1 SCHOOL DISTRICT TAX INFORMATION FORM Department of Community & Economic Development Governor's Center for Local Government Services Commonwealth Keystone Building 400 North Street, 4th Floor Harrisburg, PA ph: I fax: municipalstatistics@state.pa.us Section 351 of the payer Relief Act, the act of June 27, 2006, Special Session 1, P.L. 1873, No.1, requires each taxing body to submit tax information to the Department of Community and Economic Development (DCED). Listed below are the effective tax rates (e.g. the school district's portion of Act 511 taxes which are shared with an affiliated municipality) and tax collectors currently on file with DCED for your school district. Please review this information and confirm it is correct. If the board of school directors has enacted, repealed or changed any tax rates, please indicate the change in the column provided for new or corrected information. There is also space provided for changes to tax collector information. Please complete all pages for each municipality within your school district. The tax information gathered via this form is used to update the EIT/PIT/LST Register (Official Register) which can be found on the Internet at: Employers are only required by law to withhold rates reflected on the Official Register. For newly enacted taxes or rate changes, please mail or fax a copy of the certified resolution to DCED at the address listed above. Before the Register can be updated, DCED must receive this form as well as a copy of the resolution or election results. Also, if DCED does not receive a Information Form from your school district by the June 1 deadline, the law allows for the rates of the previous year to remain in effect on the Official Register. However, DCED will update rates received after the June 1 deadline on the Real-time Register. Employers may, but are not required to, withhold rates shown in the Real-time Register.
