Trillium is a Crdinated Care Organizatin that prvides Oregn Health Plan (Medicaid) benefits fr ver 90,000 residents f Lane Cunty and the Reedsprt cmmunity f Duglas Cunty. We partner with lcal rganizatins t prvide integrated primary and behaviral healthcare fcused n addressing the scial determinants f health t remve barriers and imprve health utcmes. Wh can apply? An agency r partnership may apply fr 2018 and/r 2019 funding. Prpsals fr 2018 funding are due Octber 31, 2018. Prpsals fr 2019 funding may be submitted at any time between January 1 and July 31, 2019 r until the available funding is exhausted, whichever is earlier.
What are the funding amunts? Funding amunts fr 2018 range frm a minimum f $50,000 t a maximum f $600,000. Funding cannt be carried ver int 2019.
