1 THE CITY OF CHARLOTTE AVIATION DEPARTMENT CHARLOTTE DOUGLAS INTERNATIONAL AIRPORT 5601 WILKINSON BOULEVARD CHARLOTTE, NORTH CAROLINA (704) Date: November 16, 2012 Bid Number: Subject: Invitation to pre-qualify to Bid on the following apparatus, supplies, materials, equipment and/or services for: LANDSCAPE SERVICES ZONE 1 This letter extends an invitation for the submission of pre-qualifying documentation for an upcoming bid to supply the Charlotte Douglas International Airport with apparatus, supplies, materials, equipment and/or services as indicated above. In order to participate in the Landscape Services - Zone 1 upcoming bid, all interested parties must pre-qualify by submitting the requested information by 10:00 a.m., Friday, November 30 th. Landscape Services - Zone 1 - Includes all areas of the Main terminal complex, related parking and Josh Birmingham Parkway from Billy Graham Parkway to the terminal entrance, CLT Main building, Valet Parking deck and adjacent slope, Harlee Ramp, Wilson Air and Wilson Air North, slope behind Wilson Air, Airport Drive to Wilson Avenue entrance and all associated properties at Wilson, and Old Dowd stretch at the fuel farm. Please refer to the attached map which highlights the intended area. How to Submit Pre-Qualification Responses: All Bidders shall: (A) (B) Submit pre-qualification response in a sealed opaque envelope with the following information written on the outside of that envelope: Bidder s company name, Bid Number as indicated above and Bid Name: PRE-QUALIFICATIONS - LANDSCAPE SERVICES ZONE 1 Mail or Deliver one (1) unbound original Bidder Assessment and supporting documentation. The "original" shall be complete and unabridged, and shall not refer to any other copy of the signed/sealed original for any references, clarifications, or additional information. When received, all bids and supporting materials, as well as correspondences, shall become the property of the City of Charlotte Aviation Department. 2 Bidder Pre-Qualification Assessment Bidder Name: Vendor Responses How long has your company been providing landscape services? A link to your website can also be included, if available. What does your company s current list of landscape services include. Copy of Business License issued by the State of NC. Copy of current certificate of insurance as evidence to meet Bid requirements. (Zone 1 - Insurance Requirements attached) Copy of NC Landscape Contractors Association license. Copy of valid herbicide and pesticide licenses from State of N.C. for the company. (Issued by the NC Department of Agriculture and bears classification and license #); Company and Supervisor must be licensed seperately for chemical application. Copy of valid herbicide and pesticide license from State of N.C. for on-site crew lead or supervisor; (Issued by the NC Department of Agriculture and bears classification and license #); Company and Supervisor must be licensed seperately for chemical application. Business License requested Certificate of Insurance requested NC Landscape Contractors Association license requested Company herbicide and pesticide license requested On-site crew lead or supervisor herbicide and pesticide license requested Copy of certificate or degree as proof of certification in either turf or ornamental maintenance from an approved agency; Person holding the certificate/degree must be assigned to and on the site whenever work occurs. (See attached listing for examples of certification types). Must provide a description of the anticipated size and structure of the crew to be assigned to the site. For example, separate chemical, mowing, and shearing crews. If seperate crews, the supervisor for each must be accredited for that type of work, or a general supervisor on site when work takes place. (Zone 1 is approximately 130 acres). Provide four (4) commercial references of similar size where work has been performed for longer than two (2) years; Must prove commercial work for two years or greater; Must list at least two properties 60 acres or greater. Airport must be able to contact references and verify work. 3 Examples of Certificate/Degree Types Certified Turfgrass Professional Certified Plant Professional TurfGrass Certifications or Accreditation Associate in Applied Science in Turfgrass Management Diploma in Turfgrass Management Technology Certification Turfgrass Management Certification Turfgrass Equipment Maintenance I Certification Turfgrass Equipment Maintenance II Certified Landscape Technician - CLT Ornamental Maintenance Softscape Installation Turf Maintenance Turfgrass Council of NC NC Nursery and Landscape Association Central Piedmont Community College PLANET *Other certifications within the green industry that are issued by a reliable agency such as NC State, CPCC, PLANET, or similar agencies that are either national or state based can also be submitted for approval.
4 Anticipated Size and Structure Bidder Name: Must provide a description of the anticipated size and structure of the crew to be assigned to the site. For example, separate chemical, mowing, and shearing crews. If seperate crews, the supervisor for each must be accredited for that type of work, or a general supervisor on site when work takes place; (Zone 1 is approximately 130 acres).
5 Commercial References Bidder Name: Provide four (4) commercial references of similar size where work has been performed for longer than two (2) years; Must prove commercial work for two years or greater; Must list at least two properties 60 acres or greater. Airport must be able to contact references and verify work.
6 Insurance Requirements Insurance: Throughout the term of the Contract, the successful Bidder shall comply with the insurance requirements described in this Form. In the event the successful Bidder fails to procure and maintain each type of insurance required by this Form, or in the event the successful Bidder fails to provide the City with the required certificates of insurance, the City shall be entitled to terminate the Contract immediately upon written notice to the successful Bidder. The successful Bidder agrees to purchase and maintain the following insurance coverage during the life of the Contract with an insurance company acceptable to the City of Charlotte, authorized to do business in the State of North Carolina: A). Automobile Liability: Bodily injury and property damage liability covering all owned, non-owned, and hired automobiles for limits of not less than $1,000,000 bodily injury each person, each accident; and, $1,000,000 property damage, or $1,000,000 combined single limit each occurrence/aggregate. B). Commercial General Liability: Bodily injury and property damage liability as shall protect the successful Bidder and any subcontractor performing work under the Contract from claims of bodily injury or property damage which arise from performance of the Contract, whether such work is performed by the successful Bidder, any subcontractor or anyone directly or indirectly employed by either. The amounts of such insurance shall not be less than $1,000,000 bodily injury each occurrence/aggregate and $1,000,000 property damage each occurrence/aggregate or $1,000,000 bodily injury and property damage combined single limits each occurrence/aggregate. This insurance shall include coverage for products, services, completed operations, personal injury liability and contractual liability assumed under the indemnity provision of the Contract. C). Workers Compensation: Meeting the statutory requirements of the State of North Carolina and Employers Liability - $100,000 per accident limit, $500,000 disease per policy limit, $100,000 disease each employee limit, providing coverage for employees and owners. The City shall be named as additional insured under the commercial general liability insurance for operations or services rendered under this Contract. The Company s insurance shall be primary of any self-funding and/or insurance otherwise carried by the City for all loss or damages arising from the Consultant s operations under this agreement. The Company and each of its subcontractors shall and does waive all rights of subrogation against the City and each of the Indemnitees. The successful Bidder shall not commence any work in connection with the Contract until it has obtained all of the types of insurance set forth in this Form, and such insurance has been approved by the City. The successful Bidder shall not allow any subcontractor to commence work on its subcontract until all similar insurance required of the subcontractor has been obtained and approved. All insurance policies shall be with insurers qualified and doing business in North Carolina recognized by the Secretary of State and the Insurance Commissioner s Office. The successful Bidder shall furnish the City with proof of insurance coverage by certificates of insurance accompanying the Contract and shall name the City as an additional named insured under the commercial general liability. Certificates of all required insurance shall contain the provision that the City will be given (30) days written notice of any intent to amend or terminate by either the insured or the insuring company. All insurance certificates must include the City of Charlotte s contract number in the description field. The City shall be exempt from, and in no way liable for any sums of money that may represent a deductible in any insurance policy. The payment of such deductible shall be the sole responsibility of the successful Bidder and/or subcontractor providing such insurance.
