inactive) Cate Road Landfill, located north of the City of Brunswick, GA, approximately 2.5 miles north of the intersection between 1-95 and State Road 25. DESCRIPTION: The Glynn County Board of Commissioners (hereafter referred to as BOC) is interested in receiving quotations for providing field sampling services and laboratory analysis associated with surface water, groundwater and methane monitoring at its inactive Cate Road Landfill; as well as post-closure inspections of the Cate Road Landfill as required by the EPD-approved Post-Closure Care Plans and in accordance with the Georgia Rules for Solid Waste Management Section and 40 CFR This landfill is closed, but requires the continuation of monitoring services as required by the Georgia Environmental Protection Division. See Attached Site Map For General Layout Of Landfill. DETAILED SPECIFICATIONS ARE LISTED BELOW: FIELD SAMPLING SERVICES & LABORATORY ANALYSIS Documents Governing Testing: All testing of ground and surface water shall be conducted as per the approved Groundwater Monitoring Plan, the Georgia Department of Natural Resources Rules for Solid Waste Management (the Rules), the Georgia Environmental Protection Division Manual for Groundwater Monitoring and the Georgia Environmental Protection Division Manual for Surface Water Monitoring. Laboratory detection limits shall not exceed established Maximum Contaminant Level (MCL's) in 40 CFR part 141. All test results must be submitted in duplicate and within the time specified in the Rules. In each report, the successful Bidder will certify compliance with established water quality standards. Should conflict arise between the aforementioned documents, the approved Groundwater Monitoring Plan shall prevail. Sample Collection: The successful Responder will collect and analyze samples from the existing groundwater monitoring wells and the surface water monitoring stations twice per year as outlined in the approved Water Sampling and Analysis Plan. Sufficient staff and equipment will be provided so that each sampling event is completed in the shortest practical period. In no event shall the sampling period be more than two (2) consecutive workdays. The successful Responder will be responsible for providing containers to hold purged water until disposal determination is made (purge water from clean samples can be discharged on site). The wells are 2 diameter. The depth to top of water is 10 to 20 and the water depth is 10 to 20. The first half sampling and reporting shall be performed in August of each contract year as follows: 24 wells (22 on site, 2 off site) and one trip blank for Appendix I VOC and metal constituents. 1 well for Appendix II list of parameters. 1 field blank for Appendix II list of parameters. 2 surface water samples for Landfill Surface Water Parameters. The second half sampling and reporting shall be performed in February of each contract year as follows:

2 25 wells (23 on site and 2 off site) and one trip blank for Appendix I VOC and metal constituents. 1 field blank for Appendix I VOC and metal constituents. 2 surface water samples for Landfill Surface Water Parameters. Methane: Determine percent methane and percent oxygen at eleven bar punch locations and one structure on site in accordance with GEPD regulations. Sampling and reporting shall be performed quarterly in August, November, February and May of each contract year. Use of Bailers: Currently, none of the wells have dedicated well purge and sampling pumps installed. Therefore, the successful Responder should base his quote on purging and sampling the wells with either dedicated bailers or disposable bailers. Each well will have a separate bailer to minimize the potential of cross-contamination of well samples. Surface Water Sampling: Two, 2" in diameter, surface water samples will be collected. Sampling will be performed twice per year as part of the groundwater sampling event. Sample handling, storage, and shipping will be consistent with the requirements for groundwater samples. The surface water samples are to be analyzed for the constituents outlined in the approved plan. Filtration: The threshold point at which samples are to be filtered for dissolved metals fraction is the 5 NTU Georgia limit. Filtration requirements pertain to only groundwater wells. The surface water samples require dissolved metal fraction only. For each groundwater well showing turbidity levels above 5 NTU after a sufficient settling period, an additional sample shall be obtained and filtered for dissolved metals analysis. The filtered sample should be submitted to the laboratory and held pending results from the total metals analyses. The dissolved sample will be tested for those parameters detected in the total metals analyses at concentrations in excess of the MCL or deemed to be a Statically significant increase over background concentrations in previous events. Settling Period: In an effort to minimize turbidity effort on metals concentrations, samplers will allow hours between well purging and collection of metals samples. It is noted that VOCs must be collected immediately following purging. Report Deliverables: The successful Responder will submit, in duplicate, a report of surface water sampling and groundwater sampling results to the below address, within thirty (30) days of the conclusion of the field sampling. Methane test reports will be due within 15 days after field sampling. The report will contain, at a minimum, a cover letter attesting that the requirements of these specifications and other relevant standards have been met, a summary sheet showing the results for individual wells or locations where parameters were detected, SWM-19 Report Form (latest revision), copies of the laboratory analytical data result sheets for every sample tested (including all blanks, duplicates and internal quality control samples), all sample chain-of-custody forms and statistical analysis, an updated potentiometric surface map signed and sealed by a qualified groundwater scientist, and any other information required by Georgia EPD. Additionally, a copy of all reports shall be sent directly to the Environmental Protection Division of the Georgia Department of Natural Resources. Glynn County Engineering Department Attention: Paul Andrews 1725 Reynolds Street, Suite 200 Brunswick, Ga Phone: (912) pandrews@glynncounty-ga.gov QUARTERLY LANDFILL COVER INSPECTION Each inspection will assess the following aspects of the landfill property: access roads, cap vegetation, final cover system, storm water conveyance systems, sedimentation basins and structures, monitoring points and

3 wells, signage, and fencing and other security measures. A site-specific post-closure landfill inspection form should be used to document inspection deficiencies noted in the field, such as: Cover erosion Patches of dead vegetation Animal burrows Subsidence Cracks in the cover Drainage ditch blockages Damage to drainage structures Damage to perimeter fencing Damage to groundwater monitoring wells Photographs should also be taken as needed to better convey inspection deficiencies. An overall site map will be provided by the County and should be utilized during the quarterly inspections to illustrate areas where inspection deficiencies were noted. A quarterly inspection report shall be prepared following each event. This report should document the inspection and its findings, as well as make recommendations for site maintenance and repairs. Each quarterly report should include, as attachments, the inspection form, site map, and any photographs that were taken as part of the inspection. All inspection reports must be suitable for inclusion in the landfill operating record. Landfill inspection reports are not required to be submitted to the Georgia Environmental Protection Division. EXPERIENCE: The successful Responder should have both a qualified and experienced laboratory group and field services group available to perform scheduled methane, surface water and groundwater sampling. Documentation of experience in other projects with contact names and telephone numbers where environmental monitoring of the type described herein is performed may be requested. Additionally, certification and accreditation of laboratory personnel and the laboratory, such as state and federal drinking water certification that demonstrates compliance with EPA standard methods and procedures will be requested prior to award. Failure to provide this documentation will result in award to another firm that can provide the requested documentation. PURCHASE ORDER PERIODS: Base Year - July 1, 2018 through June 30, 2019 First Option Year July 1, 2019 through June 30, 2020 Second Option Year July 1, 2020 through June 30, 2021 Third Option Year July 1, 2021 through June 30, 2022 Fourth Option Year July 1, 2022 through June 30, 2023 Glynn County will evaluate the County's future needs and determine the necessity for continuing these services no later than May 30 of each fiscal year.

4 AWARD OF PURCHASE ORDER: A Purchase Order will be awarded to that responsible Responder whose quotation, conforming to the request for quotation will be most advantageous to the County; price and other factors considered. The BOC however, reserves the right to reject any or all quotations and to accept any single item or combination of items in the quotation, and to waive informalities and minor irregularities in all quotations received. The purchase order shall be deemed to result in a binding contract without further action by either party. CONTRACT ADMINISTRATION: The Contract Administrator for this request and any contract(s) arising as a result of this request shall be Necie Carroll, (912) All questions concerning this request and all questions arising during the contract period(s) are to be addressed to the Contract Administrator at the following address: INSURANCE: Glynn County Purchasing Attention: Necie Carroll 1725 Reynolds St., Suite 300 Brunswick, GA Phone: (912) rcarroll@glynncounty-ga.gov The successful Responder shall maintain such insurance as will protect him from claims under workmen's compensation acts and from any other claims for damages to property, and for personal injury, including death, which may arise from operations under this contract, whether such operations be by himself or by any sub-contractor or anyone directly or indirectly employed by either of them. Glynn County Board of Commissioners shall be named as Certificate Holder and the Primary named insured along with the successful Responder. The cost of this insurance is to be included in the Request for Quote response. A copy of the CERTIFICATE (S) OF INSURANCE shall be submitted with the response. *The limits of insurance are as follows: a) general liability insurance of at least One Million (1,000,000) Dollars (Combined Single Limit per occurrence) and Two Million (2,000,000) Dollars aggregate; b) automobile insurance of at least One Million (1,000,000) Dollars (Combined Single Limit per accident for bodily injury or property damage); and c) Workers Compensation Insurance as will protect potential bidder or offerer from Workers Compensation Acts. PAYMENT ARRANGMENT: The amount due shall be paid within thirty (30) days after services are rendered and receipt of a correct invoice. An invoice will be considered correct when detailing the services performed, date of services, and accepted firm fixed price. This payment schedule will remain in force unless a shorter time is stated in the quotation and is accepted by the BOC. Invoice Address - Invoices will be sent to the Glynn County Engineering Department, listed above. Discounts - Prompt payment discounts offered for a period of less than fifteen (15) days will not be considered in determining low quotation. However, such discounts, when offered, will be taken provided payment is made within the time specified. Time, in connection with discounts for prompt

5 payment, will be computed from the date of final acceptance of all services for which payment is claimed, or the date a correct invoice is received by the County; whichever is later. Failure of Payments: It is expressly agreed that if the County should fail to pay the successful Responder, through no fault of the Responder, within thirty (30) days after the receipt of an acceptable invoice, the Responder shall receive interest on the balance due. The rate of interest will be one percent (1%) per month not to exceed three (3) months (3%). Georgia Prompt Pay Act Exclusion: All parties expressly agree that the provisions of the Georgia Prompt Pay Act, Title 13, Chapter 11, of the Official Code of Georgia Annotated, are superseded by the terms and condition of this solicitation. REPRESENTATION: This Request for Quote is submitted to Glynn County, Georgia Board of Commissioners (County) by the representative who is an authorized officer of the company and said company is licensed to do business in Georgia and Glynn County. Further, the representative is authorized to make these depictions and certifies these representations are valid. The Responder recognizes that all representations herein are binding on the Company and failure to adhere to any of these commitments, at the County's option, may result in a revocation of the granted Purchase Order. Consent is hereby given to the County to contact any person or organization in order to make inquiries into legal, character, technical, financial, and other qualifications of the Proposer. The Responder understands that, at such time as the County decides to review this Request for Quote, additional information may be requested. Failure to supply any requested for information within a reasonable time may result in the rejection of the Proposer's proposal with no re-submittal rights. The successful Responder understands that the County, after considering the legal, financial, technical, and character qualifications of the Responder, as well as what in the County's judgment may best serve the public interest of its citizens and employees, may grant a Purchase Order. The successful Responder understands that this Request for Quote is made without prior understanding, agreement, or connection with any corporation, firm or person submitting a proposal for the same, and is in all respects fair and without collusion or fraud. I understand that collusive bidding is a violation of state and federal law and can result in fines, prison sentences, and civil damage awards. The signature and notary placed on the bid price form certifies all the above is understood by the Responder of this Request for Quote.

7 CONTRACTOR AFFIDAVIT AND AGREEMENT By executing this affidavit, the undersigned contractor verifies its compliance with O.C.G.A , stating affirmatively that the individual, firm, or corporation which is contracting through a Purchase Order with Glynn County has registered with and is participating in a federal work authorization program [Employment Eligibility Verification (EEV) / Basic Pilot Program, operated by the U.S. Citizens and Immigration Services Bureau of the U. S. Department of Homeland Security, in conjunction with the Social Security Administration (SSA)] in accordance with O.C.G.A Further, the undersigned contractor states affirmatively that the individual, firm, or corporation contracting with Glynn County will continue to utilize and participate in the EEV federal work authorization program throughout the term of this contract. The undersigned further agrees that, should it employ or contract with any subcontractor(s) in connection with the physical performance of services pursuant to this contract with Glynn County, contractor will secure from such subcontractor(s) similar verification of compliance with O.C.G.A on the Subcontractor Affidavit provided in Rule or a substantially similar form provided by Glynn County. Contractor further agrees to maintain records of such compliance and provide a copy of each such verification to Glynn County at the time the subcontractor(s) is retained to perform such service.
