5 Form No UTILio4 FDOT s ontrt y notifying the FDOT in writing within five 5 dys from the dte tht the UAO is notified of the id mount Unless this eletion is mde the Utility Work shll e performed s prt of the Projet y the FDOT s ontrtor d e f g h If the UAO elets to remove the Utility Work from the FDOT s ontrt in ordne with Suprgrph 2 the UAO shll perform the Utility Work seprtely pursunt to the terms nd onditions of the FDOT s stndrd relotion greement the terms nd onditions of whih re inorported herein for tht purpose y this referene nd in ordne with the ontingeny relotion shedule whih is prt of the Plns Pkge The UAO shll proeed immeditely with the Utility Work so s to use no dely to the FDOT or the FDOT s ontrtor in onstruting the Projet The UAO shll perform ll engineering inspetion testing nd monitoring of the Utility Work to insure tht it is properly performed in ordne with the Plns Pkge exept for the following tivities n nd will furnish the FDOT with dily diry reords showing pproved quntities nd mounts for weekly monthly nd finl estimtes in ordne with the formt required y FDOT proedures Exept for the inspetion testing monitoring nd reporting to e performed y the UAO in ordne with Suprgrph 2 e the FDOT will perform ll ontrt dministrtion for its onstrution ontrt The UAO shll fully ooperte with the FDOT nd the FDOT s ontrtor in ll mtters relting to the performne of the Utility Work The FDOT s engineer hs full uthority over the Projet nd the UAO shll e responsile for oordinting nd ooperting with the FDOT s engineer In so doing the UAO shll mke suh djustments nd hnges in the Plns Pkge s the FDOT s engineer shll determine re neessry for the proseution of the Projet 3 Cost of Utility Work The UAO shll not mke ny hnges to the Plns Pkge fter the dte on whih the FDOT s ontrt douments re miled to Tllhssee for dvertisement of the Projet unless those hnges fll within the tegories of hnges whih re llowed y supplementl greement to the FDOT s ontrt pursunt to Setion Florid Sttutes All hnges regrdless of the nture of the hnge or the timing of the hnge shll e sujet to the prior pprovl of the FDOT The UAO shll e responsile for ll osts of the Utility Work nd ll osts ssoited with ny djustments or hnges to the Utility Work determined y the FDOT s engineer to e neessry inluding ut not limited to the ost of hnging the Plns Pkge nd the inrese in the ost of performing the Utility Work unless the djustments or hnges re neessitted y n error or omission of the FDOT The UAO shll not e responsile for the ost of delys used y suh djustments or hnges unless they re ttriutle to the UAO pursunt to Suprgrph 4 The initil estimte of the ost of the Utility Work is8000 At suh time s the FDOT prepres its offiil estimte the FDOT shll notify the UAO of the mount of the offiil estimte for the Utility Work Upon eing notified of the offiil estimte the UAO shll hve five 5 working dys within whih to ept the offiil estimte for purposes of mking deposits nd for determining ny possile ontriution on the prt of the FDOT to the ost of the Utility Work or to elet to hve the Utility Work removed from the FDOT s ontrt nd performed seprtely pursunt to the terms nd onditions set forth in Suprgrph 2 d hereof At lest thirty 30 lendr dys prior to the dte on whih the FDOT dvertises the Projet for ids the UAO will py to the FDOT n mount equl to the FDOT s offiil estimte plus 2 for dministrtive osts of field work tultion of quntities Finl Estimte proessing nd Projet ounting sid mounts re to e hereinfter olletively referred to s the Allownes plus 10 Pge 3of9
